3 Instructions for the trainee Definitions of terms Abbreviations Your answers to the assessment tasks need to show the assessor that you have a full understanding of the topic. The assessor may require you to discuss your responses. This assessment is open book, which means that you can use any information you wish when you are completing this assessment. This information may include learning materials, books, the internet and your organisation s policies and procedures. Your answer is portfolio evidence of practical tasks. You will collect copies of documents which you have used or have written, from a recent actual situation where you have supported a person. The word trainee in this assessment refers to the person being assessed. Other terms that may be used are candidate, student or employee. Risk is the chance of injury, damage, loss, or hazard to a person or organisation. Risk can be to an individual s identity or physical, emotional, or cultural well-being, or to an organisation s reputation, viability, or resource base. Risk management is the process of identifying, assessing, and controlling, mitigating, or transferring risks arising from organisational, social, and environmental factors. It includes making decisions that balance potential risks with the potential benefits that could arise from accepting a risk. ER refers to the evidence requirements of the unit standard the evidence (descriptions, explanations, documents etc) that you must provide and/or the actions that you must do or demonstrate. MER means more evidence required. References Notes Your organisation s policies and procedures. This unit standard cannot be assessed against in a simulated environment. This assessment must be carried out in the workplace. For any documents you supply, remove all identifying details of the people receiving services or support. You need to show that your work complies with your organisation's policies and procedures. If you require assistance with any aspect of the assessment, please contact your assessor. An organisation s policies and procedures are the policies and procedures of the trainee s employer and include ethical codes, standards and other organisational requirements. Workplace assessor or assessor means the person who will assess the trainee. A verifier is a workplace supervisor or manager who understands the assessment, works closely with the trainee, and can confirm that the trainee has performed the task competently. For assessment, trainees practice must reflect appropriate values, processes, and protocols in relation to working with Māori and Pacific peoples and/or people from other cultures. 4 Task One Assess risk, develop risk management strategies, and prepare a risk management plan within a health organisation (portfolio) Outcomes 1, 2, 3 Portfolio requirements For this task you need to put together a portfolio of information to show that you have assessed risk, developed risk management strategies, and prepared a risk management plan within a health organisation. Portfolio documentation You must gather together a collection of copies of relevant documents, which you have used or have written, to show that you have met the requirements of this task, ideally within the last 12 months. You will identify risks that are relevant and sensitive to the information supplied in the context for the risk assessment. You will provide an evaluation of the probability and potential impact of the risks, and any potential benefits balanced against potential harm. You will develop risk management strategies that are relevant and sensitive to the identified risks, the risk assessment, and stakeholder input. You will provide examples of how your risk management strategies are based on a recognised model and/or philosophy for the type of risk being managed. You will identify how the risk(s) will be controlled, mitigated, or transferred, and monitored. You will prepare a risk management plan that covers four relevant factors in risk management, and is prepared following three requirements of your organisation's policies and procedures. These documents should include signed consent forms from the people with whom you have worked. This will give you permission to include documents such as a copy of the context for the risk assessment. Please remove all identifying details of the people involved. Your portfolio could also contain: Copies of relevant notes you have developed as you worked to develop risk management strategies eg memos, reflective journal for use in supervision. Copies of all forms you may have used to carry out your role to show that you have assessed risk, developed risk management strategies, and prepared a risk management plan within a health organisation. Copies of all documents that your organisation requires you to comply with or complete as part of your work eg copies of specific policies and procedures. Full details of this task are listed in the portfolio checklist on the following pages. Use this checklist to ensure you have completed the task. Your verifier will also use this portfolio checklist to verify that you have done everything in a satisfactory manner. Assemble your portfolio in a logical order. Make a list of documents provided, along with a summary of the main points in your portfolio. Put this list in a table on your own paper. Write your name on the page and attach it to the assessment. The trainee prepared a risk management plan that covers four relevant factors in risk management. Relevant factors may include context, risks identified, assessment, strategies, communication and consultation process, monitoring and evaluation systems. The trainee prepared the risk management plan following three requirements of the organisation's policies and procedures. Requirements may include agreed timeframes, format and method of presentation for a risk management plan, lines of communication and authority for presentation of a risk management plan.
