4 1. See what can happen (2009) Accident Case Studies Plant Supervisor - Health & Safety Role Model. Don t rush around; don t cut corners; if you think something is unsafe don t use it; if you think anything is unsafe report it to your line manager. Carol, is very clear about how her accident happened and knows now it was preventable she s really committed to putting safety first. Carol Wright AB West Brom IOSH F & D Group Event Accident case studies DVD with real AB people involved in actual accident and near miss situations. Carol Objective: - Engage people with the Safety First Vision of establishing an interdependent H & S culture; people looking after themselves and their colleagues. Context: - LTI Accident rate relatively high with unsafe acts a as common factor. Evidence of unsafe behaviours arising from audit processes (and CCTV!) Low rate of hazard and near miss reporting. I feel very bad about the accident it made me feel scared to drive my FLT I tried to stop him driving the truck he wasn t trained I wish I d told someone to stop him doing it. It s not just those who are injured who are affected ask Lamin. Simple 2 way communication or use of the hazard and near miss reporting system could have prevented the accident from happening. So what did we learn (2010) IOSH F & D Group Event The Road to Safety: - Safety journey analogy New incidents but with similarities to the 2009 campaign Reinforce Safety First care for self and care for others vision Formalise hazard and near miss process Introduce Stop Think Go de-mystify risk assessment and enable point of work RA On the Road to Safety If there s a hazard or risk or if you re in doubt STOP. THINK about and size up the risk and take act to eliminate the hazard or reduce the risk. Only proceed to GO when happy everyone is safe.
