CHSO R CHSM R Pr. CHSA R UNSUCCESSFUL Applicant notified of shortcomings Missing/ inadequate documentation Incorrect category applied for Application fee not paid SUCCESSFUL

3 CATEGORY OF REGISTRATION Down load the Scope of services for your chosen category from the SACPCMP web site. Make sure you familiarise yourself with the requirements. A generic guide has been developed to give applicants a basic idea of registration requirements. This is only a guide; CHS Officer (CHSO) - persons operating at site level CHS Manager (CHSM)- persons operating at a management level Pr. CHS Agent (Pr.CHSA)- Professional CHS Agent - persons operating at a professional level for the client or on the professional team. Registration Criteria Down load and complete the SACPCMP application form; Attach certified documents as required on page 4; SACPCMP 003A The example references CHSO route in particular Section A Personal Particulars. Ensure they are accurate and up to date. Certified copy of ID. Certification date not older than 3 months. Section B Category of Registration. Select the category best suited to your experience, knowledge and qualifications Section D (D1) Registrations with Professional Institutes. If your company is a member of any of the MBA s or SAFCEC then state this here. Also personal registrations with voluntary bodies like; ACHASM, IoSM or SAIOSH Section C Educational Qualifications. Note highest qualification obtained to date. Certified copies of qualifications must be submitted.
4 Section D (D2) Continuous Professional Development; - Note down evidence of activities or studies done that show your commitment to remaining relevant in the field of Construction Health and Safety, this could include: Short Courses/ Workshops / Conferences / Presentations / Relevant Journal Articles published. Section E Practical experience in CHS The three sections require the applicant to declare their experience. This must be verified in the Project Profile A1 and Project Reports A2. (Detailed in Step 2 & 3- figure 2&3) APPLICATIONS WILL NOT BE PROCESSED IF ANY OF THE REQUIRED DOCUMENTS ARE NOT SUBMITTED Section H Documents to be submitted with the form; a quick easy reference of all documents which must accompany the application. Ensure all forms are there and certified as required. Section F Employers Details Provide current employer details Section G Declaration Registered persons are bound to operate ethically, read and understand the Code of Conduct. Registered persons are bound by this code and may be deregistered if failing to comply with this code. Sign and Date the form.
5 PROJECT PROFILE ANNEXURE 1 This section is for you, the applicant, to show case your experience through the projects which you have had been involved in. The project profile gives the assessor insight into the scale, complexity and type of projects you have been involved in. Use the headings on the form, research and state as much information as you are able to obtain. Down load Annexure 1 Project Profile format. You may develop a spread sheet to record the required details. Annexure A1 Profile: Information is required for the last 2 4 years. Name of project Project 1 Project 2 Project 3 Type and description of project Geographic location of project Name of client and position and contact details of client representative List of participating organisations Year started and year completed (or planned completed date) Original completion date Actual completion date Percentage of practical completion Total value of project Percentage participation of your organisation in the project Your specific role in the project Was the project successfully completed If the applicant is involved in multiple projects over the period then show case the projects that best display your experience and list all others.
6 PROJECT REPORT ANNEXURE 2. This is the most important part of the application and needs to be executed correctly in order for you to receive the best possible scoring of your knowledge, skill and experience You must convince the assessor of your competence to be registered and operate in your chosen category of registration. Develop two separate project reports of 1500 words each that can adequately show the assessor your understanding, competence and knowledge in the pre-identified nine knowledge areas. With a short introduction and conclusion this amounts to a little over 150 words per knowledge area. Do not stray from the requested knowledge areas! Down load Annexure 2 Project report. Annexure A2 Report: Report 1 Project Successes. Write about your achievements on projects. How you have dealt with every day events. You should be able to record success in each of these areas without difficulty. Use practical examples to display to the assessor your understanding and knowledge TIPS. Do not simply copy and paste material from reference materials. If you do use a reference in your report then acknowledge the source. Do not leave any of the knowledge areas out, at least try and explain your limited exposure to the particular knowledge area. Remember complete the A2 report to show your individual experiences and understanding - it is YOU that is registering. Write your report in the 1st person using I and My as opposed to the company or we.
