1 NOTICE INVITING APPLICATIONS FROM ELIGIBLE AGENCES FOR EMPANELMENT OF ARCHITECT, COSULTANT, VALUER FOR OUR BANK'S BRANCHES/OFFICES/FLATS IN Varanasi REGION Bank of Baroda, Regional office, Varanasi region,177/1, Assi Road, Lanka Varanasi invitesapplicationsfromreputed Architect,Valuers,Consultants, having wide network around Varanasi Region in various categories as mentioned below to carryout different works for our Bank's Branches/Offices and flats in Varanasi,and district in Eastern UP, Existing empanelled persons need to apply again. Purpose: Bank intends to select Consultants, contractors and Service Provider's carryout various works for our bank's branches, offices & flats in Varanasi, near by district in eastern UP in different categories as under: Agencies, which fulfill the criteria specified against each job/category, may apply and furnish the information in prescribed format to the addressee given hereafter, WORKS Civil Furnishing Electrical Structural/ Strengthening work CONSULTANTS/ CONTRACTORS Architects/ Structural consultants Electrical Consultants Various other work The Empanelment will remain valid for three years depending upon the work done by the Consultants, Contractors and other Service providers or till such date as decided by the Bank subject to renewal for further period of one year, on the same terms and conditions and at the discretion of the Bank. 2 of application: Other Terms & Conditions: a) All the Annexure are to be submitted in one envelope only. b) No price/ cost/commercial information is being sought.so envelope should not contain any commercial/ financial information. This information will be called separately in future, from the qualified/ empanelled bidders. c) Selection of applicants will be depending on various parameters set on pre-qualification application. d) The applicant must have wide network in the Varanasi Region. e) Application form and other documents can be downloaded from the Bank's Website or can be obtained from the premises Dept. of the aforesaid office up to PM on last date, on production of a request letter from the contractor. Cost of the tender documents is Rs 100/- (non-refundable) which may be paid by way of Demand Draft/Banker's cheque in favour of "BANK OF BARODA" payable at Varanasi. 3 Pre qualification criteria for empanelment of consultants, contractors and other service providers - in Each category. Architects/Interior Designers Structural Consultants Should be holding a masters/bachelors/degree in the field. Must be a Member of Council of Architects should have reasonable presence for professional work in Uttar Pradesh. Agency should have completed 1 job of 0.80 Cr. or 2 jobs of 0.50 Cr or 3 jobs of 0.40 Cr. during last five years. Annual Average turnover for last three years should not be less than 0.30Cr. Should have done Post graduation in structural Engineering from a reputed university in India. Reasonable presence for professional work in Uttar Pradesh. 10 years Post qualification experience. completed 1 job of 0.80 Cr or 2 Jobs of 0.50 Cr or 3 jobs of 0.40 Cr. during last seven years. Annual Average Turnover for last three years should not be less than 0.30 Cr. Electrical Consultants Should have done graduation in Electrical Engineering (Degree) from reputed university in India and having 10 years experience. Reasonable presence for professional work in UttarPradesh completed 1 job of 0.80 lacs or 2 jobs of 0.50 lac or 3 jobs of 0.40 lacs during last seven years. Annual Average Turnover for last three years should not be less than 0.30Cr. valuers Should be a member/ fellow of Institution of valuers and registered with Income tax department in India. Reasonable presence for professional work in

4 Uttar Pradesh. 10 years experience in the field of valuation. 6 charge. 09 Yearly turnover of then organization during last 5 years ( year wise) and furnish audited balance sheet and Profit & Loss A/c (Audited) for the last -3- years. 10 Name and address of Bankers ( Solvency certificate from a bank of be enclosed for indicating satisfactory financial capacity of the organization 11 PAN No. 12 Details of VAT registration with a copy 13 Details of registration for sales tax 14 Detailed description and value of works done ( Proforma 1) and works on hand (Proforma-2) 15 Details of Key personnel Permanently employed ( Proforma -3) 16 Other infrastructural information to be used/referred for this project ( Proforma -4) 17 Furnish the names of -3- responsible persons along with their designation, address, Tel No. etc. for whose organization, you have completed the above mentioned jobs and who will be in a position to certify about the performance of your organization 18 Whether any Civil Suit / Litigation arisen in contracts executed/ being executed during the last 10 years. if yes, please furnish the name of the project, employer, Nature of work, contract value, work order and brief details of litigation. give name of court, place, status if pending litigation. 19 Information relating to whether any litigation is pending before any Arbitrator for adjudication of any litigation or else any litigation was disposed off during the last ten years by an arbitrator. if so, the details of such litigation are required to be submitted Attach a separate sheet if required.
