5 Biometric Screening Event Planning Standard non-fasting finger stick package includes- Total cholesterol, HDL, glucose, blood pressure, height, weight, waist circumference, and BMI. Scheduling Policy A minimum notice of six (6) weeks is required to book a screening event. Getting Started - To begin discussion for planning your onsite event, the following information is required: o o o o Number of eligible staff at location Event start time Location address Contact information (per onsite location) Minimum Participation Requirements Twenty (20) individual screenings per event is required. Standard Event - Screenings typically occur Monday through Friday, between the hours of 7am and 7pm. Screenings are usually performed with a minimum of five (5) hour (example 7am-12pm) and no longer than seven (7) hours. Eligible Participants Subscribers and Spouses enrolled in the group s Blue KC Medical plan are eligible to participate at the onsite event. Non-Blue KC Medical Plan individuals can participate at $45.00 per participant (non-fasting finger stick test) 5

8 First Time Experience Onboarding Questions Members will be required to complete an initial First Time Experience questions when accessing the AHY platform for the first time. o Questions are specific to identifying health goals and current lifestyle activities. o Responses to these questions will personalize the site to the individual. o These questions are not associated with the Health Risk Assessment. Once the First Time Experience questionnaire is completed, members will be awarded points and can then access the AHY Platform to register for screening events and complete the HRA. 8

11 Registration Options Online Registration AHY Platform: Members can register for their (onsite or physician form)appointment by visiting MyBlueKC.com and clicking the A Healthier You link on the homepage. o Members will be required to complete a first time experience questions when accessing the AHY platform to personalize the site to the individual. Available to Blue KC subscribers and spouse. Blue KC A Healthier You App Registration: Download the Blue KC A Healthier You app from the Google Play store or Apple store. o o o Available to Blue KC subscribers and spouse. Individuals will need to register as a new user through the app since it is not affiliated with Blue KC website login information. Blue KC Member ID card information is required to register on the app Manual Employer Registration: Employers have the ability to register members manually for the onsite event. Form is provided to track appointments: o Only one name per line. o The appointment sign-up list is due to AHY by noon the day prior to the screening event. o The onsite staff will require a copy of the final list upon arrival. 11

12 Appointment Confirmation Once registration is completed, participants will receive a secure confirmation which contains paperwork for the event. Participants will be prompted to print all documentation and bring with them to their scheduled screening appointment. The appointment confirmation page will display the individual address where paperwork will be sent. Passwords to access the screening paperwork is contained in the . Members will receive a reminder the day before their appointment including the paperwork and instructions to print. Members have continual access to print their paperwork prior to their appointment by logging back into the AHY platform registration system. 12

13 Manual Submission Form Completion Criteria For the Manual Submission Form to be submitted as complete: o Members must complete all fields o Member must submit lab values Once Manual Submission Form has been submitted an confirmation will be sent to individual. o Automatic confirmation s will be issued to the member within 48 hours to confirm acceptance of a completed form. o If the form is missing information that requires action from the member an will be sent to the member specifying the missing information and requesting the individual resubmit. o Reminder s will be sent to the member monthly to obtain missing information. 13

14 Registration Monitoring Track appointment signups! As an administrator, you can check the registration progress of your event by logging into the admin section of the online administrative tool. Registration Reporting: The Sign-up Report has two different options. One option allows you to access the daily appointment schedules signed up for your facility event(s). The second option allows you to view the participation percentages of all events in the system. 14

15 Screening Event Day The day before the event you will receive the sign-up sheet with the daily appointments and blank paperwork. The morning of the event, print a copy of the daily appointment schedule for the Lead Screener. Please have participants arrive no more than 5-10 minutes prior to their scheduled screening time. This shortens wait times, time away from work and assists with event flow. Upon participants arrival they will check in: o o If the participant is on the schedule technician will check to ensure they have their pre-populated form or provide them a blank registration form to fill in manually. If the participant is not on the schedule registration staff will determine their ability to work them into the schedule at that time or assign them a time to come back. Following check-in, participants will be directed to a screening station as soon as one is available. Once the participant s screening is finished, participants will be given a copy of their results and be asked to complete a satisfaction survey as they leave. 15

16 A Healthier You Reporting Participation Reporting & Aggregate Results Participation reporting will be provided based on the following schedule: o Biometric screening & HRA completion lists will be provided within five (5) business days following the conclusion of the final onsite event. o An additional Biometric screening & HRA completion list will be provided one week prior to the HRA deadline. o MSF deadlines should always be two weeks prior to the HRA deadline. o The final Biometric screening & HRA completion list will be provided five business days following the HRA deadline. o Aggregate reports containing Biometric screening & HRA data will be delivered directly to the client 30 days following the HRA deadline (50 participants reporting requirement). 16

19 A Healthier You Incentive Support Reporting Participation Lists Biometric screening & HRA completion lists will be provided within five (5) business days following the conclusion of the onsite event or the HRA campaign. An additional Biometric screening & HRA completion list will be provided one week prior to the incentive deadline. The final Biometric screening & HRA completion list will be provided five business days following the incentive deadline. Aggregate Reports Aggregate reports containing Biometric screening and/or HRA data will be delivered directly to the client 30 days following the HRA deadline (50 participants reporting requirement). Aggregate Platform Activity reports will be delivered quarterly. Health Action Credit Completion reporting will be delivered at the end of the program year. Points based accrual lists will be provided on a monthly basis prior to the incentive deadline. Sweepstakes prize lists will be provided based on payroll schedules for taxation purposes.
