2 Instructions to the Applicant Talk to your assessor if you need help to complete this application. Your assessor can ask you questions and write your spoken answers in this application if needed. Section 1: Applicant details page 3 This section contact details for you, your employer (if you have one) and the RTO who will assess this application. Section 2: Work & life experience page 4 This section lets you list the things you already know (knowledge) and can do (skills) which might be relevant to the unit of competency. Section 3: Previous training page 5 This is where you can list training courses that you have already done that might be relevant to the unit of competency. Section 4: Knowledge questions page 6 These questions allow you to do your own assessment of your knowledge against the critical aspects of assessment of the unit. Section 5: Evidence page 10 The candidate should provide a range of evidence to support sections 2, 3 and 4. The candidate can use third part reports, workplace documents and/or training. Where additional evidence is needed, the assessor will use the candidate s responses to section 5 Assessment questions to determine if RPL can be granted. Section 6: Assessment benchmarks page 11 This section is completed by the assessor. All RPL applicants must successfully complete the online skills demonstration, as well as the verbal/oral role-play assessment activities, in order to meet the assessment conditions specified in the training package. 2
