1 Welcome to the IUSO HR EYE-NEWS: February 2015 Edition Greetings IUSO team members, from your Human Resources and Payroll Department! HOLIDAYS: We have one upcoming paid holiday for University appointed staff and faculty. Campus Holiday is treated as a floating holiday, which is available to use on or after March 1. Employees may use this holiday on a workday of their choice, with prior supervisor approval. Campus Holiday March 1 *Friendly reminder: The Campus Holiday must be taken at the end of the last full pay period of the following year or it is lost. W-2 INFORMATION: If you elected to receive your W-2 electronically, your 2014 Form W-2 is now available for download or printing. The form can be found in the Employee Center, Tax information section of OneStart ( by clicking on the link titled View W-2 data. Instructions for how to retrieve the form can be found on the Indiana University Financial Management Services webpage Questions related to your Tax Form W-2 may be addressed through at iuw2@indiana.edu or by phone at (812)

Staff Member Years of IU Service Anniversary Date Tina Bailon 5 years January 25 th Melissa Larsen 5 years April 19 th Carolyn Masters 10 years April 5 th CONGRATULATIONS, MIKE HALLA: It is with mixed feelings that we announce the retirement of Mike Halla from IUSO on February 27, Mike has been with IUSO for 5 years as the Clinic IT Manager, Data Management Support, and he has been with IU since Please stop by Mike s office 269 on Friday, February 27 th, from 1pm to 4pm to wish him well and have a piece of celebratory cake. Congratulations Mike and thank you for your incredible service to IUSO! COMMITMENT TO EXCELLENCE AWARD RECIPIENT: SUSAN BARLOW: Congratulations to AECC s Susan Barlow, who has received the Commitment to Excellence Award from IU Optometric Student Association! The award was presented at the Eye Ball on Saturday, February 21.

3 INDIANA UNIVERSITY POLICIES: Policies communicate IU s expectations of employees and support its compliance with laws and regulations. Please review the below links to see which set of policies pertains to you in your current position! These principles are intended as a high level statement of values and expectations at Indiana University. The Principles themselves do not create additional or different rights or duties; rather, they help to promote an organizational culture that encourages ethical conduct and a commitment to compliance with the law and University codes, policies, and procedures. Find the Principles of Ethical Conduct at: CLINIC POLICIES NOW POSTED ON THE INTRANET: All Clinic policies are now posted on the IUSO Intranet Home Page: On the Home Page select IUSO Policies & Plans, then Clinics and you will find the option to view internal IUSO policies and procedures, as well as, University policies that relate to clinic functions. The approved and most current Clinic policies will be maintained on this site. Clinic staff and faculty will be informed when new or updated policies are posted. In the near future, look for more policies to be posted for HIPAA & Compliance, Information Technology, and other areas of IUSO. If you have questions regarding a specific policy, please contact the individuals listed on the policy. If you have a policy you would like to get posted, please contact IUSO Compliance & Privacy Officer, Claire Tempel at ctempel@indiana.edu.

4 HOW TO REPORT A WORKPLACE ACCIDENT: Workers compensation provides benefits to employees who sustain injuries by accident arising out of and during the course of their employment. Visit for a complete procedure, links to the necessary forms, and contact information. EMPLOYMENT BOARDS: Employment boards are located in the break rooms of IECC and AECC as well as outside the 2nd floor kitchenette in the Atwater building. These boards contain valuable employment information for all IUSO employees including: Labor Law posters University Compliance posters, including information for reporting concerns How to Report a Workplace Accident flyers Emergency Procedures handbook, flyers, and map E-Verify informational posters Current HR Announcements ANTHEM BREACH: Indiana University remains in frequent contact with Anthem to understand the extent of the cyber-attack and how it might affect IU employees. The information listed on includes important steps you can take to protect yourself from identity theft. University Human Resources will continue to monitor and communicate new information as it becomes available.
