JOB RESPONSIBILITIES: To effectively implement and manage the statewide implementation and utilization of the Supports Intensity Scale (SIS) and Health Risk Screening Tool (HRST); manage community inclusive residential models developed in accordance with the Home and Community Based Settings Final Rule models, the following responsibilities and duties are required: 1. Coordinates all activities with the contractor responsible for conducting the SIS interviews to ensure deliverables are met and the process for demographic information about those to be interviewed to the contractor, and assuring that results of the interviews are shared appropriately. 2. Manage the AAIDD and HRST contracts; serve as primary contact with AAIDD and HRST. 3. Extract data from the SIS and HRST online data systems; conduct basic statistical analyses as appropriate, and prepare regular summary reports by state, provider, and location. 4. Develop a teaching curriculum to use with provider management and authors of residential, day, and case management plans outlining specific steps needed to incorporate the results of a person s SIS or HRST assessment results into their plans reflective of the principles of person centered planning. Provide ongoing teaching to providers using the developed curricula on incorporating SIS or HRST results into consumers plans. 5. Synthesize individual provider and regional center SIS reports in a user-friendly format, provide overview of findings with executive director and facility administrators, assist them in the development of a business plan to respond to the aggregated SIS results, provide support in the implementation of the business plan and monitor implementation of the plans. 6. Answer questions of families and other stakeholders regarding the use of the SIS, HRST or community inclusive residential models. 7. Develop and manage agency policies regarding the use of the SIS and HRST. 8. Provide leadership with the ongoing implementation and expansion of the community inclusive residential projects, including coordination with the community inclusive residential model consultant(s), participating providers, and executive management. Ensure effective integration of roles of transition coordinator, District Offices, and Executive Management, facilitate bi-monthly meetings, update standards, and enhance fidelity of projects. MINIMUM AND ADDITIONAL REQUIREMENTS: Must have a bachelor s degree in a health or human services field with at least five (5) years of experience working in the field of Intellectual Disability, and two (2) years of practical application involving development of spreadsheets and analyzing data. Applicant must possess some experience providing technical assistance and/or teaching or giving training presentations. Must have excellent written and verbal communication skills, work effectively with and in diverse teams, and have strong

3 work ethics. Must maintain a valid SCDL. PREFERRED QUALIFICATIONS: Familiarity with Support Intensity Scale (SIS) and Health Risk Screening Tool (HRST) data and their usage, positive behavior supports, self-determination and the current community inclusive residential models. Experience providing services to people with intellectual and developmental disabilities, traumatic brain or spinal cord injuries or Autism Spectrum Disorder is preferred.
