3 Organizational Overview This information will be used during the Awards Ceremony for winning companies. Describe the mission, vision and philanthropic culture of your organization: CenterPoint Human Services is a state-mandated Local Management Entity and Medicaid funded Managed Care Organization in charge of overseeing the delivery of publicly-funded mental health, developmental disabilities and substance abuse services in Forsyth, Stokes, Davie and Rockingham Counties. CenterPoint works closely with community partners,advocates and service providers to address the service needs of the people and communities we serve. Mission: CenterPoint Human Services is a managed care organization responsible for assuring that accessible, quality and accountable care is available for those with mental health, intellectual and developmental disabilities and substance abuse challenges. Vision: CenterPoint Human Services will become the best practice public model of managed care for exceptional and compassionate oversight of the provision of care for those with mental health, intellectual and developmental disabilities and substance abuse challenges. Values: We will live out our mission by: Respecting and valuing all individuals Serving individuals and families with compassion and care Maintaining an unwavering commitment to high quality care given by our provider network Being responsible stewards of the funds entrusted to us Describe ONE unique and meaningful event, activity, or communication that ignited the success of your campaign: (This will be shared at the Awards Banquet if selected as a winner) The big event this year CenterPoint had was the Chili Luncheon for employees to give a donation which also paid for their lunch for the day! We had more participation than ever because of more activities this year! (see list below). It helped us also to meet/exceed our overall company goal! 4 Standard 1 - Volunteer Culture Describe how volunteerism fits into the organization s philanthropic mission. Include specific volunteer activities (listing recipient organizations) and, if possible, provide number of employees, volunteer hours and dollar value of volunteer participation. (Estimated value of volunteer time for 2013 is $22.55 per hour) Include organizational incentives for participating in community volunteer activities. Up to 5 points We are community-minded and give a minimum of 150 hours each year to volunteer in the community. Usually the majority of the hours are dedicated to helping with the United Way campaign as well as volunteering time to a local Habitat for Humanity. Our steering committee is strongly considering doing a day of caring project. We also promote all the United Way affinity groups, Young Leaders United, Women s Leadership Council and Leadership Circle which all involve volunteering within the community. Care Coordination Department had a food drive for local shelters.

5 Standard 2 Partnership with community to raise awareness of needs and foster a spirit of giving Describe the organization s philanthropic relationship with the community. Document specific rallies, events, or programs which generate enthusiasm for community involvement and support. Unique partnership and awareness activities should be highlighted such as specialized meetings, tours, speakers and fairs. Activities which have positive results for year-round engagement should be cited as well. Up to 5 points Each year CenterPoint partners with providers, nonprofits and community businesses to sponsor FUNday at Central Park in King. FUNday began in 2006 with the simple idea of hosting a special day in which clients and families could forget about their day-to-day challenges and just have FUN with other people from their community. With the help of the Mental Health Association in Stokes County and many volunteers, FUNday exploded into a totally FREE, totally FUN event that has become a family tradition. Last year, CenterPoint partnered with Unlimited Success, an affiliate of Partnership for a Drug-Free NC and more than twenty organizations to provide a day of food, entertainment and fun activities for all ages. In May each year CenterPoint hosts an event at its facility recognizing National Mental Health Month. The event is a community collaboration of various local agencies and organizations involved in the mental health field. Annually in November CenterPoint hosts a Provider Fair which is designed to provide access and supply information to the public about the various services available in terms of mental health, developmental disabilities and substance abuse.

6 Standard 3 - CEO/Senior Leadership Involvement and Giving Given that Leadership participation is a Best Practice item: Describe how senior leadership (CEO/Direct reports) hosts, manages, organizes, participates in, and influences philanthropy, community support, and campaign activities. Describe how company leadership in general promotes a culture of giving. Specific events that are sponsored (and led) by the leadership team should be noted. Complete the leadership giving table below. Up to 10 points* A gift of $1000 or more is considered a leadership gift. Betty Taylor, Area Director/CEO has always been a strong United Way supporter. As the leader of CenterPoint, Taylor encourages all employees to give what they can and to participate in as many fundraising events as possible. This year she challenged the United Way Committee to develop additional fundraising opportunities which resulted in a +500% increase in special events $ s vs In addition to her normal support, she is also a member of the United Way of Forsyth County Women's Leadership Council. This is a strong council that enables engages and empowers women in the community. Their main focus is to increase the high school graduation rate. This is Betty Taylor s 5 th year in the Forsyth County Women s Leadership Council. Dr. Stephens Bronze Leader. This year Dr. Stephens addressed the employee kickoff meeting for the campaign with a strong message. Both Betty Taylor and Dr. Stephens were active participants throughout the campaign- - - including coming in costumes for the Halloween event!! 7 Standard 4 - Corporate or Foundation Contributions and Other Non-traditional and In-kind Gifts/Support State where management places the United Way Campaign within its prioritization of activities. List specific sponsorships, resources, materials, loaned executives, advertisements, videos (YouTube as an example). List specific social media activities/events/activities. CenterPoint is a Local Management Entity/Managed Care Organization (LME/MCO)/public entity and not able to give a corporate donation or any type of resources. However, we do give our time and collaborate with agencies who are partners with United Way such as the following: o Mental Health Association of Forsyth County o Centers for Exceptional Children o Community Care center o Partnership for a Drug Free NC United Way is the only organization wide fund raising campaign for CenterPoint Human Services employees each year. Our giving spirit is aligned with our corporate philosophy. 8 Standard 5 Employee campaign coordination, incentive and recognition Describe the work of the campaign coordinator and/or committee and list specific activities, decisions and events. Share specific incentives used in the campaign and how employees were recognized within the workforce for their contributions. Up to 5 points United Way Champions: United Way Champions, Cheryl Jeffries, Wenna Phelps, and Christine Mabe are the main contacts for United Way. Primary responsibilities: Build a committee, act as mediator between Betty Taylor, Area Director/CEO and the Planning Committee, answer employee questions, serve as a point of contact for all employee activities, and assist in anything needed regarding the campaign. Our new United Way Department Champions were instrumental in increasing employee awareness and building momentum to a successful campaign FUN raising challenge. Steering Committee Members: Each year we have very dedicated employees who served as Steering Committee Members. Their primary responsibilities included: constantly communicate with all employees, assist and give ideas to employees for fundraising, answer employee questions, serve as a point of contact for all employee activities, and assist in anything needed regarding the campaign. Our Steering Committee is very creative and likes to think "out of the box." They are truly committed to the United Way and its mission! The committee is always very creative with their campaign theme as well as coordinating creative campaign events and activities! Incentive Administrative days off are offered for donation giving

9 Standard 6 - Overall Per Capita Gift Describe what employee, leadership, workplace, or community issues made a difference this year in per capita giving by employees. (Note: If campaign results increased (or decreased) by a measurable amount, it is expected that those changes were driven by some recognizable improvement or change. Example: Instituted specific goals by department for the first time equal to the corporate campaign objective which generated enthusiastic and competitive environment.
