5 5.1. Tracking and Administration Total Oilfield uses training courses to improve employee safety by providing knowledge, improving skills and changing behaviour. We use job task analysis to check out our employees and contractors. This is documented and filed with the HSE Coordinator. We train our supervisors to monitor and direct our safety program. We train our workers to improve job skills, knowledge and to teach them to work safely. Our standard compliance courses must be completed before any field personnel begin work. The courses may include: Standard First Aid & CPR, H 2 S Alive, WHMIS, TDG, Enform PST training, Hand Signals Course, Hazard Analysis and Codes of Practice Review. Selected personnel will receive specialized training courses. Training will be evaluated to ensure it produces results. Once trained our employees are expected to demonstrate a measurable improvement in skill and behaviour. The QHSE Coordinator will identify, track and record all employee and owner/operator training. Total Oilfield will provide all training for employees. However, it is the owner/operators responsibility to acquire and pay for the standard compliance courses Standard Compliance Courses Workplace Hazardous Materials Information System (WHMIS) All new employees and Owner/Operators are required to take and pass WHMIS training First Aid and CPR All new employees and Owner/Operators are required to successfully complete an approved standard first aid and CPR training course with an approved training facility upon starting with Total Oilfield. This training is required to be maintained throughout employment with Total Oilfield. It is recommended that CPR training be updated once per year H 2 S All new employees and Owner/Operators are required to successfully complete Enform H2S Alive training upon starting with Total Oilfield. This training is required to be maintained throughout employment with Total Oilfield Transportation of Dangerous Goods (TDG) All new employees and Owner/Operators are required to successfully complete TDG training upon starting with Total Oilfield. This training is required to be maintained throughout employment with Total Oilfield Training Page 5

6 Hazard Assessment The hazard assessment training is conducted utilizing a home study package, which must be returned to the QHSE Coordinator to receive credit for this standard compliance course Codes of Practice Review The two operations at Total Oilfield that require a code of practice are Respiratory Protective Equipment and Confined Space Entry. Both of this codes of practice will be reviewed the employee or owner/operator in detail to ensure all aspects of the code of practice are communicated and understood Hand Signals All new field employees and Owner/Operators are required to successfully complete hand signals training upon starting with Total Oilfield. This training consists of videotape and receiving a wallet-size card for quick reference Regulatory Requirements All new Operators and Owner/Operators are required to successfully complete training in regulatory requirements such as hours of service, weights and dimensions and load securement upon starting with Total Oilfield. This training is required to be maintained throughout employment with Total Oilfield Training Page 6

7 5.3. Safety Orientation Every employee and owner/operator who is new to Total Oilfield will receive a safety orientation before work commences. All visitors will be provided with a safety orientation and will be required to sign in and out of the area visited. The employee and owner/operator orientation will include: Company safety policies and rules Working alone policy Worker's responsibilities to wear appropriate work clothing Specific job hazards Safety precautions Job responsibilities Regulatory requirements Road test, hours of service, weights and dimensions, etc. (Operators only) Company enforcement policy Worker's obligation to refuse to do unsafe work Company's responsibility to provide a safe workplace Emergency evacuation procedures Alarm systems Incident/accident reporting Fire protection Location of first aid facilities Records of all orientations will be kept as part of each employee's safety training record. Each employee must sign an acknowledgment form following the orientation. A copy of the orientation checklist is attached below Training Page 7

10 5.4. On-The-Job Training Total Oilfield will ensure workers have the necessary education, experience, and training to perform their job tasks. In addition, Total Oilfield will provide on-thejob training to all workers new to a job. Senior personnel who are familiar and competent in their job will conduct the training. 11 5.5. Job Competency Total Oilfield Rentals has an organizational chart that lists the job titles within Total Oilfield Rentals. This chart also shows the reporting structure between the jobs titles and the levels with our organization. Total Oilfield Human Resources and Safety departments have minimum qualifications required to perform each job within our organization. This is a combination of education and work experience that is different for each job performed. This list is subject to change as necessary Proof of Training All employees will be required to provide proof of training for their applicable job requirements, upon orientation. Copies will be taken and kept on file, and are kept in accordance with the Freedom of Information and Privacy act Job Specific Training New employees will be provided with job specific training, any employees transferring to a new position will also be provided training for the applicable position. All employees will be trained on the tasks they perform on a regular basis Verification of Training A competent person (Field Supervisor, Driver, etc) will verify that an employee is competent to perform their roles and responsibilities before being allowed to work independently. This program works in combination with the green hand/short service employee program Training Page 11

12 5.6. Green Hand/Short Service Employee Program Total Oilfield Rentals LP recognizes the extra commitment and training required for new employees, and has developed a plan to identify and ensure competency among new green workers. The goal of the Green Hand Program / short service employee is to heighten the visibility of new workers and prevent injuries during their initial months of service. Green workers those new to their duties who will face increased risk because they re not familiar with the hazards face a harsh reality. Facts: new or inexperienced workers will have twice the workplace injuries of experienced workers, and of those, the ones less than 24 years of age will have a 60% chance of being injured in their first 6 months. Total Oilfield Rental s Green Hand Program will assist a green worker through a high risk period by identifying the new employee with a green hard hat. This will aid in the experienced workers to keep an eye on them Definition Green workers can include new hires, transferred workers, reassigned or promoted employees, and summer students. Those employees designated to wear the green hard hat must do so for a three month period of time or until they have showed the sufficient level of competence. An employee is considered a Short Service Employee if they have less than 6 months experience with Total Oilfield. Short service employees must not work alone. Where required, Total Oilfield shall notify the customer prior to the start of work commencing, that a short service employee will be coming onsite. The individual might have to wear the hard hat longer to complete specific orientation or safety policies and to pass Total Oilfield Rental s job skill exam. Wearing the green hard hat means that the green worker will have the benefit of having experienced co-workers looking out for them. Seeing the green hard hat would mean co-workers would not make assumptions about the green worker s experience or work habits and would not expect them to: do their tasks as quickly as an experienced worker, know the site-specific practices and procedures, be familiar with the site permit to work or similar company-specific controls for keeping the workplace safe, be familiar with operations, especially in an emergency, and know task hazards and activity-related dangers or how to avoid them, be willing to ask questions, or be sure of who to ask Training Page 12

13 Instead, co-workers would introduce themselves and offer help or advice and would: make sure to know where green workers are and what they are doing, work with them until satisfied the job they are doing will be carried out safely, providing positive and valuable guidance, and above all patience mentor them to make sure and visually confirm that they have reached task competency before letting them work alone, buddy up with them in emergencies, and encourage them to ask questions and get feedback to gauge their understanding green workers are often unsure of what to do but are also often reluctant to ask questions. As any effective safety plan, success of a Green Hand Program is commitment to the concept behind the plan and will ensure that green workers are singled out for help, not practical jokes, initiations, or abuse behaviors which are prohibited by Total Oilfield Rentals Guidelines When first hired Ensure all new workers receive the health and safety orientation to Total Oilfield Rentals (A Division of Total Energy Services Inc.). After the health and safety orientation is complete, the green worker will receive a green hard hat. After Sixty days After sixty days, and before the 3 month probationary period ends, the immediate supervisor (driver) will go through the Swamper Field Hazard Checklist with the employee to assess his/her knowledge of the information that was covered in the health and safety orientation. The individual will then write an exam. This exam, once marked, will be forwarded to the Safety Department for placement in the employee s file. After the exam has been marked, the branch manager will go through the checklist and exam. From this, and taking into consideration the employee s safety performance, the branch manager will make a decision Training Page 13

14 Decision 1: If the green worker has shown that they are knowledgeable about the company s safety policy and have met the safety performance expectations, then they graduate out of the green hard hat policy. At this time the employee will hand in their green hard hat and receive a white hard hat. Or Decision 2: If the green worker has not shown that they are knowledgeable, or there is a concern over the individual s safety performance, then they will have to continue wearing the green hard hat until they show that they are knowledgeable and/or have improved their safety performance. Additionally the Branch Manager will discuss with the employee, the areas required for improvement and continued employment Subcontractors Subcontractors working for Total Oilfield Rentals must manage their Short Service Employees with the same requirements of this Short Service Employee program Training Page 14
