Provide effective coordination and support of programme and project activities, particularly those associated with programmes which address the physical health needs of people experiencing serious mental health or addiction issues (such as Our Health in the Mind programme and Equally Well) and with programmes that support sector-wide development (Our Health in Mind primary care support activities) (see below for further information about the Our Health in Mind action plan) 2. Ensure a collaborative approach is taken with engagement from key stakeholders into programme development and programme activities 3. Work effectively with a range of Clinical Governance groups to ensure the implementation of approved activities 4. 3 5. Robust project, quality improvement and implementation methodologies are utilised 6. Contribute to the development of business cases as required, particularly those related to the Our Health and Mind Action Plan or Equally Well. Our Health in Mind: Growing wellbeing for our community A five-year Strategic Action Plan Our Health in Mind: Growing wellbeing for our community has been endorsed by the WDHB board. The action plan aligns with the Board s purpose to promote wellness, relieve suffering, prevent, cure, ameliorate ill health for people (and their family/whānau) who experience mental health and substance misuse. The plan takes a wider systems improvement focus, keeping better life and health outcomes for people and their family/whānau at the centre. The scope of the action plan is across the lifespan and whole system. This incorporates, but is not limited to, primary care services (practice and PHO based), community agencies, NGOs, tele health and digital health tools, school based health services, aspects of DHB secondary specialist services and other government agencies. The Plan has a specific focus on the contribution, development and support of primary and population mental health approaches and services to better meeting the needs of local people, families and communities. 4 KEY TASKS Programme coordination Hold a key role in ensuring the sustainable delivery of existing programme and project activities which will be mainly primary care initiatives including Our Health in Mind, including: Progress report to a range of audiences Lead the planning of actions which will meet the approved goals and business case activities Ensure engagement of all key stakeholders Utilise improvement science methodologies, as appropriate Provide timely communication, information and updates to all key stakeholders Additionally this will involve ensuring close links are maintained to other Initiatives in the sector related to primary care and improving responsiveness to mental health, well-being and substance misuse Strategy, business case and action plan development Work in close partnership with the SMH&A leadership and with any relevant governance groups to develop appropriate action plans, strategies, and to contribute to any new business cases All programme and project planning is based on sound evidence, research and stakeholder engagement and is well aligned with WDHB s priorities and wider regional and national initiatives to tackle long-term mental illness and addiction issues EXPECTED OUTCOMES Appropriate evaluation measures and metrics are used to measure the impact of actions Monthly governance group meetings occur for Our Health in Mind and are an opportunity for direction setting, monitoring and decision making based on members experience and knowledge Provide support to the Chair of the Governance Group, as required Attendance and reporting at other service governance meetings as appropriate Clear and timely communication processes are established including regular reporting for the Board, and key leaders are regularly kept informed of progress and risks Work plans and business cases are linked and aligned with other relevant work programmes and initiatives and are designed to have a positive impact on the lives of people All activities remain aligned with WDHB s priorities and wider initiatives with long-term mental illness and addictions issues There is regular engagement with relevant organisations and other health/community system colleagues Utilise appropriate DHB processes and formats and recognised business and quality methodologies to develop business cases and plans Timely evaluation of business case activities facilitates strategic planning for the next phase of activity High quality, well written reports and plans are produced as required Key system partnerships and local stakeholders have real time knowledge, involvement and active opportunities for participation in the design of new initiatives and business cases Related processes are informed by up to date local/regional initiatives, evidence, strategic guidance and effective collaboration and engagement across multiple and diverse stakeholders Waitemata District Health Board -JOB DESCRIPTION - Programme Coordinator Page 4

6 PERSON SPECIFICATION Minimum Qualification Bachelor s professional qualification relevant to health Post-graduate study to minimum of Post- Graduate Certificate level Experience Previous experience of working in community and/or primary care mental health or addictions context (minimum 6 years) A good understanding of mental health and addiction issues and services Demonstrated experience in programme and/or project coordination and implementation of a strategic plan Project and change management experience Leadership experience within health a complex health organisation or system (eg Primary Care, NGO, community organisation) Collaborative working with multiple agencies/stakeholders Previous experience of designing and implementing or service or quality improvement project Experience in utilising a range of evaluation measures and techniques Experience in reporting on development activities Preferred Relevant mental health or addictions Post graduate qualification e.g. health and/or social care management, community development, clinical qualification, Public Health Master s degree, or near completion Project management certificate level qualification Full registration with relevant regulatory body Management / leadership training evidence of study (Certificate Level minimum) Has existing credibility, (or potential to develop),with local mental health/ addictions providers, community and families Evidence of excellence achievements, eg, prizes, presentations, papers Lived experience of mental health and/or substance issues, either personally or as family/whānau Waitemata District Health Board -JOB DESCRIPTION - Programme Coordinator Page 6
