1 Page 1 of 8 COUNTY OF SONOMA INVITES APPLICATIONS FOR THE POSITION OF: Health Program Manager An Equal Opportunity Employer SALARY $ $43.77 Hourly $2, $3, Biweekly $6, $7, Monthly $75, $91, Annually OPENING DATE: 10/12/12 CLOSING DATE: 10/26/12 THE POSITION The County's Department of Health Services (DHS) is seeking skilled professionals to fill two current and two anticipated full-time vacancies as Health Program Managers (HPM) in its Health Policy, Planning & Evaluation, Public Health and Administrative Services Divisions. The employment list resulting from this recruitment may also be used to fill future full-time, part-time or extra-help (temporary) vacancies as they occur during the active status of the list. DHS also anticipates opening a recruitment to fill several vacancies in its Behavioral Health Division in the near future. Please note that if you are interested in a position with Behavioral Health, you will need to apply separately for that recruitment. The two current vacancies are: Health Action Program Manager in Health Policy, Planning & Evaluation Division: This HPM is responsible for supporting the Health Action Council, a partnership of local leaders, organizations and individuals committed to creating a healthier community through collective action. DHS convenes Health Action as a catalyst to create large-scale social change through cross-sector coordination and collaboration in the areas of Education, Health System, Income/Economic Security. The incumbent will supervise a team responsible for the oversight of program activities and will work with the Health Action Steering Committee to guide Health Action s direction and monitor progress toward the vision of being the healthiest county in California by The HPM will engage a broad spectrum of stakeholders to lead a community dialogue about community health issues, enrich the community s collective understanding of local health issues and solutions, help create a shared vision for community health improvement based on the multiple determinants of health, and offer leadership to develop and implement initiatives and policies to create a healthy community. The HPM is responsible for maintaining communication with the Board of Supervisors and key community partners on goals, priorities and progress; developing and nurturing strategic partnerships; facilitating organization collaboration and providing leadership in ongoing strategic planning, information development, fund development and alignment of community resources. Public Health Program Manager: The HPM assigned to the Quality Improvement (QI) Program in the Public Health Division will focus on developing division wide program

2 Page 2 of 8 systems and processes to address opportunities for quality improvement in the range of programs and services offered in the Public Health Division, including; maternal, child and adolescent health, early childhood development, environmental health and safety, animal care and control, disease control, surveillance and response, and clinical services. The incumbent will build around existing QI frameworks and best practices that will align with requirements for Public Health Accreditation. The HPM will work collaboratively to share successful learning among all the Public Health Sections that will result in increased capacity within Public Health to implement evidence-based practices and continuous quality improvement, to use data for planning and program evaluation, as well as, to communicate effectively about performance management and population health outcomes. The two anticipated HPM vacancies are: Community Transformation Program Manager in the Health Policy, Planning & Evaluation Divison: Sonoma County recently received the highly competitive Community Transformation Grant (CTG), which is one of only 40 for small communities nationwide. The HPM will have primary responsibility for daily oversight and management of this grant. Sonoma County s CTG program provides a high-profile, exclusive, and exciting opportunity to implement broad community-based strategies for addressing health disparities, chronic disease, and clinical prevention services. The incumbent will work closely with policy makers, non-profit organizations, community leaders, and County staff to ensure aggressive health outcomes are achieved through many innovative strategies and programs. The HPM will have primary responsibility for organizing and implementing the strategies and monitoring progress toward program goals and objectives; evaluating performance outcomes; managing contracts and grants to include federal/state reporting requirements; and supervising staff consisting of professional, technical and clerical personnel in assigned programs. Administrative Health Program Manager: The HPM in Administrative Services will support the Sonoma County Health Officer in coordinating public health policy and program initiatives (including direct oversight and support to the Public Health Disease Control, Surveillance, Response and Clinical Services Sections) and in fostering initiatives occurring throughout the department. DHS is seeking to create a healthier community using a wide range of approaches, from addressing the built and food environments, increasing social equity, tobacco control, and optimization of mental health; to traditional public health functions of infectious disease control, environmental health, jail health services and other programs. Over the years, the Health Officer will be seeking to support development of innovative policies and practices that promote health, and further optimize services to the community and to strengthen the department s capacity in these areas. The HPM will provide technical assistance to the Health Officer in working with professional staff throughout DHS; performing research on a wide variety of special projects; preparing reports and plans on strategic activities; assisting in budgeting or grant writing; assisting in preparation of presentations; and communicating with the public. The HPM works with and confers with other DHS personnel, other County department staff, contractors and external partners on health issues in order to forward these activities. Ideal Candidate The ideal HPM candidate for all four assignments described above will have:

3 Page 3 of 8 In depth knowledge of organizational, operational and programmatic concepts and practices applied by public, private and nonprofit agencies and organizations engaged in public health and other health related activities. Proven experience developing and managing programs designed to deliver services with a collaborative approach to community health in countywide and local settings and/or to ensure continual quality improvement in established programs. Aptitude in the application of administrative or analytical methods and techniques necessary for working effectively within a broad population health framework. Ability to develop and deliver oral presentations to a wide variety of audiences that convey complex and technical information effectively. Experience facilitating collaboration and community engagement in planning and program implementation. Success in the fiscal areas of effective budget development/management, fund development, and resource acquisition. Skill in preparing correspondence, reports and other written materials for a range of key stakeholders including local, regional and state policymakers. Graduate-level educational background in public health. Application submissions require the supplemental questionnaire be completed. MINIMUM QUALIFICATIONS Education: Academic coursework which would provide the opportunity to acquire the knowledge and abilities listed. Normally, a Baccalaureate Degree from an accredited college or university or employer approved training in psychology, sociology, counseling, health care administration, public administration, business administration, management, accounting, statistics, personnel or closely related courses would provide such opportunity. A Masters Degree is desirable. Experience: Any combination of experience which would provide the opportunity to acquire the knowledge and abilities listed. Normally, three years of full time, paid experience in a position which provided the opportunity to perform professional clinical, counseling and/or managerial services with a public or private organization engaged in the provision of health services, and /or counseling would provide such and opportunity. Desirable Qualification: Experience in direct first line supervision of professional, administrative and clerical is desirable. License: Some positions within the Health Services Department may require the possession of a professional license in medicine, clinical psychology, clinical social work, marriage, family and child counseling, registered nursing; or specifically related to the division managed. Possession of a valid driver's license at the appropriate level, including special endorsements as required by the State of California, may be required depending upon assignment to perform the essential job functions of the position. Special Qualification: Positions within the Alcohol and Other Drug Services Division must register, obtain, and maintain a Counselor Certification as defined by the California Code of

4 Page 4 of 8 Regulations, Chapter 8: Certification of Alcohol and Other Drug Counselors. Upon appointment to this class, employees will have six months to register and five years to obtain Certification from an ADP approved certifying organization whose curriculum includes a minimum of 240 hours of formal classroom training. Human Resources will waive the Counselor Certification requirement for licensed professionals (or registered interns) who successfully pass a written examination similar to that administered for Certification. This examination will be administered by the Sonoma County Human Resources Department, and will be designed to measure those areas included in the curriculum required for certification. KNOWLEDGE, SKILLS, AND ABILITIES Considerable Knowledge Of: principles and practices relating to the management of a managed care health services delivery system which includes methods to conduct analytical studies related to program improvements; acceptable cost projection methods, acceptable revenue assumptions procedures and accepted customer survey methods; current issues and trends affecting the program(s) under management control. Working Knowledge Of: operations and functions of County government including budget development and personnel management procedures; contemporary principles, practices and theory of management techniques; Federal, State and local laws and regulations pertaining to the corresponding Health Services division; contemporary medical, and/or psychiatric protocols and practice; or psychological effects of chemical dependency; modern education, treatment & rehabilitation techniques used in the appropriate health services program; group dynamics as it relates to public organizations; research methodology, report writing and basic statistics and their application; effective personnel and supervisory techniques; written and oral communications skills, including language mechanics, syntax and English composition; modern office methods and procedures. Ability To: manage a health services delivery system which includes the skill to conduct analytical studies related to program improvements; the skill to develop realistic cost projections, the skill to produce acceptable revenue assumptions and the skill to develop, conduct and analyze customer surveys; establish program goals and measurable objectives and perform acceptable data collection, interpretation and evaluation to assess program performance; understand, interpret and apply Federal, State and local legislation, rules, regulations and ordinances and relate them to the program(s) goals and objectives; adapt currently accepted treatment and/or management philosophies and theories and apply them to program operations; communicate effectively by writing or by presenting formal and informal oral presentations; effectively manage and supervise the work of professional, technical and clerical employees; establish and maintain effective working relationships with County management personnel, employees, community based organizations, the public and others who have an interest in health services; work effectively with the medical and/or psychiatric professionals; use principles of inductive and deductive reasoning to validate recommendations; persuade, rationalize and project consequences of decisions and/or recommendations; analyze administrative problems, budgets, and a variety of program, systems, and procedures. Each applicant will be evaluated based on the following criteria: Relevance of work history, related experience, and achieved level of education and/or training as described in the application and responses to the supplemental questions. Candidates demonstrating possession of the minimum qualifications will be placed on an employment list in order of most qualified to least qualified based on the achieved score received in the Application & Supplemental Questionnaire Appraisal Examination.
