I. REVISION Exhibit C Bid Sheet, has been revised as of June 13, 2018 and attached to this addendum. You must submit your bid on the revised Exhibit C Bid Sheet, dated June 13, II. BELOW PLEASE FIND THE ANSWERS TO QUESTIONS RECEIVED FROM PROSPECTIVE PROPOSERS ON THIS PROJECT: a. Question: Is this a new requirement or is there an incumbent(s)? Question: How much is intended to be spent once the contract is awarded? There is no budget for the contract. j. Question: How many employees currently work under this contract? There are currently 5 to 10 members staffed periodically. k. Question: How many vendors will be awarded as a result of this solicitation? One vendor. l. Question: As a woman owned business, certified by the WBENC and the State of Florida, do we need to subcontract to a MBE as well? No. m. Question: Is subcontracting mandatory? No. n. 3 o. Question: Are suggested changes to the Standard Written Agreement allowed? No. p. Question: Does the chosen vendor need fill all the positions outlined in Exhibit C? Yes. All categories must be proposed and bids without all categories proposed will be deemed non-responsive. Actual hiring will be done on an as-needed basis. q. Question: Is there any additional information we can supply outside of the required forms and Exhibit C, or is the decision purely price driven? There is no additional information required outside of what is requested in ITB- DOT-18/ JG. r. Question: Please confirm that no performance or payment bond is required. No performance or payment bond is required. See Section 12 of the ITB package. s. Question: In Exhibit A, the URL does not work. When clicking on it, copying and pasting, and typing out the URL it brings one to a webpage that states The webpage cannot be found. Please advise where we can view the Broadband Classifications so we can price the positions correctly. Question: Are you open to weekly billing? No. Billing shall be Monthly. See, Exhibit A, Section u. Question: How are DBEs paid? Only the awarded vendor will be paid by check. v. Question: Will you be transitioning current temporary employees if new vendors are chosen? The Vendor must submit the candidates for Department review and approval. w. Question: Do you have any drug testing requirements, and if so, do you administer 5 or 10 panel tests? Can a cup test be administered in our office? Who absorbs the costs? The Department's minimum requirement is for the vendor to abide by the requirements of the form (Form ) provided for the drug testing program. The vendor would absorb the cost of any drug testing program it runs. x. Question: Are there performance guarantees that vendors must meet? Vendor must perform all services covered in Exhibit A, Scope of Services. y. Question: What is your payment method (i.e. check, ACH, credit card)? Check. z. Question: I was looking for the Anticipated DBE Participation Statement. I understand it must be submitted with the bid. Is this a separate form? This contract will not involve DBE participation, but will include MBE utilization. The form for MBE utilization is on page 4 of the bid package. There is nothing preventing the use of a DBE for this project, but DBE program goals are utilized when projects are funded using Work Program funds. This project will be funded with the use of Operating funds. If you would like to include information Page 4 of 9

5 on any DBE groups hired, then please enter this information in the MBE Planned Utilization form ( ). aa. Question: This is regarding Section 9.2 Bidder Qualifications that requires each bidder to submit a notarized letter attesting to the qualifications set forth in Special Condition 9.1. If my company is responding to this bid but as a No Bid response do we still need this notarized letter to be considered? All items requested in the ITB must be provided or the bid will be deemed nonresponsive. bb. Question: If the company submitting the bid is an MBE (state and federally certified) does the company need to complete this form listing itself as the MBE? Yes, please fill out the MBE utilization form in the ITB package with your company s information. cc. Question: What is the evaluation criteria? Total Price in Exhibit C, Bid Sheet. All items requested in the ITB must be provided or the bid will be deemed non-responsive. dd. Question: Is there any pre-defined proposal format of SBBC? Or the vendor can prepare response in their own format? Please use the Exhibit C, Bid Sheet. All items requested in the ITB must be provided or the bid will be deemed non-responsive. ee. Question: Does the department require Vendors experience and qualification details apart from forms and pricing in the proposal? All items requested in the ITB must be provided or the bid will be deemed nonresponsive. See, Section 9.1 through 9.4 of the Invitation to Bid. ff. Question: What are the most frequently used job title in the subject mentioned ITB? Office and Administrative Support has been the most frequently used job title used, but hiring is done on an as-needed Basis. Maintenance & Repair Workers, General, are also a new addition to the job titles used for temporary staffing. gg. Question: What is the average length of the assignment? The length of the assignment will vary depending on the Department's needs. hh. Question: Describe how vendors under contract will receive a fair share of business without vendor rotation of job orders implemented in the procurement process? Page 5 of 9

6 There is only one vendor. No other vendor will be used. ii. Question: Are the Bid responses that are due on June 19th to be submitted online (submitted through the MyFloridaMarketPlace) or as hard copies? Bids are not to be submitted online or via MFMP. All bids must be typed or printed in ink (See Section 21 of the ITB). The bids must be executed and submitted in a sealed envelope (See Section 22 of the ITB). jj. Question: If the bid responses are to be submitted as hard copies, how many copies do we need to provide? One copy of your written bid is sufficient. kk. Question: With exception to Forklifts, can we reject jobs where driving autos / trucks is part of the job description? Attached Logistician Job Description, page 3 - Operating Vehicles and Equipment references driving vehicles. None of the advertised positions will be Operating State Vehicles. All items requested in the ITB must be provided or the bid will be deemed non-responsive. ll. Question: How will time be recorded for hours worked by Temporary Employees? Is there any rounding of time or is it minute to minute? Time will be recorded rounded to the nearest thirty minutes. Ex. If the staff was scheduled to start at 7:30 and he/she clocked in at 7:45, it still will be considered as 7:30 but the Hiring Supervisor will hold them responsible for on time performance. It will be the Hiring Supervisors decision to address nonperformance compliance. mm. Question: ITB Section 11 under Special Conditions - Can we waive the additional insured on our general liability policy requirement? Would the state accept this division of the risk? The requirements of the Invitation to Bid package cannot be changed. All insurances requested must be produced. nn. Question: Do any of the positions require background checks, and if so, what kind of screening needs to be run? Vendor Shall utilize the U.S. Department of Homeland Security s E-Verify system to verify the employment eligibility of all new employees hired by the Vendor/Contractor during the term of the contract in addition to past employment reference check. oo. Question: Page 6 of 9

7 Are there any disqualifying offenses? The failure to be meet the requirements of section 9 of the Invitation to Bid would disqualify a vendor. If a candidate for temporary work pleaded no contest or guilty to any criminal offense, then this would be a reason for disqualification. pp. Question: Standard agreement is listed as an item to submitted in the order of forms, please confirm if the standard written agreement is to be return with bid submittal. The Standard Written Agreement (SWA) does not have to be submitted with the bid package, but the SWA will become a part of the operating agreement with the successful vendor. qq. Question: MBE- We did not see a scoring sheet, are their additional points for MBE utilization as subcontractors. There are no additional points for MBE utilization as subcontractors. rr. Question: On the Office and Administrative Support Occupation Profile it identifies a Level 5 which is not on the Bid Sheet. Should we price Level 5? Office Administration position needed is only up to level 4. There is no need to price Level 5. ss. Question: On the Installation, Maintenance and Repair Workers, General Occupation Profile it identifies Levels 1 & 2. On the Bid Sheet, it identifies Levels 1-3. Should we eliminate Level 3? Level 3 of Section 4.0 ( ) on Exhibit C, Bid Sheet has been deleted. Please see Addendum Number 2. All bidders must use the bid sheet in Addendum Number 2 or they will be deemed non-responsive. tt. Question: Is it mandatory to bid on all categories / sections? All categories must be proposed and bids without all categories proposed will be deemed non-responsive. Exhibit C shall completed. Any line items having zero dollars ($0.00) or left blank will be deemed non-responsive. uu. Question: Are we able to determine the rate of pay that we feel is competitive? If not are there established pay rates from FDOT? It will be the determination of the vendor, but the vendor must comply with all Federal and State laws. Page 7 of 9

Question: Due to the large range of minimum to maximum broadband pay ranges by pay band, are we to assume the job functions required for the temporary staffing will be correlated to the minimum end of the range for each band/level? It will be the determination of the vendor. ww. Question: Unable to find a pay band correlated to Level 3 of position ; Maintenance and Repair Workers, General. Would pay band 006 be a proper assumption or is this an oversight in the Exhibit C bid sheet? Level 3 of Section 4.0 ( ) on Exhibit C, Bid Sheet has been deleted. Please see Addendum Number 2. All bidders must use the bid sheet in Addendum Number 2 or they will be deemed non-responsive. xx. Question: How many temporary positions by classification were used in the past 12 months? For the past 12 months, the following positions have been filled: Logisticians ( ): 2 Accountant & Auditors ( ): 2 Office of Administrative Support ( ): 9 Maintenance & Repair Workers, General: 0 (Newly added classification) Note: During the entirety of the Contract, the Department cannot and does not guarantee the same number of positions per classifications will be requested or hired. Demand for any type of temporary work is variable and highly dependent upon the needs of the Department and the availability of funds. yy. Question: What is the average duration of the temporary positions? The length of the assignment will vary depending on the Department's needs, but most hires are still active from their time of their initial hire until the end of the current contract. zz. Question: How many temporary procurement contractors did the Department request during the past year? There are currently 5 to 10 members staffed periodically. aaa. Question: What is the required response time from when Department makes a request for a temporary contractor to when a vendor is expected provide candidates? Maximum of two weeks for new employee and refer to section for a replacement staff. Page 8 of 9

9 bbb. Question: What is Department s process for requesting candidates from multiple vendors? There is only one awarded vendor for this Contract. ccc. Question: Does this solicitation require resumes? If so how many? Resumes will be required for every proposed candidate for temporary work. Bidders must follow Section 9 of the Invitation to Bid. ddd. Question: Does this solicitation require references? If so how many? A minimum of three references will be required for every proposed candidate for temporary work. Bidders must follow Section 9 of the Invitation to Bid. eee. Question: Is it mandatory to pay minimum wages as per PAY grade and classification specified in the bid sheet? It will be the determination of the vendor, but the vendor must comply with all Federal and State laws. fff. Question: Does wage determination applicable on a resulting contract? No wage determination has been made. The Department will use the rates set forth by the winning bidder in Exhibit C, Bid Sheet. ggg. Question: Can you provide an estimate of volume needed, either by headcount or manhours budgeted, for each position? No. Hiring will be done on an as-needed basis. hhh. Question: Can you provide a number of positions filled historically in the requested positions? Past 1, 2 or 3 years? We are able to provide information on hiring for the previous year. Please see xx, above. Revised documents are posted on VBS. All other documents of the subject project remain unchanged. Failure to file a Protest within the time prescribed in Section (3), Florida Statutes, or failure to post the bond or other security required by law within the time allowed for filing a bond shall constitute a waiver of proceedings under Chapter 120, Florida Statutes.
