5 The agency is responsible for the supervision of the execution and enforcement of the provisions of the state s i i g laws a d rules. Pri e consideration is given to the protection of the safety and health of persons employed within the mining industry in the state of WV. In addition the agency protects and preserves mining property and property used in connection with mining activities.

7 2014 FATAL MINING ACCIDENTS 1 January 16, 2014 Mettiki Coal Company, E- Mine, (U ) Tucker County, WV The Office of MHST is investigating the accident. 2 April 18, 2014 Allied Barton Security, (C ), Law River Co., LLC, Crown Hill Dock (L ) Kanawha County, WV Security Guard drowned in sediment pond. Office of MHST is investigating the accident. 3-4 May 12, 2014 Patriot Coal Company, Brody #1 Mine (U A) Boone County, WV A coal outburst occurred resulting in a double fatality, the continuous miner operator and the roof bolter operator who was operating the MRS unit were fatally injured. The accident occurred on the #1 section in the #6 entry during second mining. 5 November 10, 2014 Red Bone Mining Company, Crawdad #1 Portal B (U007683B-A) Monongalia County, WV A section foreman was fatally injured after being struck by a piece of rock while operating a roof bolting machine. The Office of MHST is investigating the accident.

22 The WVOMHST and the WV University Mining Extensions have set up a partnership to develop a comprehensive training manual for inspectors. All supervisors have attended a basic management class. All inspectors have attended the following classes: 3hr. Violation writing class Basic First-Aid 3hr. Fire Training 4hr. Coal-Dust Practices Survey Program Class Ventilation and Roof Control Classes

23 All regions have monthly staff meetings All Surface Inspectors state wide meet bimonthly. All Electrical Inspectors state wide meet Quarterly. All Diesel Inspectors state wide meet twice a year. All regions have Ventilation Inspectors and meet regularly.

27 DRUG POLICY SINCE JANUARY 1, CERTIFICATIONS SUSPENDED. 88 POSSESSED FOREMAN CERTIFICATIONS 22 WERE ACTING AS SUPERVISORS 45 WERE COVERED UNDER A COLLECTIVE BARGAINING AGREEMENT 251 INDIVIDUALS HAVE BEEN SUSPENDED FOR 3 YEARS BY THE APPEALS BOARD FOR NOT TAKING ACTION AFTER RECEIVING THE LETTER OF TEMPORARY SUSPENSION 104 INDIVIDUALS HAVE BEEN REINSTATED 62 ARE CURRENTLY IN A TREATMENT PLAN 4 HAVE BEEN PERMENENTLY REVOKED FOR FAILING A 2ND SUBSTANCE ABUSE TEST PRESCRIPTION DRUGS ARE THE PRIMARY CAUSE OF FAILED TESTS MARIJUANA IS SECOND OF THE 641 FAILED TESTS SINCE JANUARY 1, OCCURRED IN 2014 INDIVIDUALS HAVE HAD THEIR MINING

HAVE ASSESSMENT CONFERENCES CURRENT BY DECEMBER 31, ADDITIONAL / SPECIALIZED TRAINING FOR INSPECTORS

41 I sure that the age cy s ai objective is one that we all share. That is to mine coal while making sure that every employee returns safely home to their family at the end of their shift. The ideal goal is a workplace free of injuries and fatalities.
