OUTLINE OF POST: The Community Links York Families Wellbeing team provides flexible, supportive, responsive housing and mental health support to Families in the community within the context of the York Early Help Local Area Teams (LATs). The role of the LAT Families Support Worker is to support and enable service users to live happily, successfully and securely within their home and community meet their day to day housing and mental health needs. This will involve working 1:2:1 and delivering goal orientated support plans. Workers should use a whole family approach and creative, empowering interventions to support the families they are working with. They will be expected to support with addressing debt and budgeting problems, ASB, claiming the correct benefits, accessing training, education and employment, social isolation, inter-family relationships and accessing physical or mental health support. They will support families to become more resilient and have the confidence to move their lives in positive directions to meet their personal goals. They do this by building trust, working creatively and encouraging positive risk taking. All this should be done within the wider context of being part of the LATs. We support service users in their homes and in the local community.

2 MAIN TASKS: 1. To work positively and collaboratively with the York Local Area Teams 2. To manage and prioritise a caseload of families with a housing need. 3. To positively support families with their day to day financial, health, and tenancy-related needs as identified in their support plan. 4. Take all reasonable precautions to ensure the health and safety of service users and self, understanding and following risk assessments as instructed. 5. To understand use the principles and framework of York Early Help assessment and strategy. 6. To understand and use the principles and framework for the assessment of Children in Need. 7. To work proactively and collaboratively with service users, their families and any other professional agencies involved. 8. To help prevent the escalation of support needs of a family. 9. To either lead or collaborate with a multiagency team as a part of a FEHA. 10. To ensure that client records and other information systems are completed accurately and within agreed timescales. 11. To ensure that support undertaken is meaningful, respectful, promotes selfdetermination and is in line with the individual support plan. 12. To adhere to organisational policies and procedures relating to risk and personal safety. 13. To contribute to the achievement of annually set individual and team targets. 14. To participate in regular supervision. 15. To represent Community Links in a knowledgeable and professional manner at all times. 16. To maintain appropriate professional boundaries at all times. 17. To identify own training and development needs in conjunction with your line manager and participate in training opportunities as directed. 18. To partake in Community Links Personal Development Review system.

3 19. Any other duties commensurate with the grade and level of responsibility of this post.
