3 Sugrema Consultancy N.V. 3 FALL CONTROL PROGRAM Setup: An organized, site specific, written document comprising a fall protection policy, rules and procedures for elimination and control of identified fall hazards Establishes minimum fall protection / prevention practices for workers exposed to heights of six feet or greater and at lower heights where potentially hazardous conditions exist (floor openings, pits, slips, trips etc.) The purpose is to establish safe work practices and thereby reduce the risk of injury from a fall 3

Prerequisite : Completion of Ladder safety training Topics : Objective Levels of fall protection Fall hazards Fall protection Applications Hazards of fall arrest systems Planning the use of fall arrest systems Harness donning Inspection Cleaning and storage Duration : 1 day. Number of participants : 15 max. Remark : A review of fall arrest equipment in use is required 1 week before the training. A selection will be made which equipment should be available during the training. If necessary, the training package will be adjusted. 8

Prerequisite : Completion of Fall Authorized person training and Ladder safety training. 10 TRAINING Scaffold builders and users training Sugrema Consultancy N.V. 10 Intended for those whose job responsibilities require them to build and work on scaffolds ( Minimum LTS level ). Prerequisite : Completion of Fall Authorized person, Fall competent person, Ladder safety and Scaffold builders and users training Topics : Introduction Definitions General requirements Requirements for specific types of scaffolds Aerial lifts Crane suspended platforms Planning Rescue Methods of controlling the hazard Duration : 2 days. 12 Sugrema Consultancy N.V. 12 TRAINING Program and training are based on OSHA regulations All training sessions are interactive sessions with computer presentations, including pictures Except for the Fall awareness and Inspectors training : Video material is included in all training sessions A written knowledge test will be taken at the end of the training. A score 80% is required to pass the test. 12

13 Sugrema Consultancy N.V. 13 IDENTIFY HAZARDS Identify the areas / jobs / tasks with the potential or history of falls and to determine the specific hazards at the areas / jobs / tasks. Review Job Safety Analysis (JSA), Standard Work Instructions (SWI), Standard Operating Procedures (SOP) etc. 13

14 Sugrema Consultancy N.V. 14 PERFORM RISK ASSESSMENT Critically consider and appraise the hazards found during the identification process. This is done by assessing the risk of the fall hazard for each job or task. 14

15 Sugrema Consultancy N.V. 15 PRIORITIZE HAZARDS Prioritize the hazards based on the risk assessment process. This will provide a management tool to make the proper decisions to reduce the risk. 15

16 DEVELOP HAZARD CONTROL STRATEGY Analyze the causes of the hazard Sugrema Consultancy N.V. 16 determine the root cause of the hazard Generate potential solutions list all the possible solutions to the hazard Evaluate and select the solution to select the best solution for addressing the hazard after looking at all sides of the solutions, including the possible consequences for equipment operation and maintenance Test the effectiveness of the solution verify that the selected solution has the desired impact 16

17 Sugrema Consultancy N.V. 17 IMPLEMENT HAZARD CONTROL STRATEGY This step encompasses all phases of completing the solution, such as education and training, procedural changes, engineering, the completion target date etc. 17

18 Sugrema Consultancy N.V. 18 AUDIT FALL CONTROL PROGRAM The purpose of an audit is to provide a tool, necessary to ensure that fall hazard control processes are properly implemented, maintained, monitored, measured and continuously improved. Audits will : Identify the strengths and weaknesses of existing fall hazard control systems, as well as, provide the road map to get the next level of performance Provide a pro-active, before the fact, measurement of fall hazard control systems. This is of critical importance given the potential severity of injuries associated with falls. 18
