1 GENERAL SAFETY INDUCTION PINDARI WA Pty Ltd Welcome to PINDARI WA At PINDARI WA we value the Health & Safety of all employees and customers. This General Induction is designed to assist you in working safely during your time as an employee.

Pindari WA First established in 1997 Evolved into Pindari WA in Head office; Dunsborough Regional office; Point Samson Contracted to supply labour and project works for Rio Tinto and BHP What we do Supply qualified trades to Rio Tinto & BHP sites Projects that require; electrical, plumbing, painting and carpentry Employ approx. 60 staff in the following roles: Minor works; refurbishment and construction jobs Backfill labour hire 2

6 HEALTH & SAFETY POLICY At PINDARI WA our Health and Safety Policy is based on a belief that the well-being of people employed at work, or people affected by our work, is a major priority and must be considered during all work performed on our behalf and that of our clients. The main objectives of our Health and Safety Policy are: To achieve an accident-free workplace To make health and safety an integral part of every position To ensure health and safety is considered in all planning and work activities To involve our employees in the decision-making processes through regular communication, consultation, and training To identify, manage, and control all potential hazards in the workplace through hazard identification and risk analysis To ensure all potential accidents and incidents are controlled and prevented Comply with relevant occupational health and safety (OHS) laws, regulations, industry codes of practices, and project requirements specified by clients or regulators. 6

7 SAFETY & THE LAW Most workplaces in WA are covered by state & Federal Legislation. The legislation includes: The Mines Safety Inspection Act (MSIA) or; The Occupational Health & Safety Act. This legislation sets out the responsibilities of both Employees & Employers. This is known as; DUTY OF CARE 7

8 DUTY OF CARE Employers must so far as is practicable, provide and maintain a safe system of work where employees are not exposed to hazards In short, Employers must provide and maintain: Mechanisms for identifying workplace hazards Safe workplaces & Equipment Adequate training and PPE as required Systems for monitoring Safety and compliance Adequate supervision & instruction Compliance with statutory obligations

9 DUTY OF CARE Employees must take reasonable care to avoid affecting their own health or the health and safety of others through any ACT OR OMISSION In short, Employees are expected to: Act in a responsible and safe manner at all times Avoid affecting their own health and safety, or the health & safety of others Follow work instructions and procedures Wear & maintain provided PPE Report Incidents or hazards Operate machinery in a safe & responsible manner Comply with company safety & environmental policies

12 INCIDENT REPORTING It is a mandatory that incidents are reported to your supervisor in the following (maximum) time frames: Near Miss First Aid Injury Medical Treatment Injury Lost Time Injury Fatality (8 Hours) (8 Hours) (1 Hour) (15 Minutes) (15 minutes) Supervisors must in turn report these incidents to their Managers.

13 SAFETY TOOLS We use various tools to identify hazards in the workplace. They may include: Job Hazard Analysis Work site Hazard Checklists Pre-start and Tool Box Meetings These Tools Are to Be Used by Staff To: Identify workplace Hazards Make the changes necessary to eliminate or reduce the risks on work sites

14 WHY DO WE IDENTIFY HAZARDS AND ASSESS THEIR RISK? To eliminate or control risks to PEOPLE (i.e. employees, contractors, public) To ensure everyone on site is AWARE of the hazards and the control measures that we are taking To DOCUMENT the hazards we have identified and work out the best control measures ASSESS THE RISK MAKE THE CHANGES! 14

15 PERSONAL PROTECTIVE EQUIPMENT Some Client sites require different Personal Protective Equipment. (PPE) At PINDARI WA we require as a minimum: Steel Cap Boots Long Sleeved Cotton Shirt Other PPE that may be required according to site conditions / regulations; Wide brim hat & sunscreen Long pants and shirt Hard Hat & Safety Glasses Hearing Protection Face Shield / respirator Gloves If you are unsure ask your supervisor

16 PERSONAL PROTECTIVE EQUIPMENT Required PPE will be detailed in Permits to Work, JHA s and Take 5 s Additional training and specialist PPE is required for high risk work, for example; Heights Confined spaces Hot works Hazardous chemicals and materials Trenching and excavation Isolation procedures Fix and mobile plant Vehicles and driving And as such specific PPE and work practices will be required depending on work site, location, local conditions (weather) and any applicable regulations; If you are unsure ask your supervisor!

17 DANGEROUS GOODS Dangerous goods are solids, liquids, or gases that can harm people, other living organisms, property, or the environment. Usually subject to specific chemical regulations (MSDS s). Dangerous goods are more commonly known as hazardous materials (HAZMAT or HazMat) Dangerous goods include materials that are: radioactive, flammable, explosive, corrosive, oxidizing, asphyxiating, biohazardous, toxic, pathogenic, or allergenic. Also included are physical conditions such as compressed gases and liquids, hot materials, all goods containing such materials or chemicals, or may have other characteristics that render them hazardous in specific circumstances. 17

18 PERMITS TO WORK Many work sites require permits to be issued before you start work. The types of permits may include: Authority to Work Permit Confined Space Permit. Hot Work Permit Excavation Permit. Vegetation Clearing Permit. Isolation Permit Vicinity Permit. Check with your supervisor before starting work 18

19 Mobile phones & Ipods on Site Personal Mobile phones & Ipods are not to be used on client sites Mobile phones & Ipods are not permitted to be used on work sites near moving machinery On some sites you may not use mobile phones when driving regardless if hands free are fitted. Operators of equipment must not use mobile phones & Ipods when operating equipment Keep phone calls near work sites to a short sharp and basic nature Do not use your mobile phone whilst driving under any circumstances. 19

20 Drug & Alcohol POLICY What do you need to know? It is against PINDARI WA policy to be under the influence of, or to sell, distribute or possess alcohol, narcotics, depressants, stimulants, hallucinogens, marijuana and any other mind altering drugs, when reporting for work, or while working for PINDARI WA is the allowable B.A.C. on most sites Random D & A testing is conducted at most sites Medications can affect your ability to perform your work safely. There may be severe consequences for breaching the policy Look after your mates be drug & alcohol free

21 SMOKING IN THE WORKPLACE Smoking is not permitted on sites where flammable goods, gases or materials may be present. Smoking is not permitted in company vehicles, offices, workshops or confined spaces Where provided, designated smoking areas should be used. For the sake of the environment please dispose of butts thoughtfully.

22 FITNESS FOR WORK PINDARI WA has a FITNESS FOR WORK procedure to ensure all staff are in the best possible state (physical, mental and emotional) to complete their work competently without compromising safety. What do I need to know? You have a duty of care to be fit for work to ensure you do not put yourself or others at risk Fitness for work can be compromised by; fatigue, tiredness, stress, drug and alcohol use Fitness for work is assessed by a deterioration in work performance and targeted screening Pindari has policies and procedures in place to guide employees and help maintain fitness at work. The Employee Assistance Program provides access to independent help. Please contact your Supervisor for details.

23 FATIGUE MANAGEMENT PINDARI WA has a Fatigue Management Policy designed to prevent injuries and incidents by reducing effects of fatigue on staff at all levels: What do I need to know? Maximum continuous active work time is... 3 hours Maximum working time in 14 days is hours Minimum break between shifts is hours Maximum working time in 24 hours is hours These guidelines should not be breached without the permission of your Manager

24 ELECTRICAL LEADS & TOOLS ELECTRICAL TAGS JAN MARCH RED APRIL JUNE GREEN JULY SEPT. BLUE OCT DEC. YELLOW Electrical leads and power tools must be inspected every time they are used. All portable power tools and extension leads must be inspected and tagged by a competent electrician every three months Safety regulations stipulate the compulsory use of Earth Leakage Circuit Breakers (ELCB s) on portable equipment Visually check leads, guards, sockets & plugs before using electrical equipment.

25 MANUAL HANDLING Many injuries result from poor manual handling. Think about the task and plan the job Can mechanical means be used? Can help be given by others? Is the path clear? Are correct lifting technique being used? STOP Take five and think about the task!

26 ISOLATION LOCKOUT - TAGOUT Tag out through the use of the Danger Tag or Do Not Start Tag or Out Of Service Tag is designed to provide protection to personnel where there is the risk of injury from the operation of faulty or unsafe plant. You MUST; Place all Danger Tags before you begin work Fill out the tag legibly with the required details Securely attach the tag, making sure that tag is clearly visible When working with other employees on the same job, each employee must attach their own Danger Tag Remove the Danger Tag only when the job has been completed and the item of plant is safe to use 26

27 VEHCILE & MACHINERY CHECKS Vehicles & must be inspected DAILY to ensure they are safe to drive / operate. You will be required to: Check your vehicle regularly Record and advise of urgent vehicle repairs Maintain the vehicle in a safe manner Unsafe items must be tagged / locked and reported to the Supervisor, who will arrange for maintenance or repairs Do not operate unsafe machinery 27

28 HEAT STRESS How do I avoid dehydration / heat stress? Drink small amounts of cool (not freezing) water regularly Use sunscreen, and wear a wide brimmed hat Take regular breaks during extreme heat Report symptoms of heat stress immediately Do not go off by yourself if dehydrated Avoid excessive intake of Tea / Coffee or alcohol the night before If your urine becomes cloudy and brown you should report it immediately and rest. Sip on water in the shade Watch for changes in behaviour in others

29 HAZARDOUS CHEMICALS & MAERIALS MSDS s are a valuable tool for identifying : What the hazards / health risks of a materials are? What PPE should be worn? Safe handling, storage, and transportation procedures? What to do if there is a spill, bodily contact or a reaction to the product? If you are required to use or transport a product you must read, understand, and carry the correct MSDS at all times. If you take hazardous materials to site you must take a copy of the MSDS with you.

30 WORKING AT HEIGHTS Working at heights can be a major source of injury and death. Where you can fall from height we must control the risks. Working at heights include: Ladders Scaffolding Elevated Work platforms Where you can fall from one level to another Considerations include: PPE - Safety harnesses and lifelines Signs, barricades, perimeter guarding. Before working at Heights think about what could happen if you fall. 30

31 Working in CONFINED SPACES A confined space is any space that has limited access and egress and is at risk from engulfment or may have limited breathable air. To work in a confined space you must be fully trained. You must have a confined space permit. You must consider how staff may be rescued. Most sites have signs on confined spaces to help you identify them clearly. 31

32 HOUSEKEEPING Many injuries occur due to slips/ trip & falls caused by poor housekeeping. Keep you work area clean & tidy. Place rubbish in bins Remove unwanted debris and equipment Signpost or bunt off your work area. Remove leads and hoses when not in use. Check the area of work for trip hazards. 32

33 FIRE EXTINGUISHERS Many work sites require you to carry fire extinguishers in vehicles.: Fire Extinguishers must be inspected every six months. Report any out of date extinguishers.. Do not use a fire extinguisher unless there is a fire. Do not attempt to fight a fire unless you are sure you can extinguish it easily. Make sure you know where the fire extinguishers are located in your work area. 33

34 TOOL BOX MEETINGS Tool box meetings are held regularly to: Discuss Safety / Environmental Issues. Promote safety issues and training. Talk about how things can be done to help staff work more safely. Solve problems in an open manner. Allow managers & staff to discuss safety issues. Please contribute to these meetings. They will make us a better business. 34

35 TRAVELING TO A REMOTE SITE Make sure you adhere to the following; Receive thorough briefing of route Brief staff of ETA If lost / broken down stay with vehicle Ensure you take a minimum of 20 lts of water Managing Director will send a vehicle along the route 12 hrs after no contact. Call office on arrival and departure from site Always take your JHA s with you! 35

36 WHEN YOU GET TO SITE Make sure you identify: The emergency contact numbers The muster point The location of the first aid station Who you must report to If you require work permits If you require any inductions The locations of the fire extinguishers and emergency showers. Always take your JHA s with you! 36

37 DON T FORGET Your commitment to quality and safety is defined by your work Your Safety is our number one priority With each task, plan the work, assess the risks with regard to Safety and the Environment Report all Safety and Environmental incidents Use your employee login (Pindari website) for more safety info Ask if you are not sure 37
