2 ACCIDENT INVESTIGATION Fact or Fiction? ACCIDENT INVESTIGATION When investigating, understand the difference between unsafe acts and unsafe conditions! Examples? Unsafe Acts vs. Unsafe Conditions Unsafe Act 85% % of accidents Unsafe Condition 15% Too much time is spent looking for unsafe conditions 2

3 Have a Plan What gets investigated Who does the investigation Training Reporting Timeliness Recommendation follow up WHICH ACCIDENTS TO INVESTIGATE? Major accidents? All accidents! Slip/trip vs Fall, Cut vs. Amputation, Muscle strain Avoid the no blood no foul mentality Near Hits WHO SHOULD INVESTIGATE? Employee Supervisors Another member of management Safety committee Human Resources Safety/Quality There are pros and cons to who investigates. It s key that whomever is chosen works for your organization. 3

4 WHEN TO INVESTIGATE? Immediately after the accident Facts are fresh Scene of accident not as likely to be disturbed Employees see that management is serious about preventing accidents Investigation Method At the scene Collect information Determine the causes of the accident Write the report Management review Take corrective actions Monitor At the scene Check for danger Get the injured help Secure the scene Identify and separate witnesses Preserve the scene Video and photographs 4

5 Collect Information Retain or document physical evidence Positions of equipment/furniture Equipment identification numbers Equipment logs Housekeeping Contaminants on floor Take lots of pictures Draw the scene Collect Information Review policies/procedures/training Determine sequence of events Why did action need to occur What should the sequence of events been Interview Witnesses Discuss the accident with injured person and any witnesses Put them at ease Not fault finding but fact finding so that a similar accident can be prevented Interview privately if possible Be considerate, compassionate, don t embarrass or scold Helps reduce fraudulent claims 5

6 INVESTIGATION METHOD Always conduct the investigation at the injury site, not in the office! Ask the employee to show you what they were doing Get input from employee on how this could be prevented in the future INVESTIGATION METHOD When investigating consider 4 factors: Equipment/Material Environment People Process/Procedure Equipment/Material??? Right tool for the job? Condition/maintenance? Location? Easy to use? Too heavy or awkward? 6

7 Environment??? Inside/outside? Wet/dry? Foggy/clear? Water/Ice? Noisy/quiet? Well lit or dark? People?? Right person for the job? Proper skills? Job knowledge? Adequately trained? Responsible for task? Motivated? Process/Procedure?? Was the correct method followed? Is it easy to understand? Can it be simplified or improved? Does it require more than one person? When should it be done? 7

8 Determine the causes Linear model (domino theory) Complex linear model/human Factors Theory Five whys (a type of root cause analysis) Complex non linear model Several others INVESTIGATION METHOD Use the 4 factors with the W questions Why is it necessary? What is its purpose? Where should it be done? When should it be done? Who is best qualified to do it? How should it be done? 8

9 Make Recommendations Have a formal reporting process. Who received your accident investigation What happens to it What action is taken to prevent reoccurrence. Who receives the report Key employees Committee Management Enforce recommendations See your corrections through Set policy from your findings The investigation is only as good as what you do with it. 9
