2 Wellness Rewards 2018 Overview 2018 Program Requirements: Wellness Profile Annual Physical Exam Earn an additional 200 points* (Must complete all of the above to earn incentive.) Incentive: $125 or a day off work Note: If you work for a department that operates 24/7, you are eligible for $125 only. Program Deadline: 9/30/18 *Health and Wellness Program/Activity Options Include: Drop 10 in 10, How To Be Tobacco Free, Time to Sleep Well, Aim for Change, Daily Steps to Less Stress, Disease Management, I.O. ME Campaigns (Real Meals, Mind Over Stress, Get Moving, Quit Tobacco), Preventive Exams, Flu Shot, Digital Health Assistant, Daily Victory and Weigh Today apps, Health Trackers, Organized Walk/Race 2

3 Logon to the Highmark Website Go to (If you have not accessed the website in the past, you will need to take a few moments to register on the site). Click on the Register tab at the top of the webpage. If you are already a registered member, click Log In. 3

4 Logon to the Highmark Website If you are already a registered member, enter your Login ID and Password in the Login box and click LOGIN. (then skip to page 7) OR If you have not yet registered, click Register as mentioned on the previous page. (then turn to page 5) 4

5 Complete Registration (New Users) Enter Your member Information. Create a unique Login ID and Password. Then select and answer a security question. Click Next. Follow the next steps to complete the registration process. 5

6 Take the Wellness Profile If this is your 1 st time logging in to your rewards program you will be prompted to answer these questions. To Accept the No response click Continue. Read the Wellness Center Agreement and click the box if you have read and agree with the Terms & Conditions and Privacy Policy items. Click Go. 6

7 Take the Wellness Profile Choose any topics that are of interest to you. You will receive more information on the topics you choose. Click Go. 7

8 Access Rewards & Take the Wellness Profile* Click on the Rewards Program link under Wellness Programs. *If you have questions or need assistance with the Wellness Profile or Wellness Rewards program, call the Wellness Resource Center at

9 My Reward: View/Complete Required Activities The Wellness Profile and Annual Physical Exam are required activities to be eligible for the incentive. Wellness Profile: From the Rewards Landing Page click Do It next to the Wellness Profile Activity to access and complete. Annual Physical Exam: Annual physicals must be completed between 10/1/17-9/30/18. Credit for this exam will be given based on claims submission by your physician. Please contact the member services number on the back of your ID card if you do not see credit after 8 weeks of your exam date. 9

10 Take the Wellness Profile Click on Take It Now to directly access the Wellness Profile. 10

11 Taking the Wellness Profile If you have previously completed the newly updated wellness profile you will be taken to this screen. Click on Update Score to continue. 11

12 Take the Wellness Profile To retake the profile you must click on Review Answers. 12

13 Take the Wellness Profile Answer the questions as best you can the more information you provide, the more accurate your personal report will be. You will need to answer every question in each section to move to the next page and complete the Wellness Profile. You may save your wellness profile answers and return at another time to finish. 13

14 Update Your Contact Information Be sure to update your contact information. You may be eligible for one or more of our free health and wellness management programs. This information may be used to connect you about these resources. 14

16 Navigation Navigate back to the Rewards landing page at any time by clicking on the Rewards link at the top of the page. 16

17 Choose Your Activities to Earn 200 Points Enroll in a Comprehensive Wellness Lifestyle Program Go to the Rewards landing page to view the Wellness Coaching Programs available. Earn 25 points by enrolling and setting a goal in any of the Wellness Lifestyle programs. Note: you can enroll in (and earn points) more than one program. Call the Blues on Call number on the back of your membership card to enroll. 17

18 Choose Your Activities to Earn 200 Points Enroll in a Disease Management Program Earn 25 points by enrolling in a disease management program. Call the Blues on Call number on the back of your membership card to enroll. 18

19 Choose Your Activities to Earn 200 Points Additional Activities Go to the Rewards landing page to view the variety of additional activities you can complete to earn points. For activities such as preventive exams that you can self-certify completion, click Certify Here to complete. Further instructions and links to access tools and programs are included on the Rewards page. 19

20 Navigation Navigate back to the Rewards landing page at any time by clicking on the Rewards link at the top of the page. Upon full completion of the program, you will receive a confirmation . You also will see a Hooray! message and your confirmation code on the Rewards Landing page. 20
