1 Shelbyville High School National Honor Society Guidelines for the Application Packet for Membership Thank you for your interest in National Honor Society. These guidelines are designed to walk you through the candidate s packet for membership to the Shelbyville Chapter of NHS, which directly follows these directions: The candidate s packet is divided into six sections, all of which revolve around three of the four pillars of NHS: Service, Leadership, and Character. The pillar of Scholarship (cum. GPA 3.667) has already been considered and was the basis for your invitation. Section I should be read thoroughly, as it outlines the standards to which you will be held accountable if accepted as a member. Sections II-VI are the main components of the packet. Please refer to the following guidelines for a detailed description of how to complete these sections. All sections of the packet must be typed or very clearly written. After completing the packet, please remove these guidelines (pages 1-2) from the packet and only turn in the application. I. Preliminary Information (explained in detail on page 3) These are the standards to which you will be held accountable if accepted as a member of National Honor Society. Before continuing with the packet be sure that you have read and understand Sections I. Once you have done this, sign at the bottom of the page, signifying that you understand and agree to the outlined policies. II. Leadership This section regards your leadership during your high school years. Participation in a conference or membership in a club, sport, or organization, by itself, does not necessarily indicate leadership. A leader is responsible for planning and coordinating an activity, delegating responsibility, and ensuring a task is completed with a high degree of excellence using the assistance of all involved. Therefore, do not record service activities as leadership activities unless you have taken a clear, active leadership role in the activity. Unlike with service (section III), you can include paid work experience, as long as it displays leadership. You are given four boxes to fill in with information regarding your leadership activities. You are not required to use all of them, but you may add more if needed. Fill in the following information: Activity: The name of the activity Total Hours: Approximately how many total hours you devoted to this role Dates: Approximate dates of this leadership role; include the year Leadership Title: Examples: secretary, captain, committee chairperson, etc. Sponsor s signature: Do not have another person sign for the actual sponsor and do NOT submit a form with a forged signature! If you are unable to attain the signature, please give a phone number at which the individual may be reached. Strong examples of leadership: Officer or captain of an organization or team, planning and running an event, working in a role where you are responsible for teaching or training others, and coaching. 1

2 III. Service This section concerns your involvement in service activities for your high school years. Service is doing what you can for others, without expectation of benefit for yourself. Examples include Key Club projects, volunteer service organization projects, volunteer projects through church or Scouts, service clubs at SHS. Eligible service hours should be done outside of school, and should not include projects done with your teachers during the school day. There are six boxes to fill in with information. You are not required to fill in all of them, but you may add more if needed. It is recommended that you have a minimum of 20 hours to be considered for membership. The selection committee considers both the quantity and quality of service. If you have service activities that you can group together (because they were done through the same organization, like Key Club or a church, or because they were the same or nearly the same activity), please look at them as one activity when filling in the boxes. Fill in the following information: Activity: Circle whether it is community service or school service and provide the name of the activity (Ex: if you grouped many Key Club events together, instead of writing individual event names, write Key Club ) Dates: Provide specific dates or a range of dates during which the service occurred Hours: Record the total hours for all the service projects you did that fall into this box Sponsor s signature Be sure to obtain the actual signature from the sponsor; do not forge the signature or have someone besides the sponsor sign. If you are absolutely unable to attain the signature, please give a phone number at which the individual may be reached. IV. Extra-curricular Involvement Inside the boxes, list your extracurricular involvement, including any awards, special recognition, honors, or scholarships. Only use the space provided. V. Recommendation Forms One form must be completed by a faculty or staff member of SHS regarding your character and leadership. These forms will be returned by the writer to Miss Smith s mailbox. Another form must be filled out by a non-academic/community individual, who is NOT SHS staff member, family member, or friend. It must be completed by an adult who is very familiar with your leadership and your character. Please provide the writer with a stamped envelope addressed to Shelbyville High School, 1001 W. N. 6 th St., Shelbyville, IL 62565, Attn: Rebekah Smith. The Recommendation Form must relate to your leadership and/or character direct your writers to provide specific examples.. VI. Character Essay Write an essay (1.5-2 pages, single-spaced Times New Roman 12) answering Why should I be selected as a member of the Shelbyville Chapter of the National Honor Society? Please include in your essay three separate character traits that define you and provide a description of an event or activity that illustrates one of these traits. You should have a separate body paragraph for each character trait. National Honor Society Selection Guidelines Character Takes criticism willingly and accepts recommendations graciously Constantly exemplifies desirable qualities of personality (modesty, cheerfulness, friendliness, and poise) Cooperates by complying with school regulations concerning property, programs, office, halls, etc. Demonstrates highest standards of honesty and reliability shows courtesy, concern and respect for others Has powers of concentration and sustained attention as shown by perseverance and application to studies Actively helps to rid the school of bad influences or environment Observes instructions and rules, punctuality, and faithfulness in obligation both inside and outside of the classroom (without instances of school/civic infractions) Manifests truthfulness in acknowledging obedience of rules, avoiding cheating in written work and showing unwillingness to profit by the mistake of others (no discipline file, suspensions, or criminal record) Service Willingness to uphold scholarship and maintain a loyal school attitude Participates in some Outside-of-School Service Activity ie. Girl Scouts, Boy Scouts, church groups, etc. Dependable volunteer that provides well-organized assistance; is available on his/her own time and is sacrificing without compensation including grades or pay Works well with others and is willing to take on difficult or inconspicuous responsibilities Willingness to render cheerfully and enthusiastically a requested service to school Willingness to respect his/her class or school in interclass and interscholastic competition Readiness to show courtesy by assisting visitors, teachers or students Leadership Is resourceful in proposing new problems, applying principles and making suggestions Demonstrates leadership in promoting school activities, exercises influence on peers in upholding school ideals, organizes and motivates peers to achieve a common goal Contributes ideas that improve the civic life of the school Is able to delegate responsibilities Exemplifies positive qualities and inspires positive behavior in others Demonstrates academic initiative and displays leadership in classroom, at work, and in school activities Successfully holds school offices/positions of responsibility, conducts business efficiently and effectively, and is reliable and dependable without prodding Is thoroughly dependable in any responsibility he/she accepts 3

4 If accepted for membership in the National Honor Society, I agree to the following: I will participate in all NHS meetings. I will participate in all NHS activities and maintain the minimum number of services hours for juniors and seniors as recommended by Shelbyville High School. I will not let my grade point average fall below a cumulative any quarter. I will abide by all school, local, state and federal laws. I will attend the Induction Rehearsal and /or Induction Ceremony. I will also sign a Commitment form (Code of Conduct) for the Shelbyville Chapter.
