1 Completing an FY18 RN & APN Self-Appraisal

2 Important Notes 1. WARNING! Use of the internet Back and Forward navigation buttons will remove ALL entries since your last Save! 2. Your self-appraisal will not be available/open until: a. You enter your personal goals b. Notify your Primary Reviewer (manager/leader) your goals are ready c. Your Primary Reviewer approves them 3. Your Primary Reviewer will not be able to initiate your appraisal without your completed goals. 4. IMPORTANT! Once goals are approved, they are locked so that their image can be loaded into the automatically created Self-Appraisal, Colleague Input, and Performance Appraisal documents! While approved goals can be manually edited within the Self-Appraisal document, these edits will not be reflected in the Performance Appraisal (PA) document. Edits would require manual entry by your leader into the PA. 5. Saving your work: a. You may click Save as often as desired. At a minimum, save before you open a new Comments box, change sections/tabs, or exit. b. If you prefer to complete and save all of your work in one sitting, consider creating a rough draft using the FY18 RN Clin 1-4 & Wage Appraisal example form in the Performance Appraisal (PA) Process section of the Performance Management website. Once your draft is ready, return to eperformance and use your example form as a guide to complete your self-appraisal. 6. Notifying your Primary Reviewer that your self-appraisal is complete: a. Click Notify to send a custom to your Primary Reviewer indicating you have completed your final draft of your Self-Appraisal b. Click Complete to submit and LOCK your Self-Appraisal against any further edits 2

5 1. Access Appraisal A. From Employee Self Service, open Current Documents (steps 1 5 below) Select Main Menu (not Manager Self Service ; MSS is for your team s appraisals) B. Select Performance Appraisal from your list of Current Documents A document status of Evaluation in Progress signifies goals have been updated and approved by your primary reviewer (leader/manager). Select Performance Appraisal. As of 11/15/17, you will notice 2 Performance Appraisal (PA) documents, one each for FY17 and FY18. These can be distinguished by the Period Begin and End dates!

7 2. Rate and Document Goals A. Click the Personal Goals tab. Under Section 1 Personal Goals, Open the Desired Goal(s) 5% Appraisal Weighting Track your progress and view deadlines at the left side of your screen Option 1 - Expand all goals at once by clicking the Expand All button NOTE: option 1 lets you skip repeating step A. Option 2 - Expand one goal at a time by clicking the gray arrow before its name NOTE: option 2 requires you to repeat step A for each goal. Track your progress & deadlines Sections 1 7 are organized into Tabs Option 1 Expand all goals Option 2 Expand and work on one goal at a time NEW! Reference Materials and Resources

Rate Your Performance on Each Goal Select the appropriate rating number from the drop down list Repeat until each goal is rated Select Your Goal Rating from the Drop Down List. Click to See Rating Descriptions All ratings require supporting comments! Be concise; bullets suggested.

9 2. Rate and Document Goals C. Add Performance Notes Related to Each Goal Click Add Performance Notes to access an editing text box. Either manually type notes or import applicable notes from your Performance Notes file Click Add Performance Notes to access an editing text box for manual entry or import of performance notes. D. Manually type supporting documentation and/or Open Stored Performance Notes, if applicable. Click Save and Return. Click HERE for Editing Bar Guide Manually type within the text box. SAVE 1. Select Performance Notes from the drop down list 2. Click Go E. Click Save before moving to another Section. SAVE

10 Editing Options Bar (always select text first) This bar is available in text box areas, such as the Performance Notes boxes in each section of the Performance Appraisal. Suggestion use Spell Check prior to completing/submitting your Performance Appraisal. Change Font Name of your selected text Apply bold, italics, and/or underline Change text color or shading Opens text in miniwindow Change Font Size of your selected text Add numbering or bullets

12 3. Rate and Document Job Responsibilities A. Under Section 2 Job Function & Responsibilities, expand then rate your performance on each Job Responsibility 45% Appraisal Weighting Click to select a job responsibility rating Click to expand a responsibility In order to revalidate on Clinical Career Ladder Behaviors, ALL ratings require supporting comments! Be concise; bullets suggested. Select Proficiency/See Rating Descriptions

13 3. Rate and Document Job Responsibilities B. Click Add Performance Notes to access an editing text box. Click Add Performance Notes to access an editing text box for entry of notes supporting the selected rating. All ratings require supporting comments! Be concise; bullets suggested. C. Type supporting comments then apply desired formatting with the Editing Bar then save. Repeat until all responsibilities have been rated. 1. Manually type desired comments. Click HERE for Editing Bar Guide SAVE 2. Select Performance Notes then Go to import your notes recorded over the year. D. Save Entire Appraisal before going to another Section

15 4. Rate and Document ASPIRE A. Click the ASPIRE tab. Under Section 3 ASPIRE Values, Open/Expand all Values 50% Appraisal Weighting Click Expand located below the section title Rate each of the ASPIRE values Expand all values Click Add Performance Notes to open an editing window where you can manually add or import Performance Notes. This comments box applies to all ASPIRE values.

Rate and Add Comments on Each ASPIRE Value Option 1 Click the drop down arrow in the Rating box, making the appropriate selection Option 2 Open the Rating Model table, which provides the Rating Scale and Description of each rating, and select the appropriate rating. Repeat until all Values have been rated. Option 1 Select Your Value Rating All ratings require supporting comments! Be concise; bullets suggested. Option 2 - Select ASPIRE Proficiency/See Rating Descriptions

17 4. Rate and Document ASPIRE C. Add Comments in the Summary that apply to all ASPIRE Values These are especially important for ratings of either 1 or 3 All ratings require supporting comments! Be concise; bullets suggested. Click Add Performance Notes to add notes related to the ratings for all 6 ASPIRE values. D. Save Entire Appraisal before going to another Section

19 5. Summarize Strengths Section 4: Click the Strengths tab. Enter Comments and/or Performance Notes Only Add overarching strengths with one or more examples of how they have been demonstrated over the fiscal year. Consider these as you begin to develop your goals for next year. Click Save and Return to save your note Click Save in the upper right corner of the screen to save your entire document Click Add Performance Notes to access a text box for manual entry or import of notes describing your strengths. 19

21 6. Identify Development Areas Section 5: Click the Development tab. Enter Comments and/or Performance Notes Only Consider professional and skill areas you would like to develop next year. Note areas of challenge or failure and ways you will mitigate these next year. Click Save and Return to save your note Click Save in the upper right corner of the screen to save your entire document Click Add Performance Notes to access a text box for manual entry or import of notes describing your areas for development. 21

23 7. Enter Self-Appraisal Tab Notes Section 6: Click the Self Appraisal tab. Enter Performance Notes Only Add any special projects, certifications, accomplishments or other comments not applicable to previous sections, related to your overall performance for the fiscal year. Click Save and Return to save your note Click Save in the upper right corner of the screen to save your entire document Click Add Performance Notes to access a text box for manual entry or import of notes describing your overall accomplishments. Section 7: Summary No action is required on this screen. SAVE 23

25 8. Save Your Work A. Click the Save icon Click Save B. Verify the on screen confirmation message appears Confirm the Save C. If you Sign Out without Saving, You Will Receive a Save Warning Clicking Yes does NOT save. Instead it closes the Save Warning box to allow you to click the SAVE button located in the top right of your Performance Appraisal screen (see Step A, above).

27 9. Print Your Self-Appraisal A. Click Print B. Move your mouse down to the bottom of the page C. Click the Printer icon to open the Print window D. Close the Performance Appraisal Print window to exit print preview A. Click the Print icon D. Close the Window after printing B. Move your mouse to the bottom of the page to activate the control panel OPTIONAL: Click the Save icon to save a copy to your computer. C. Click the Printer icon to open the Print window

29 10. Submit and Lock Self-Appraisal A. Click the Notify icon to send an to the Primary Reviewer (leader) indicating Self-Appraisal is ready Click Notify B. Compose and Send Your Notification Automatically populated with your leader s address Type your message here; specify the purpose of the (i.e. Self-Appraisal is complete) Click Send to submit your Click Cancel to exit and return to your appraisal

30 10. Submit and Lock Self-Appraisal A. Click the Complete icon Click Complete to finalize and LOCK your appraisal. B. Confirm Appraisal Clicking Confirm will finalize your appraisal, lock it from any further edits, and send an automated completion to your Primary Reviewer/manager. Clicking Cancel keeps your appraisal open for additional edits and entries. Click Confirm to finalize and LOCK your appraisal. Click Cancel to return to your appraisal. C. 31 10. Submit and Lock Self-Appraisal D. Return to Current Documents to refresh status Status is still Self-Appraisal Update and Complete. Click Return to Current Documents. E. View Updated Status Status is now updated to Self-Appraisal - Completed. Save and Complete buttons no longer available. Steps and Tasks is now updated with a green check mark and VIEW.
