In doing so, programmes and services delivered by this role will: Support and strengthen community-based efforts to create safe and supportive social and physical environments Support and enhance a strategic approach across the health sector to enhance community mental health and reduce suicide (particularly in young people). Use an approach that recognises determinants of health and complex risk factors that influence issues of home safety, youth risk taking behaviours and violence prevention. Work collaboratively with agencies and organisations that foster supportive environments and individual resilience to promote positive emotional and mental wellbeing in youth particularly; and Work collaboratively with agencies and organisations specifically to reduce the rate of alcohol, illegal and other drug use. In undertaking this role, there will need to be some flexibility of working hours to accommodate community meetings, and travel throughout the Tairāwhiti District.

3 INDIVIDUAL DUTIES AND RESPONSIBILITIES Key Result Areas Facilitation and Coordination Provides appropriate and evidence-based guidance and advice to providers on health promotion issues and policies related to injury prevention, mental health, alcohol and other drugs Co-ordinates local action in relation to national and regional plans and strategies Effectively contributes to the strategic direction of the Hauora Tairāwhiti team and delivery of the annual plan Develops and maintains new and existing relationships with local civic and community leaders, and whānau, hapū and Iwi services in a range of settings such as education, sports, church and cultural and other youth support services, including development of effective and evidence-based strategies for youth mental health. Health Promotion Practice Identifies appropriate models and strategies for community engagement Gathers information which is evident of the need and impact of health services in communities and utilises this information for educational and advisory purposes Implements practice in accordance with Ngā Kaiakatanga Hauora mō Aotearoa/Health Promotion Competencies for Aotearoa New Zealand Effectively contributes activities/projects to meet the objectives of the Annual Plan, the Hauora Tairāwhiti District Annual Plan andmāori Health Plan Applies appropriate evaluation methods to all projects plans and delivery Ensures the principles of the Treaty of Waitangi, Te Pae Mahutonga, the Ottawa Charter, Jakarta Declaration and Bangkok Charter are incorporated in all aspects of project planning Financial and contractual processes for required for projects are understood and adhered to. Policy and Implementation Gathers, analyses and presents accurate data to inform decision makers in health and social services (i.e information or decision reports for the DHB governance structures, and other organisational committees as appropriate) Ensures the appropriate planning models are used in any form of advocacy or implementation of policy (i.e Health Impact Assessment, whānau Ora Tool) Consultation frameworks especially withinmāori communities are adhered to Is pro-active and co-ordinates draft submissions that reflect research to support Hauroa Tairāwhiti position

5 PERSON SPECIFICATION Skills, Knowledge, Abilities and Attributes Minimum diploma level health based qualification or 3 years working in a health promotion role Energy and enthusiasm, confident, sense of humour and a willingness to be flexible A current and full drivers licence Proven knowledge of the determinants of health, the Treaty of Waitangi and the Ottawa Charter Knowledge of the physical and mental health issues relevant to young people A willingness and an ability to collaborate with organisations focused on young Māori (Rangatahi) An ability to work with and liaise with other community agencies that have a youth focus Intermediate proficiency in Microsoft office applications Demonstrated communication skills across all mediums Demonstrated strong time management techniques Demonstrated experience in planning and implementing programme/activities Demonstrated knowledge of youth culture, and successful experience in working effectively and creatively with young people A personal commitment to Auahi Kore/smokefree and healthy living principles Conversational understanding of Te Reo Māori Knowledge of laws and government policy that affect youth Manager Date / / Employee Date / /
