8 SCORING The scoring is based on whether the element exists, is developed, communicated, implemented, in practice and is effective. THE SCORING SYSTEM Level O: The element does not exist. Level 1: The element is in the preliminary stages. Level 2: The element is less than fully developed. Level 3: The element is developed, implemented and somewhat effective. Some improvement is required. Level 4: The element is fully developed, well implemented and extremely effective. 1/8/2016 8

13 USING THE TOOL The DM Self Assessment is a web based tool that can be accessed only via a dedicated URL owned by NIDMAR licensed organizations. The DM Self Assessment consists of 39 questions and the respondent will be required to answer "Yes", "No", or "Unsure" to each question. To proceed to complete the online DM Self Assessment, access the tab marked DMSA Questions and click on the link "Commence Assessment". Prior to completing the online DM Self Assessment, it is recommended that the Terms of Use be reviewed - a pdf file can be downloaded when clicking on the tab marked "Terms of Use (pdf)". Some basic information is required in order to complete the DM Self Assessment such as a valid address. Other information is optional, and is identified on the webpage. It will take approximately 30 minutes to complete the questions. The tool will "time out" after 10 minutes of inactivity and the entered data will be lost. It is not possible to go back at a future date and resume completing an assessment. Once all questions have been answered, the respondent will push the "Submit" button and the final report will be ed shortly thereafter. Acclaim Ability Management Inc. 1/8/
