Please describe your current job duties and how they relate to NSPII. I am responsible for conducting Medical Fraud Investigations. 5. How does your career, or other professional involvement, qualify you to seek the office you are running for with NSPII? I am extremely passionate about when it comes to my job and job responsibilities. I am also passionate in regards to networking with other entities and investigators. NSPII is a great organization that understands the complex nature of insurance fraud and the hurdles that investigators deal with on a daily basis. Besides the great training that NSPII provides, the networking and contacts available within NSPII makes our jobs a bit easier. I want to make sure that the training, networking and resources stay intact and improve. 6. How long have you been a member of NSPII? 11 years 7. What committees have you served on? (List most recent and years) I am the current President of the Ohio NSPII Chapter. I was the Chairperson for the Annual Ohio NSPII Golf Outing for 4 years and Chairperson of the Training and Education Committee. 8. Do you regularly attend your State Chapter meetings? Yes 9. 2 11. If elected, will you take an active role in NSPII and with your present position, attend all annual meetings and board meetings during your term in office? I attend all meetings unless I have a work commitment that I am required to attend or be present for words or less, why do you feel you are qualified for this position? I am qualified for this position as I feel that, as insurance investigators, we are only as good as the training we receive and the contacts/networking that we have access to. No one person has all the answers. However, with the talent that NSPII members have, together, we are more efficient and effective. I will work to keep the networking a top priority so that all members will benefit from their membership. I feel that my experience on the OH Chapter Board would be very beneficial to the National Board because I can hit the ground running if chosen for the role of 2 nd VP.

3 CANDIDATE PROFILE JESSICA DAVIS 1. Name: Jessica Davis 2. Position running for: Board Member (2-year term) 3. What is your present employment position? Investigative Services Section of Erie Insurance 4. Please describe your current job duties and how they relate to NSPII. I investigate possibly fraudulent claims for the state of Indiana. I handle property, auto, and worker s compensation claims. My job duties as a professional insurance investigator align very well with NSPII s mission as an organization to help stop insurance fraud. 5. How does your career, or other professional involvement, qualify you to seek the office you are running for with NSPII? I have been an insurance fraud investigator for the past 7 years and have experience in property, auto, injury, and now workers compensation. I also have a strong background in fire investigation, including two bachelor s degrees for fire investigation and fire administration. I am currently pursuing my master s degree in public administration with a concentration in criminal justice. This knowledge combined with understanding the basic workings of an insurance company make me a strong candidate for a board member. 6. How long have you been a member of NSPII? 7 years 7. In 100 words or less, why do you feel you are qualified for this position? I feel I am qualified for this board member position simply because I am dedicated and eager to be a member of this organization on a higher level. I have been a dedicated member for around 7 years including attending as many meetings and trainings as possible. I then joined the board for the Indiana chapter and volunteered to be the interim treasurer. I have served the national board as a board member for the past 2 years. I like to see this valuable organization succeed and grow, and I am excited for the opportunity to continue to be a part of that.

5 CANDIDATE PROFILE CLAYTON CLAY JAMES 1. Name: Clayton Clay James, FCLS, CIFI, CFE 2. Position running for: Board Member (2-year term) 3. What is your present employment position? Investigator III / Analyst, The Cincinnati Insurance Company 4. Please describe your current job duties and how they relate to NSPII. As an SIU Analyst, I provide investigative assistance to SIU Operations, Claims and other business units within the company. 5. How does your career, or other professional involvement, qualify you to seek the office you are running for with NSPII? I have been working in the Special Investigations Unit for over six years, first as an investigator and now as an analyst. Prior to that, I worked as a fire investigator in the public sector. I worked for a total of 21 years in the Fire / EMS service before coming to the Insurance Industry in How long have you been a member of NSPII? I first joined in 2009 while a fire investigator in the public sector and have maintained membership ever since. 7. What committees have you served on? 6 In Dec. 2017, completed an analysis of member location by city, state, and zip code for the National Board. Previously served as a committee member for the Greater Cincinnati Regional Arson & Fire Investigation Seminar (GCRAFIS) and executive committee for the KY State Arson Task Force. I m currently a member of IASIU, ACFE, Southwest Ohio Chapter of the ACFE, and International Association of Crime Analysts (IACA). 8. Do you regularly attend your State Chapter meetings? Yes 9. Yes words or less, why do you feel you are qualified for this position? I m dedicated to the mission of working to detect, deter, and reduce insurance fraud and its impact on society. I ve served previously in other state and regional training organizations where I learned strong communication and organizational skills. I ll bring my experience to the National Board to serve our members as we support the overall efforts of our organization to provide recognition, encouragement, and support to individuals who demonstrate the highest degree of professionalism in conducting insurance investigations.

7 CANDIDATE PROFILE R. SHANE OTTO 1. Name: R. Shane Otto 2. Position running for: Board Member (2-year term) 3. What is your present employment position? Sr. Claims Analyst-SIU, Allstate Insurance 4. Please describe your current job duties and how they relate to NSPII. SIU Investigator handling investigation of potentially fraudulent claims. I work with industry professionals on a daily basis and believe NSPII is an excellent organization to bring these professionals together and provide best in class training. 5. How does your career, or other professional involvement, qualify you to seek the office you are running for with NSPII? I am actively involved against the fight against insurance fraud and believe my experience in the industry is a valuable resource for the National Board. 6. How long have you been a member of NSPII? I have been an NSPII member for 7 years. 7. What committees have you served on? (List most recent and years) I am currently the First Vice President of the Indiana Chapter as well as a current board member of the National Board. I will assume the Presidency of the Indiana Chapter for I am also the committee chair for the 2018 National Seminar. 8. Do you regularly attend your State Chapter meetings? Yes 9. In 100 words or less, why do you feel you are qualified for this position? I have been in the insurance industry for approximately 15 years, and approximately 5 years in the SIU role. I am active in both the National Board and Indiana Chapter Board of NSPII and am an adamant proponent of ongoing training and development. I feel NSPII is the best resource to provide these opportunities. I want to see both our National and State Chapters excel and believe my ongoing involvement in NSPII can help push our organization to be the number one resource for all professionals involved in the fight against insurance fraud.

9 CANDIDATE PROFILE PATRICIA J. TROMBETTA 1. Name: Patricia J. Trombetta 2. Position running for: Board Member (2-year term) 3. What is your present employment position? Shareholder at Bonezzi Switzer Polito & Hupp Co. LPA 4. Please describe your current job duties and how they relate to NSPII. As an insurance defense lawyer, I represent insurance companies and insureds in a wide variety of case types from building defects to coverage, but a large part of my practice has been assisting in investigations with insurance carriers in potential fraud cases, in both property and casualty contexts. Even when working cases like run of the mill auto accident claims there is always potential for fraud due to exaggeration of claims that require informal investigation into the fraud to properly evaluate the damages. 5. How does your career, or other professional involvement, qualify you to seek the office you are running for with NSPII? As an insurance defense attorney for more than 25 years and my previous career as a claims litigation manager, I am well-versed in the issues facing the insurance industry. My experience being the managing shareholder of my former firm s Kentucky practice and as a shareholder at a medium size firm has provided me with the context to work with a group of individuals to steer the organization. 6. How long have you been a member of NSPII? Previously for several years but recently for approximately 2 years. 7. What committees have you served on? (List most recent and years). I was board member for the Cincinnati Claims Association (I cannot recall the years), I am co-national chair of the CLM

10 committee on bad faith. I am a member of the WITL committee of DRI serving on its publications committee. I was on the committee to rewrite the local rule on arbitration in Hamilton County a number of years ago. I am a member of the Hospitality and Retail Committee of DRI. 8.
