15 MVR Point Policy (motor vehicle record) Applies to all employees operating company vehicles MVR pulled twice annually Point entries remain on drivers record 24-mos (rolling). 8-point max limit Violations in personal vehicles apply equally All accidents considered at fault unless documentation can prove otherwise Violations not on MVR apply equally e.g. red light camera etc. Training can lower points by -2 (must occur within 30-days of offense and can only be applied once per 12-mos) An 8-point violation is automatic termination

16 MVR Reviews DOT requires annual review of drivers Motor Vehicle Record Loomis goes above and beyond and reviews bi-annually 3 rd Party reviews all MVRs and scores for compliance with policy, violations are escalated to VP Safety Drivers are required to notify mgmt. within 72-hrs of any violation, DOT requires 30-day notification MVR are only as good as reporting e.g. reciprocity, driver schools etc.
