1 Performance Measure Guidance and Examples In addition to aggregate-level demographic data, grantees will report on five performance measures linked to program outcomes for the type of grant awarded. Below are sample performance measures for the various grant types. Please include three to five performance measures in the results section of the proposal. 3 This section is designed to provide guidance on the Process for Applying Performance Measurement to Policy Grants. i It is intended to be used as a guide for organizations submitting proposals to address policy strategies in partnership with the Jim Casey Initiative. The diagram above highlights six categories of activities to guide performance measures in this area. They are: 1. Problem Identification & Definition 2. Policy Analysis 3. Policy Development & Adoption 4. Education & Outreach 5. Policy Implementation 6. Impact Evaluation

4 NOTE: During the Jim Casey Initiative s Strategic Planning Process, high-action and highly aligned policy areas were identified, defined and analyzed. Respondents to this grant should focus on items 3 through 6. The table below provides an example for each area above. 1. Policy Category Sample Activity How much? How well? Difference Made?
