3 From Here to HRA in Five Easy Steps Building health savings for your employees and your company is easy. 1. Opt to offer employees a HealthEquity HRA integrated with your Florida Blue coverage. 2. Define your HRA plan by completing the HealthEquity application. 3. HealthEquity will veify your plan setup. 4. Florida Blue and HealthEquity set up employee accounts. 5. Get welcome materials sent to you and your employees by HealthEquity. Setting Up HRAs 1. Sign up with Florida Blue. Select the integrated HealthEquity HRA on your application with Florida Blue. 2. Complete HRA Application Complete the appropriate employer HRA application. There are three plan design options available. Mail or fax the completed form to: HealthEquity Attn: Reimbursement Account Client Services 15 W Scenic Pointe Dr, Ste 400 Draper, UT Fax: HealthEquity verifies your plan A HealthEquity reimbursement account client services specialist will contact you to review your application and answer questions around claims funding and other plan details. 4. Florida Blue and HealthEquity set up employee accounts Once Florida Blue receives your health plan employee enrollment information, it will send your HRA plan enrollment and claims files to HealthEquity. Upon receipt of the data from the health plan, HealthEquity will configure your plan in our system, and contact you to take you through your online employer portal. What s Next? Once HealthEquity receives your group s enrollment file from Florida Blue: Your employees will receive an HRA Welcome Kit within seven to ten business days. It s that simple! 3

4 FAQs Q. How do I fund the employees HRAs? A. You ll receive a welcome call from HealthEquity to answer any questions you have about funding and other matters related to your HRA administration. HealthEquity will also send you reminders or invoices as/if funds need replenished. If you have any questions, simply contact HealthEquity Reimbursement Account Client Services anytime from 7 a.m. to 6 p.m. MT Monday through Friday at or raclientservices@healthequity.com. Q. How long does the process of setting up the HRAs take? A. The entire process takes no more than five business days, after we have spoken with you and reviewed the group set up application. Processing your employer application will take two business days. Setting up your employees HRAs with HealthEquity will be completed within two business days of receiving the enrollment information from Florida Blue. Q. Who pays the setup and monthly administration fees? A. Account setup and monthly administration fees are billed to you directly by HealthEquity. Q. Who creates plan documents and performs nondiscrimination testing? A. HealthEquity will provide plan documents to your employer group within 120 days of account setup. HealthEquity will also perform nondiscrimination testing, if desired, and encourages that this testing be done earlier in the year, even though it s not required to be complete until the end of your plan year. Q. Where do I send my employees if they have HRA questions I can t answer. A. Simply have them call your dedicated HealthEquity Member Services line, HealthEquity specialists are available 24/7/365. Q. How do I contact HealthEquity? A. HealthEquity Reimbursement Account Client Services is available from 7 a.m. to 6 p.m. MT Monday through Friday at or raclientservices@healthequity.com. 4
