2 Clinic Down Exercise Play Exercise Briefing During the exercise it will be important to remember and comply with the following statements: You should only use Internet Explore as your internet browser for today s exercise. The focus of this exercise is your Healthcare Center s (Clinic s) INITIAL RESPONSE to an active shooter incident. The exercise time line begins the day prior to the shooting then ends 12 hours after the shooting has occurred. It is important NOT to jump ahead in the exercise scenario. In other words don t focus on what might happen, focus on the Common Operating Picture that is presented during the exercise to make informed decisions. In any exercise a number of assumptions and artificialities may be necessary to complete play in the time allotted. During this exercise, the following apply: 1. The scenario is plausible, and events occur as they are presented. 2. There is no hidden agenda, nor any trick questions. Exercise Basics: 1. This exercise contains a total of 12 scenario and inject pages. 2. The exercise is self-paced and it should take approximately two hours to complete. However, based on the time spent on each scenario and inject the exercise may be completed in under two hours or it may take much longer than two hours to complete the exercise. 3. There is a scheduled 15 minute break approximately half way through the exercise. 4. All videos embedded within this exercise can be enlarged to FULL SCREEN by simply clicking on the full screen icon in the lower right hand corner of the video. 5. Thoroughly review all scenario information and injects as they are presented during the exercise. At the end of the exercise you will need to complete an evaluation (Hotwash) on how your team preformed during the exercise. The Hotwash will consist of your team providing comments on three areas in which it performed well (Strengths) and three areas your team will need to improve upon (Areas for Improvement) to enhance its preparedness, response and recovery capabilities. 5 Clinic Down Scenario # 001 Image of Angry Employee Thursday 10:19 AM Your clinic has been under significant financial pressure over the last fiscal year. Maintaining staff morale has been challenging as your employer continues to struggle with the recent financial downturn. In response to these pressures, layoffs were announced late last week. Today, those employees who are being terminated are being notified by Human Resources. Many of these employees do not take the news well and one employee, Adam West, becomes belligerent upon hearing the news. He storms out of the Human Resources Department and begins to collect items from his desk. All the time, he is muttering under his breath about how he s being treated unfairly. 6 Clinic Down Inject # 002 Image of Notification While numerous employees are upset about the layoffs, no one expected the explosive outburst by Adam West as he left the building. Will clinic management notify outside agencies regarding the actions of Adam? Yes No If the above answer is yes. Identify who will be notified and how the notification will be made. 8 Clinic Down Scenario # 004 No Image Friday 10:42 AM After yesterday s layoffs, remaining employees have returned to work. Everyone is thankful to have survived the cuts and are concentrating on the upcoming weekend. Suddenly Adam charges through the front door. He is brandishing a handgun and shouts Where s the boss? I told you all I was going to make him pay! He begins to fire at employees, visitors and clients as he makes his way towards his former boss s office. 9 Clinic Down Inject # 005 Image of Panicked Employees Everyone in the building is shocked to hear gun shots echoing through the halls. Some people begin to take cover while others are unsure what to do. Describe in detail the immediate protective actions that need to be taken by employees, clients and visitors based upon the current circumstances. (Note: By immediate protective actions, describe what will need to happen in the first five minutes to ensure the safety of individuals.) (Note: For future reference you may want to access the Just In Time Disaster Training Library for additional information on the Active Shooter Response. 10 Clinic Down Inject # 006 Image of Wounded Individuals As Adam continues shooting, multiple employees and by-standers are shot. Adam has been seen reloading his gun several times. Within two minutes of a call being made to 9-1-1, law enforcement officers begin to arrive on scene. Describe in detail how employees will interface with arriving law enforcement during the initial response to the shooting. 11 Clinic Down Inject # 007 Image of Workplace As law enforcement arrives, employees, clients, by-standers and visitors can be seen fleeing the building. Does your clinic have a procedure to conduct an accountability check of its employees after they have evacuated the building? If the above answer is yes. Briefly describe the employee accountability procedure and identify the individual(s) by title responsible for implementing the procedure. (Note: Access Occupational Safety & Health Administration (OSHA) guidelines for building Evacuation Planning.) 12 Clinic Down Inject # 008 Image of Law Enforcement Friday 11:03 AM As law enforcement initiates its tactical response they come across multiple individuals who have been wounded or killed by Adam. Law enforcement finds him barricaded alone in one of the examination rooms and begins to negotiate his surrender. While law enforcement negotiates with the shooter, management and employees will need to focus their efforts on a number of internal issues. Identify and describe the various internal issues that management and employees will have to deal with along with identifying the employee that will be responsible for taking action regarding the issue. (Note: Remember that it is 11:03AM and the shooting began approximately 20 minutes ago.) In the wake of the shooting rampage 9 employees and 3 visitors have been killed with another 10 individuals injured. 15 Clinic Down Inject # 011 Image of Command Center The decision has been made to fully implement the clinic Emergency Operations Plan and establish an incident management system such as ICS. Identify the individuals that will fulfill the various incident management system positions and briefly describe the responsibility of each position identified. (Note: Access an example of ICS positions.) 16 Clinic Down Inject # 012 Image of Incident Action Plan Friday 2:43PM Most of the management and employees assigned positions within the clinic incident management team have assembled at a safe and secure location. One of the first actions that will need to occur will be to develop a prioritized checklist known as an Incident Action Plan (IAP), focusing on the IMMEDIATE RESPONSE issues now confronting the clinic. Develop an IAP focusing on the actions to be taken over the next 12 hours. In preparing the IAP list the objectives; identify all resources that will be needed to accomplish each listed objective; and list the expected timeline to complete each identified objective.
