2 Report Information Virgin Australia Just Culture Policy Virgin Australia is dedicated to providing and maintaining a safe and healthy workplace for team members, contractors, visitors and guests as is practically achievable. Virgin Australia has a 'no blame' policy for the reporting of accidents and incidents. This means that individuals are not punished for actions, omissions or decisions taken by them based on their experience and training which results in a reportable event. The purpose of reporting is to learn from the experience and implement appropriate processes to ensure the safety of team members, contractors, visitors and guests and serviceable equipment. Whenever an accident, incident or personal injury occurs or whenever working conditions, which are potentially hazardous to persons, equipment or buildings are detected, they must be reported and investigated. Report Submission Aircraft Maintenance Fatigue reports are submitted by Virgin Australia Aircraft Maintenance (VAAM) staff to record information on fatigue incidents. The Aircraft Maintenance Fatigue report is used by: All Aircraft Maintenance Engineers All Stores and Materials team members All other support team members. The report is used to: Record information on actual fatigue incidents To identify potential fatigue issues Identify fatigue trends Manage fatigue in the Company Comply with regulatory requirements. page 02

3 Company ereports It is important to select the correct company when completing an ereport. IMPORTANT: The Aircraft Maintenance Fatigue report is part of the Virgin Australia Airlines (VAA) suite of ereports. VAA Reports VAI Reports VANZ Reports Operational Personnel Reports submitted by all operational team members and contractors relating to: Australian domestic operations International operations including sectors to/from: Christmas Island Cocos Keeling Islands Honiara, Phuket Non-Operational Personnel Reports submitted by all team members and contractors based in Australia. Operational Personnel Reports submitted by all operational team members and contractors relating to: International operations including sectors to/from: Los Angeles Abu Dhabi Kuala Lumpur Non-Operational Personnel Reports submitted by all team members and contractors based in: United Arab Emirates United States Operational Personnel Reports submitted by team members and contractors relating to: Pacific and Trans-Tasman operations Non-Operational Personnel Reports submitted by all team members and contractors based in New Zealand and Pacific Islands. page 03

4 Aircraft Maintenance Fatigue Report To complete a new report: 1. Login to Safety Reporting: VINE > Safety Reporting > Click Virgin Australia Airlines (VAA). 2. Click Aircraft Maintenance Fatigue Report from the list of available reports. Note: The active report is indicated by an orange highlight. 3. Complete the following mandatory fields on the report: Field Date and Time of Occurrence Occurrence Title Details Enter the date and time of the fatigue event in the format: dd mm yy hh:mm Enter a brief description of the occurrence (up to 80 characters). page 04

5 4. Complete the following fields on the Incident Details tab: Field Name of staff fatigued Staff Division Fatiguing Shifts Details Enter the first and last name of the fatigued team member. Select the team member s Staff Division from the drop down list: Engineers Storemen Office. Enter one of the following in the field: Day day / night night Permanent night Permanent day Other. page 05

6 Staff Number Home Port Crew Number/Team Manager/Supervisor Notified Reasons for fatigue Suggested Remedy Previous Schedule Enter the team member s staff number. Select the home port from the drop down list. Enter the Crew Number or Team. Check the box if your Manager/Supervisor has been notified. Provide an explanation of why the team member is fatigued. Include any relevant sequence of events that led to fatigue. Enter a suggested remedy for the situation. Examples may include: changes to rosters, extended work hours, less overtime. Enter dates worked, days off and times for shift work. page 06

7 5. Complete the following fields on the Rate Fatigue Factors tab: Field Fatigue Factors Fatigue Comments Control Measures What action did you take? Details Select the rating for each Fatigue Factor. Rate the factors from 1-10, use each number once. Enter N/A if the factor did not contribute to fatigue. For any Fatigue Factor with a rating of 1 10 (not N/A), describe why this factor contributed to fatigue. Select the control measure that was put into place from the list: Not commencing shift until fit for work Self-monitor Supervisor/colleague monitor Other. Describe the action taken. page 07

8 6. Complete the following fields on the Your Details tab: Field Reporter s Name Reporter s Workplace Reporter s address Details Enter your first and last name. Note: The reporter may be different from the fatigued team member, for example their manager. Select from the drop down list: Aircraft Maintenance (VT) VA Engineering. Enter your work address. 7. Click Submit, located at the bottom of the form. 8. Click OK on the confirmation window, the new ereport number will be displayed. 9. Close the Internet Explorer window to exit the ereporting Portal. page 08
