General Summary of Function/Purpose of Position The function of this position is to enter data information quickly and accurately into a specified computer database for a wide variety of clinical and environmental specimens. The person in this position also performs a variety of clerical activities as needed such as, word processing, filing, scanning, mailing reports, answering telephones to route calls to appropriate persons/laboratories, performing data base look-ups for submitter requested patient information and other office related duties as assigned, with minimal supervision.

The total percentage of all duties performed must equal 100 percent. Duty 1 General Summary: Percentage: 80 Perform data functions to code and convert raw data into input form in three specified data bases. Data code information, from many different requisition forms into a specified data base. Sorts, organizes, scans and logs batches of forms to be data entered. Performs submitter requested patient information look-ups in a specified database, with minimal supervision and completes word processing assignments on a daily basis. Participates with the opening/closing of the unit on a daily basis. Maintains a neat and orderly work area. Performs other office related duties as assigned. Duty 2 General Summary: Percentage: 10 Answers telephones to route calls to appropriate person/laboratories, performs submitter requested patient look ups in the data base with minimal supervision. Completes word processing assignments, as assigned. Answers telephones to route calls to appropriate person/laboratory testing units. Look up submitter information by patient name, submitter name, test requested, date submitted etc., in a specified data base, or hard copy files. FAX, re-fax and/or mail duplicate reports to appropriate submitting agencies. Sorts and organizes batches of forms appropriate for a specific d-entry program, as assigned for daily work. Review, understand and sign for updates in the Unit's "procedure manual". Performs other office related duties as assigned Duty 3 General Summary: Percentage: 5 Duplicate and distribute laboratory reports when necessary. Photocopy reports. Collate and file reports Distribute reports internally & externally. Stuff envelopes and assist with mailing reports. Performs other "office related" duties as assigned. 3 File test requisition forms and prepare records for long term storage. Utilize numerical filing and document scanning procedures. Use specified protocol to archive records. Participates in archiving forms for record storage. Performs other "office related" duties as assigned. 16. Describe the types of decisions made independently in this position and tell who or what is affected by those decisions. Decisions on situations that need to be referred to supervisor, based upon established guidelines and procedures, as outlined in the Data and Specimen Handling Unit (DASH) procedure manual. 17. Describe the types of decisions that require the supervisor's review. Any actions and activities outside the realm of established guidelines, including questions regarding data entry of the appropriate assay, to assure correct testing in the laboratory, during the usual course of daily unit operation. Will need supervisor review in case of computer and/or scanner malfunction. 18. What kind of physical effort is used to perform this job? What environmental conditions in this position physically exposed to on the job? Indicate the amount of time and intensity of each activity and condition. Refer to instructions. Physical Activities: Sitting for extended periods, standing, climbing, stooping, lifting (5-10 lbs), and walking. Conditions/Hazards: In a general office setting, cold, heat - occasional offensive odors. Possible exposure to infectious substances from incoming mail and/or specimens, from the DASH Unit bench work area. 19. Entering patient/specimen information into a specified database with accuracy and speed in order to maintain the daily work load. Employee must be able to use judgment in interpreting and applying guidelines to specific situations. Employee must be willing to assist co-workers when necessary and work as a team member. 24. 4 The current position description, daily work assignments and essential duties of this position relating to data entry knowledge within the BOL require the ability to identify and resolve problems encountered by the unit within the computerized data bases, and the knowledge gained thru daily experience of the operation of the DASH Unit, as it relates to the Bureau, Health Agencies, Local Health Departments and Regional Laboratories. 25. What is the function of the work area and how does this position fit into that function? The function of the unit is to receive, process, distribute specimens for laboratory analysis and completed laboratory reports; to provide data processing, clerical support to laboratories; to provide data for program analysis, and formulation of policy; to provide laboratory records management. This position performs data coding operations, distributes completed reports and performs other office duties as assigned. 26. What are the minimum education and experience qualifications needed to perform the essential functions of this position. EDUCATIO: Education typically acquired through completion of high school. EXPERIECE: Data Coding Operator 5 o specific type or amount is required. Data Coding Operator 6 One year of administrative support experience. Data Coding Operator E7 Two years of administrative support experience, including one year equivalent to a Data Coding Operator 6, involving data coding/entry. KOWLEDGE, SKILLS, AD ABILITIES: The ability work cooperatively as part of a team. The ability to sit for long periods of time. The ability to accurately perform data entry functions as assigned. The ability to understand, follow and carry out written, oral instructions and guidelines. The ability to use correct English, spelling and punctuation. The ability to make decisions and take necessary actions within the scope of specified training. The ability to effectively communicate with the public via phone, or in person, in a professional manner. The ability to work under stressful conditions.
