1 Thank you for your interest in serving our community through the A-Team! Enclosed are two forms: A-Team Membership Form and A-Team Report Form. The forms are also available at Please take a moment to fill out the A-Team membership form and return it to the Extension Office by April. 1 if you are interested in participating in A-Team this year. (You must fill out this form & return it by April 1 to be eligible for A-Team.) A-Team Membership Form: A-Team members must be at least 12 years old by Jan. 1st of the current year. Complete this form. Check Areas of Interest these are suggestions 4-H members can volunteer in lots of different ways. If you would like to get a t-shirt, list the size & color on the form. The cost will be $5.00. (4-H Council will pay the remaining balance.) (t-shirts are optional). Return A-Team Membership form & $5.00 by April. 1. ($5.00 must be paid to order a t-shirt) A-Team Report Form: This form helps you track your volunteer hours.
