Accountability Supervisors are to ensure all personnel who intend to operate an excavator are trained and pass competent and authorised to do so 6. Training Requirements All personnel who intend to operate an excavator are to pass the theory assessment before operating an excavator (includes experienced operators) training new operators must be under supervision. Training new operators must be under supervision until the trainee is passed as competent by the trainer and sign off by SMO as authorised 7. Document Storage The electronic master version of this document, together with the associated Forms and Templates, is stored on the Safety Managers computer. Purpose and Expected Outcomes and Flowchart To provide a standard process for the creation, review, authorisation and issue of documents which require control, specifically those operational documents shown in the document structure below. To ensure a common format of documentation is followed, to eliminate duplication and to ensure accurate and current information is readily available to all employees. PROCESS FLOWCHART. 4 10. Tasks General Injury & property Damage Conduct the pre start and shut down checks as per the training procedure. It is important that all excavator operators are aware of their working environment around them e.g. visually look for Over head power cables, unstable ground, under ground services (water, power cables, gas pipes) overhanging faces, people working in the area. And the types of material which the excavator is going to sit on when working or loading dump trucks. Contacts the Shift Supervisor if this machine becomes bogged during operations, do not attempt to tow or push the machine from the bog prior to contacting the Supervisor. Loading If a Dump Truck reverses incorrectly under the Excavator the operator shall sound the loading machine horn twice to signal to the truck driver to re-position the truck into a more appropriate loading position. Where the excavator is working from the top, the capacity of the material being excavated to support the excavator must be constantly assessed. The height of the digging face must be adjusted to ensure the excavator is working in a safe environment Face heights should be such that the completed face can be cleared of any loose or unstable material likely to spill off the face. Avoid contact with truck tray at all times, if contact does occur report to your Supervisor immediately. The Excavator operator shall ensure that the bucket is at a height that will clear the edge of the Dump Truck tray. The first bucket shall be loaded to approximately ¾ capacity so as to lesson the initial impact of the material dropping into an empty tray. MVDC SWP046 June 2008 Page 4

5 Loading Injury & property Damage When loading rocky or lumpy material, the Excavator operator must ensure there is adequate fine material in the tray of the truck prior to loading larger rocks. This will assist in the loading of larger material to be as smooth as possible for the truck driver. Advise the truck driver of any exposure to blocky material before loading. The Excavator operator shall move to one side any rock that is observed to be larger than half the width of the bucket. Rocks exceeding this size shall be placed for secondary breaking prior to loading. At no time will a Excavator operator be permitted to load rocks exceeding half the bucket width. All parties shall ensure there is communication, via the two-way radio between the Excavator operator and the Dump Truck operator to inform of possible changes in the work area e.g. if a large rock is to be loaded the Excavator operator shall inform the Dump Truck driver on the two-way radio. The time/load tally sheet shall be filled out between buckets. The Dump Truck operator must ensure before they fill out the sheet that they are aware of where the Excavator is at the time before completing paperwork. The Shift Supervisor shall issue the Excavator operator with a dig plan at the start of each shift. The dig plan shall identify the shot numbers, material type and location of no dig tapes. The Shift Supervisor shall use the dig plan to identify the Excavator operator s work area and instructions for the current shift. MVDC SWP046 June 2008 Page 5

6 Loading Injury & property Damage Excavator operators are not permitted to commence operations in a work area if they have not received a dig plan for the current shift from the Shift Supervisor. If a Excavator operator is in any doubt as to the location/material type or work area instructions, the Shift Supervisor shall be consulted prior to loading of any trucks. Where it is necessary to change out operators during a shift, the Shift Supervisor shall ensure that the new operator has received a current dig plan. The Shift Supervisor shall explain the digging requirements at the actual dig face to avoid any confusion. The Shift Supervisor shall ensure the location and method of excavation is in accordance with his instructions. Excavator operators shall immediately communicate any changes in work area/material type to Dispatch via the two-way radio. Timely communication of changes is essential to ensure material is dispatched to the correct dumping destinations at all times. MVDC SWP046 June 2008 Page 6

7 Tramming Over head power cables, other vehicles machines and people Position the idlers at the front of the machine and travel motors to the rear. Carry the bucket at full height and tucked in / crowded position For long distance travel: Monitor undercarriage for excessive temperatures Bottom roller temperature should not exceed 120C degrees Water cart to be available to cool components down Swing the upper housing to the rear 15 mins every hour to alternate load on the undercarriage Walk the machine for 1 hour / cool down for 10 mins Ensure the machine is kept in the centre of the road Maintain tramming position: - Boom up Stick in Bucket crowded At the completion of each shift and when the machine has completed operation during a shift, it will be removed from the bench and parked up a minimum of 15 metres away from the working face, On flat level ground with easy access for services vehicles and maintenance personnel. 8 Pulling Batters Ensure you are aware of the batter design before commencing the task Pull batter material out at a minimum of one metre from the batter toe Use a clino meter to regularly check angle versus design Use a spotter at all times when pulling final pit walls Ensure crest of batter is delineated with flagging tape, and the tape is in design position 11.
