3 Blood Collection INTRODUCTION The use of blood and blood components can save lives and provide clinical benefit to many patients. Staff members with responsibility for the collection of blood components from the transfusion laboratory perform a key role in ensuring the right products are administered to the right patients. All staff undertaking blood product collection duties must be trained and competency assessed to the standards outlined in the trust and procedures and in accordance with National Patient Safety Agency (NPSA) requirements. COLLECTION PROCEDURE Only staff members that have received Trust recognised training and completed the required competency assessments relating to the Blood Track courier and the trust paper documentation systems, are allowed to collect or return blood or blood components from the hospital blood bank or trust satellite fridges. All wards have been issued an electronic fob allowing access to the blood bank issue room. Collection using the Blood Track courier system Collection using the Blood Track courier refers to packed red blood cells and Fresh Frozen Plasma (FFP) Ensure you take a fully completed blood collection slip to the laboratory issue fridge or satellite fridge location: The completed slip must contain the following mandatory information: - Full name - Date of birth - Hospital number - Type & quantity of products required - Details of the requesting ward / department - Name of the person collecting the product/s - Date and time of the collection - Touching the login screen on the kiosk which will ask the operator to identify themselves - The person collecting the product must then scan their ID bar code under the infrared scanning port located on the red kiosk. - Touch the TAKING OUT picture on the monitor screen; this will unlock the fridge door - Touch the next screen to confirm whether the blood is to be transported by hand or in a transportation box. - The required unit/ units can then be identified and removed from the fridge - Check the patient details of Name, Date of Birth & Hospital Number recorded on the collection slip match the information recorded on the luggage label attached to the blood pack - Place the blood bag under the scanner; ensure that the bag number is under the red line resting the bag on the shelf directly underneath the scanner - Check that the name, hospital number and Date of Birth on the screen match the details on the Page 3

4 luggage label on the blood pack & blood collection slip and complete the blood collection slip check list - If correct, press the YES button; if not, press the NO button, put the blood back and find the correct one. - The green box appears on the screen and tell you that everything is OK If you have finished, press the Log Out button - If a red box warning shows on the screen, contact the Blood Bank IMMEDIATELY. - If removing more than one unit of blood repeat the above process for each unit - The blood must be delivered to the ward as quickly as possible. The blood collection slip must be returned to the ward with the blood. (The blood collection slip must be filed in the blood slip collection box kept centrally on the ward for a minimum of 4 weeks after which time it may be destroyed.) - The Doctor/Registered Nurse/Registered Midwife/Operating Department Practitioner receiving the product must sign the 1st Compatibility Form and record the time of receipt. - A transfusion must be commenced within 30 minutes of blood leaving the Pathology fridge or for blood packs within 2 hours, if held in a cold box. Do not store in the ward pantry/fridge. - Two Compatibility Report Forms will be issued with the first pack of blood. These forms have a red border Collection of Platelets & Cryoprecipitate Ensure you take a fully completed blood collection slip to the laboratory The completed slip must contain the required mandatory information: - Full name - Date of birth - Hospital number - Type & quantity of products required - Details of the requesting ward / department - Name of the person collecting the product/s - Date and time of the collection Contact the laboratory technician on your arrival in the Blood Bank; the laboratory technician will issue the platelets or Cryoprecipitate directly from the laboratory using the blood Track Courier system. The person collecting the platelets or Cryoprecipitate must then: Check the patient details of Name, Date of Birth & Hospital Number recorded on the collection slip match the information recorded on the luggage label attached to the platelet pack and complete the blood collection slip check list & document the collection on the corresponding product issue sheet. 5 Collection of batch products e.g. Anti D, recombinant Factor V11a, Albumin & Prothrombin Complex Concentrate (PCC) Ensure you take a fully completed blood collection slip to the laboratory The completed slip must contain the following mandatory information - Full name - Date of birth - Hospital number - Type & quantity of products required - Details of the requesting ward / department - Name of the person collecting the product/s - Date and time of the collection The person collecting the blood product from the blood bank must check that all details on the collection slip match the details on the labels on the blood product. They must then sign out each unit recording the time and date on the corresponding patient product issue sheet and complete the blood collection slip check list. For batch products including Albumin, Anti-D immunoglobulin Factor V11a, & Prothrombin Complex Concentrate (PCC) product collection must be documented on the clip board register All batch products will be stored on the blood issue room bench prior to collection. If the required product is not available the person undertaking the collecting should contact the laboratory technician for advice. The product must be delivered to the ward as quickly as possible. The blood product collection slip must be returned to the ward with the product. (The collection slip must be filed in the blood slip collection box kept centrally on the ward for a minimum of 4 weeks after which time it may be destroyed.) The Doctor/Registered Nurse/Registered Midwife/Operating Department Practitioner receiving the product must sign the 1st Compatibility Form and record the time of receipt. Any discrepancies - STOP - and contact laboratory Return procedure using Blood Track Courier Only persons who have received trust training and have been competency assessed in accordance with the Blood Track Courier collection procedure are allowed to return blood. The person returning the blood to the blood bank must identify him or herself by: Touching the login screen on the kiosk which will ask the operator to identify themselves Scanning their ID bar code under the infrared scanning port located on the red kiosk. Touch the PUTTING IN picture; this will unlock the fridge door Page 5

6 Place the blood bag under the scanner; ensure that the bag number is under the red line resting the bag on the shelf directly underneath the scanner The green box and sound will tell you that everything is OK Put the blood into the fridge, close the door and then press the Log Out button If a red box warning shows on the screen, contact the Blood Bank IMMEDIATELY. Return of batch products e.g. Anti D, recombinant Factor V11a, Albumin & Prothrombin Complex Concentrate (PCC) Document return on the product issue sheet, detailing - Date of return - Time of return - Name of person returning the blood. Platelets, Cryoprecipitate and FFP must be returned to a member of laboratory staff. A Photocopy of this completion statement ONLY must be sent to Learning and Development, Home 1, for input on to the Trusts Central Training Data Base (OLM) as evidence that your staff member has completed this section of Mandatory Training. A further copy should be placed in your staff members personal development file.
