1 Strategic National Stockpile Planning in Texas Paula Hughes Purpose: Provide an overview of the Strategic National Stockpile: Definitions. SNS components. Discuss planning activities that have taken place in Texas. Discuss how SNS planning relates to pandemic influenza planning. Strategic National Stockpile A national repository of life-saving pharmaceuticals and medical materiel that will be delivered to the site of a chemical or biological terrorism event, or natural or technological disasters, in order to reduce morbidity and mortality. SNS Program Components 12-hour Push Packages (Push- Pack) Managed Inventory (MI) Rapid Procurement Vaccine Management Cities Readiness Initiative CHEMPACK Project FMS Pandemic Influenza Response Technical Assistance 12-Hour Push Pack SNS Planning in Texas Developing an SNS State Plan Develop SNS Plan under Annex H (Health and Medical Services) of State Emergency Management Plan. Coordinated regional and local plans: Ensure coordinated SNS planning in all core functions throughout Texas. 1

2 Management of SNS / Command and Control Planning leads identified at state, regional, and local levels. Roster of leads maintained with 24/7 contact information. Call-down of leads conducted regularly and information updated. Request of SNS Public Health and Emergency Management Coordination is vital. Natural Disaster or other overt event: Standard emergency management request for assistance. Request of SNS assets: Covert event: Response may already be in progress when need for SNS is discovered. Public health already engaged at local, regional, state level. Request Procedure Regional supplies of medications & equipment are overwhelmed or will be overwhelmed. Need to provide prophylactic antibiotics to jurisdiction s population is recognized. Request Procedure SNS assets are requested through the State Emergency Management plan: Local EOC to Disaster District Committee to State Operations Center to CDC Receiving, Staging, Storing (RSS) Health Service Region is lead. Warehouse facilities identified throughout the state: 12,000 + square feet. Loading docks, warehouse equipment, clean, concrete floors, climate controlled. MOU signed. RSS teams identified and trained. Distribution. Security. 2

3 Repackaging Prophylactic antibiotics in 12-hour push package are in unit-of-use bottles. Repackaging may be necessary if: Unit-of-use in the SNS inventory are insufficient. Shipments of prepackaged drugs from vendors are delayed. Prepackaged medicines in the 12-Hour PP are not effective against a particular threat and new drugs arrive in bulk. Tactical Communications Key element in continual and timely flow of assets to dispensing sites, treatment centers, and other locations. Effective tactical communications plan must have redundancy: Telephone, , fax, HAM radio, etc. WebEOC Public Information and Communication Critical in emergency situation. Provides public with vital information about the event. Where/when to go to receive prophylaxis. DSHS has developed media kits / shelf kits. Contain pre-scripted press releases. Various methods of distribution (TV, radio, print). Distributed to county judges, local, and regional health departments. Security Escort of materials into Texas. TARU Team Escort. RSS warehouse. Distribution vehicles. Dispensing (POD) sites. Controlling Inventory DSHS developed the Texas Inventory Management System (TIMS). Secured website. Web-based. Expandable. Handle one jurisdiction or multiple jurisdictions. Handle receipt of push package or MI. Excel spreadsheet as backup. Distribution Vehicles. Drivers. Fuel and Repair. Dispatch and coordination. 4 Dispensing The Most Critical Action Determine number and location of sites. Design efficient sites. Recruit and train volunteers to staff sites. Establish protocols for protecting adults, children, immuno-suppressed and geriatric populations. Dispensing (cont.) Volunteer Coordination: Recruitment. Training. Credentialing / Identification. Nursing Homes. Large private industry with medical staff. May include family members. Treatment Center Coordination Determine where at the facility the assets will be delivered. Determine who will sign for the assets including any controlled substances. Track the inventory. Training, Exercise and Evaluation Training: Regular ongoing training is critical. All planning leads and volunteers. Function-specific training. Exercise: Annual local, regional, and state exercises. Evaluation: Local, Regional and State Technical Assistance Reviews. 4

6 Cities Readiness Initiative Enhance ability to provide prophylaxis to 100% of population within 48 Hours. 30 counties in Texas: Houston MSA Dallas MSA San Antonio MSA CHEMPACK Project Forward placement of nerve agent antidotes. Qualifies for shelf life extension program. Two type of containers: EMS Container: Emergency responders; mostly auto-injectors. Hospital Container: Hospital dispensing; multiuse vials; precision dosing; long term care. Select the Launch Button to begin the course. 7 Federal Medical Stations (FMS) Hospital bed set-ups. 250 bed units. Deployed during hurricanes in Pandemic Influenza Response CDC pre-determines the Texas allocation of antivirals that will be received from the SNS. CDC begins forward placement of the SNS antivirals after World Health Organization declares beginning of Phase 4 (small clusters of human to human transmission). Pandemic Influenza Response World Health Organization declaration of Phase 4: SNS antivirals and supporting materials deployed to Texas. Received and stored at stateowned facility. DSHS activates Receiving, Staging, Storing (RSS) operations. Pandemic Influenza Response Antivirals from the SNS will be allocated according to target groups. Antivirals from the SNS will be distributed upon near exhaustion of community supplies and/or the inability of manufacturers to fill orders. Technical Assistance SNS Technical Advisory Response Unit (TARU). Includes 5-7 people including public health experts, logisticians and emergency response specialists. Transfers materiel to state and assists state. Next Steps Continue to: Refine and improve overall SNS planning. Recruit and train volunteers for SNS functions. Revise and improve plans for Receiving, Staging, and Storing (RSS) Build partnerships / relationships with emergency management and law enforcement. 7
