1 City of Sulphur Springs 125 S. Davis Street Sulphur Springs, TX Attached is the application to request dumpster services, a copy of service rules, container rates and an explanation of the deposit. A deposit equal to the delivery fee and one dump (or monthly charge) is required when the application is submitted. Payment must be received in office or online before the dumpster will be scheduled for delivery. Cash, Check, MasterCard, Visa, or Discover will be accepted. Roll-off containers can be delivered on the next business day as long as the application and deposit are received by 2:00 P.M. ABSOLUTELY NO CONSTRUCTION MATERIAL CAN BE PLACED IN MONTHLY DUMPSTERS. All service requests must go through the City of Sulphur Springs. All balances must be paid in full before containers will be scheduled for the final removal. 3 Open Top Service Rules To place an order for: A Dump and Return or a Final Removal Call us at or us at lsmith@sulphurspringstx.org Orders must be placed no later than 2 P.M. on a regular business day. Service can be provided the next business day for most requests. Placement and Pick-up times are not known or available. Service is done in order of receipt of request. Use of Container: Load container evenly. Contents cannot go above top rail. The maximum legal weight is 10 tons. Containers loaded over 10 tons cannot be hauled and must be unloaded by the customer. Dirt, roofing material, brick, and carpet are dense and heavy. Containers will need to be filled less than ¾ of the way with these materials. Container will not be serviced if it exceeds the weight limit. Container must be placed on a hard and level service. Containers will not be serviced if excessive mud or dirt surrounds the container. A minimum clearance of 22 feet overhead and 30 feet in front of the container is required. Hazardous material cannot be placed in container. Waste trucks weigh around 30,000 lbs. Please take this into consideration when you choose the location of the container. The City of Sulphur Springs or Republic Services will not be responsible for broken surfaces due to the weight of the truck and container. Service Charges Charges may be incurred if service is attempted and unable to be completed due to a blocked container, overweight container, contents hanging over top of container, muddy conditions, or prohibited items in the container. Relocate Customer may call the city to request that a container be moved to another location on site. A relocate charge may apply. Customers will be held accountable for any damages to the container caused by improper use. Signature Date

4 Waste Services Deposit Explanation All customers are required to pay a deposit for waste services. The deposit will be equal to the total of one haul (or month) and the delivery fee. Customers are required to pay all charges associated with this service before the container(s) will be removed. Customers may not use the deposit balance to pay any portion of their bill or activity prior to the final haul. The deposit will be applied to the charges from the final haul. If a refund is due, a check will be issued within 2-4 weeks of the customer s receipt of the final bill.
