2 Introduction We would like to introduce our family run business J & J Training Services who are a RTITB accredited company offering Mechanical Handling Equipment Training solutions tailored to suit your company s individual requirements. along with my father we started a Training Business in January 2009, we designed and operate the only purpose built training centre in Angus situated in Market Street in Forfar, Our facility is the only RTITB accredited training centre in Angus, Tayside and Perthshire. Our facility has a training room which is 5 sq mtrs, which can accommodate 20 seated trainees, complete with white boards, flip charts, and a modern LCD projector for safety video s to be displayed. Our Training area is 5000 Sq Ft with five three tier racking system for a more realistic warehouse feel for training. Training your staff with J & J Training Services will ensure your staff are trained to the high standard in accordance to the training guides provided by RTITB and HSE, and be able to operate in a safe working environment and understand the employee s responsibilities. On 1st of June 2011 we acquired the portfolio of FLT INDUSTRIAL TRAINING,the owner Jim Reid retired after 25 years of training, as one of many people trained by Jim Reid during his 25 years of supplying training, we will strive to continue to deliver the same high quality and reliable service supplied by FLT Industrial Training.

Any Other Relevant Information The National Operator Registration Scheme (NORS) is the centralised validation, registration and certification scheme for all RTITB accredited training. As the assembly and use of MEWPs can be physically demanding, users should be physically fit and in good health and should, generally, not have problems with eyesight or hearing, heart disease, high blood pressure, epilepsy, fear of heights/vertigo, giddiness/difficulty with balance, impaired limb function, alcohol or drug dependence or psychiatric illness. If you have any problems with literacy or language comprehension, or have any doubts about your fitness to use MEWPs, you must bring them to the attention of your employer. This need not preclude you from using MEWPs, provided your employer conducts an assessment and is able to put into place adequate measures, to take account of any difficulties you may have. 20 Ladder Safety & Inspection The Ladder Safety & Inspection course is a comprehensive full day course designed to equip operators of temporary access equipment with the necessary skills required to conduct full ladder inspections and to recognize and eliminate the dangers of incorrect ladder usage. Any Other Relevant Information As all personnel are at risk of injury through carrying out manual handling tasks as part of their duties, this course can be adapted to suit all workplaces.

23 DRIVER CPC Course location. Courses can be run on your premises anywhere in the UK if you are able to provide a suitable training room. We will check the room prior to the course to ensure that it meets basic JAUPT requirements. We can provide all audiovisual equipment. We suggest that you provide refreshments during the day. Course Scheduling & the 35 hours requirement. PCV drivers need to complete their 35 hours of training by September LGV drivers need to complete their 35 hours of training by September The 35 hours can be phased over this period to suit you e.g. 7 hours per year for five years, or 14 hours in each of two years and 7 hours in one year, or any combination so long at the 35 hours is completed before the deadline date for PCV or LGV drivers. Course Timings, Weekend Training & Course Size. Most course run between 9:00 a.m. and 5:00 p.m. but some clients prefer to start earlier. A full 7-hour session must be completed in one day. Or the seven hours can be completed over two days providing that the second 3½ hour session starts within 24 hours of the end of the first 3½ hour session. We can run courses during the normal working week or on a Saturday. We are registered with JAUPT to deliver courses for up to 15 delegates on one course. Attendance & DSA Registration. Drivers must attend the full seven hours of training to qualify for having their seven hours attendance uploaded to the DSA register. Drivers failing to attend the full seven hours will not have their details registered with the DSA so even their partial attendance will not count. Late arrivals or early leavers will not be registered as having attended. Drivers completing seven hours training will have their details uploaded to the DSA register. Once a driver has completed 35 hours of training, the DSA will immediately issue a Driver Qualification Card (DQC), which is sent to the driver's home address. We are registered with JAUPT to deliver courses for up to 15 delegates on one course.

24 Driver license. Please note that on the day of the course we will need proof of ID for every delegate. This can be a photo card licence plus the paper counterpart. Or it can be an old style paper licence plus a current passport. No other forms of ID are acceptable. If we don't either of these on the day we will not be able to upload the 7 hours accreditation onto the DSA database.

26 General Terms and Conditions And Guidance Notes FOR ON-SITE COURSES- PLEASE ENSURE AVAILABILTY OF:- 1 All delegates (uninterrupted attendance is important for certification) 2 Suitable delegates (All delegates must be medically fit to operate the equipment) 3 Special Needs (If any delegates have special needs, we must be advised at least 7 days prior to course commencement) 4 Appropriate Equipment:- in a safe and sound mechanical condition 5 Practical Training Area:- with racking, if applicable 6 Theory Workroom:- with table, chairs and electrical socket 7 Suitable loads: - For FLT operator training, a supply of empty pallets and stackable loads. For Crane operator training, a selection of loads and slings 8 Consent:- that the instructor running the course will need to use the equipment as part of the training process 9 Access Platform and any Man-Up machine: - a current LOLER certificate must be available for inspection at or before the commencement of the course. 10 Personal Protective Equipment (PPE):- This should be provided to all delegates in accordance with HSE Regulations and your own company rules and regulations 11 On Man-Up Machines: - Escape procedures cannot be taught as part of basic training. 12 All Courses are conducted in English: - It is vital, for Health and Safety reasons, that all delegates have a sufficient working knowledge of English to allow them to participate fully. Courses Held at J & J Training Services Training Centre:- Same as 2, 3, 10, 11 & 12, Please note the centre will be open ten minutes prior to course commencement. General Terms and Conditions:- 1 Settlement:- unless previously agreed with J & J Training Services Director (in writing) Payment is due on receipt of invoice. Please ensure that the enclosed invoice is forwarded to your Accounts Department immediately for payment. Late Payment: - J & J Training Services reserves the right to charge reasonable administration cost/interest were settlement is unduly delayed. 2 CERTIFICATES: - Will be forwarded on completion of training once the invoice has been settled in full. 3 CANCELLATIONS/POSTPONEMENTS (includes lack of delegates or facilities upon arrival- see above) Course fees are payable in full should cancellation or postponement be due to client/customer failure to ensure availability as detailed above.
