8 Chapter 5: Safety management General Safety 5.7 Is the vessel provided with a safe means of access? Other Inspector Comments: Terminal ladder was utlized to board the vessel. Fire Fighting Equipment 5.12 Is sufficient fire fighting equipment on board, which may include hoses, nozzles, firemen's outfits, breathing apparatus and portable extinguishers and is it in satisfactory condition, and ready for immediate use? Other Inspector Comments: There were two (2) Fire extinguishers on board. The fire extinguishers were last serviced on 15MAR2015. Operational Safety 5.28 Is continuous communication between the barge and tug, or the barge and dock, being maintained? Other Inspector Comments: Vessel was communicating with the tow boat and dock by intrinsically safe hand held radios Are dangerous cargo signals (red flag or red light) displayed? Other Inspector Comments: Red pendant was displayed on the deck. Vessel Security 5.61 Are measures in place to prevent unauthorized boarding? Other Inspector Comments: Visitor's identification verified. Deck and gangway watch maintained. Chapter 6: Pollution prevention Pollution prevention 6.3 Is the vessel provided with a USCG approved Vessel Response Plan (VRP)? Other Inspector Comments: USCG approved Vessel Response Plan no expires on 19JUL Is suitable equipment provided to deal with small oil spills? Other Inspector Comments: One oil spill kit to mitigate small spills was stowed on deck. 6.7 If fitted, are hydraulic lines on deck free of visible leaks? 9 Chapter 7: Structure Structure 7.1 Is the hull free from visible structural defects that warrant further investigation? Inspector Observations: Heavy indentation was sighted on the hull plating: 1). Port side forward IWO of no. I cargo tank heavy indention 10 feet in length x ( visible 1.0 feet breath) x 5 inch deep contact damage was noted on the side plating. Rub rails was bent and set in. 2).. Port side middle IW O cargo tank no. 2 contact damage with deep indentation of 12 feet x 6 inch deep ( within visible range) was visible on the side plating and rub rails. 3). Starboard side after deck and side plating dented and bent 4 feet x 8 inches. Initial Operator Comments: The indentions were previously reported internally as captured by a Barge Superintendents semi-annual inspection (July 2015). The indentions are considered normal wear and tear, and were deferred until budgeted maintenance and COI expiration due 4/15/2016. Repairs will be made at that time. There are no incidents reported that tie back to this damage. A BP structural review was completed on 03MAR If any cargo, ballast tanks or voids were inspected or sighted from the deck, were they in a satisfactory condition? Other Inspector Comments: Four (4) void spaces was inspected from the top of the access. The void space was dry and the hard coating was in fair condition. Chapter 8: Cargo handling General Cargo Handling 8.5 Are legible and up to date pipeline and/or mimic diagrams of the cargo system, inert gas system and venting system available? Other Inspector Comments: Cargo system and venting system diagrams were in the folder. ot fitted with a inert gas system. 8.8 Are the cargoes being carried listed on the Certificate of Fitness, COI and/or Cargo Authority Attachment? Other Inspector Comments: Cargo Authority was issued on 28JU2000 and was attached to the COI Are the responsible persons familiar with the carriage requirements for the cargoes on board in general? Other Inspector Comments: Tankerman PIC expires 27JUL If fitted, is the general condition of the cargo tank heating system satisfactory? Other Inspector Comments: Cargo tank steam fixtures and pipes were sighted in good condition As applicable, are cargo pumps, booster pumps, ballast pumps and stripping pumps, eductors and their associated instrumentation and controls in satisfactory operational condition, free of leaks? Other Inspector Comments: Cargo pump was operational. o ballast pump fitted. Pressure relief valves test certificates were on board. 9/12

10 8.22 If fitted, is the Emergency Shut-Down (ESD) System fully operational? Other Inspector Comments: ESD tested and found operational If the vessel is equipped with derricks or hose handling booms, are they in satisfactory condition, marked with Safe Working Load? Other Inspector Comments: ot provided with hose booms If multiple cargoes are being handled, are signs placed at each cargo manifold, identifying the grade of cargo? Other Inspector Comments: Only one grade of product was being handled Are cargo pipelines tested annually? Other Inspector Comments: Cargo pipeline was last tested on 18AUG If the vessel uses its own cargo hoses, are they in good order, pressure tested annually to their design working pressure, and is a record of all hose tests and inspections maintained on board? Other Inspector Comments: Cargo transfer hose feet was last tested on 03MAR Are relief valves fitted to the cargo pipeline system? Other Inspector Comments: Relief valve on the cargo pipeline was last tested on 18AUG2015. The seals were intact If cargo segregations using blank flanges are fitted, are the flanges fully bolted? Other Inspector Comments: Cargo segregation blanks were not fitted Are the valves serving the cargo and ballast system in satisfactory operational condition? Other Inspector Comments: Valves on the cargo pipelines were maintained. Ballast system was not fitted on the vessel. 10/12

11 Cargo Measurement 8.70 Are tank calibration tables available and approved by a cargo inspection organization Other Inspector Comments: Tank calibration tables were approved by Inspectorate America Corporation If fixed cargo measuring equipment is fitted, is it operational? Other Inspector Comments: ot fitted with fixed cargo measuring equipment Are cargo tanks provided with an overfill protection system (High Level Alarms) and is the system fully operational? Other Inspector Comments: Vessel was fitted with overfill protection system. High level and overfill alarms were tested and found operational If fitted, is the cargo high level alarm system used during the cargo transfer observed? Other Inspector Comments: Vessel was discharging cargo If a flow meter is fitted, is it operational and calibrated in accordance with the requirements of the approving authority? Other Inspector Comments: o flow meter was fitted on the cargo discharge lines. Venting and Inert Gas Systems 8.86 If the vessel is fitted with a cargo venting system, is it in a satisfactory operational condition? Other Inspector Comments: A common venting line with a P/V valve was fitted for cargo tank venting Are the P/V valves in good order, fitted with flame screens, inspected and cleaned as part of a regular maintenance routine? Other Inspector Comments: P/V valve was last serviced on 18AUG If cargo tank stop valves are fitted which permit the isolation of individual tanks from the common venting system, are the individual cargo tanks provided with two means of full flow relief of vapor, air or inert gas mixtures to prevent over-pressure or under-pressure? Other Inspector Comments: Stop valves not fitted If the vessel is equipped with a vapor-return system, is it operational? Other Inspector Comments: Vapor tightness was last tested on 18AUG2015. 12 Chapter 9: Mooring Mooring 9.1 Is the vessel effectively moored? Other Inspector Comments: Vessel was moored to the dock with 5 polypropylene lines. 9.4 If fitted, are the winches that are employed for mooring in a satisfactory condition? Other Inspector Comments: Two (2) mooring winches with wires were installed on the forward deck and two (2) on the after deck. Chapter 10: Towing and pushing vessels Towing and pushing vessels Are the connections between tugs to barges and between barges being maintained in a satisfactory condition? Other Inspector Comments: Vessel was not connected to the barge at the time of inspection. Chapter 11: Machinery Machinery 11.9 If fitted, are the boiler fuel emergency stops operational? Other Inspector Comments: Boiler was not installed on the barge If fitted, are the 12 volt and 24 volt batteries in satisfactory condition? Other Inspector Comments: o batteries fitted. Chapter 12: General appearance General appearance 12.2 If fendering is fitted is it in a satisfactory condition? Other Inspector Comments: o rubber fenders fitted. Rub rails were welded around the perimeter of the vessel Is the general condition of service pipework satisfactory, is it free from significant corrosion, pitting, soft patches or other temporary repairs?
