NEW UNIVERSITY HOSPITAL IN AARHUS AN ENTERPRISE BUILT IN LOGISTICS? Lars Ganzhorn Knudsen, Aarhus University Hospital, Central Denmark Region med+logistica, Leipzig, 2017, May 18th Quellen: Bild Miles Studio fotolia.com Fraunhofer Seite 1

An enterprise built in logistics the patient logistic perspective Agenda A short introduction to the danish hospital structure Aarhus University Hospital Rethinking logistics in the design phase Patient logistics out patients Patient logistics acute patients Conclusions Fraunhofer Seite 2

The regions in Denmark North Denmark Region Central Denmark Region 1,210,000 inhabitants Capital Region of Denmark Region of Southern Denmark Region Zealand Fraunhofer Seite 3

Emergency structure in Central Denmark Region Fraunhofer Seite 4

National trends in Denmark Reduction in number of hospitals and beds Centralization and specialization Fewer hospitals with ED s Focus on prehospital emergency care Focus on intermediate care GP s collaborating in larger clinics Number of discharges over the last 8-10 years slight increase Outpatient visits huge increase Average length of stay is now 3.5 days lowest in EU 2015 Hospitals to be renovated + new hospitals built (6 billion to be spent) Fraunhofer Seite 5

Aarhus University Hospital (Somatic & Psychiatric) Aarhus University Hospital in Skejby today: approx sqm New buildings: app sqm Plant investment (gross) 1.4 bn Euro Operating budget (gross) app 1.0 bn Euro App employees App students Annual somatic activity: 100,000 admissions (inpatients) 850,000 outpatient visits acute patient visits 40-50% of the regional hospital activity Up to 40,000 daily transportations Fraunhofer Seite 6

Rethinking logistics in the design phase Big is beautiful but it challenges logistics in all areas of hospital operation supplys, medicine sterile goods patient flow, etc In the case of the new hospital construction projekts in Denmark the central government has made the following requirements: 8% efficiency as a consequence of the new hospitals (operating budget) 2% annual efficiency (operating budget) 30 days clearing guarantee 30 days treatment guarantee (specific diseases) Fraunhofer Seite 9

Rethinking logistics in the design phase exampel orthopedic out patient Out patients are waiting in larger waiting areas. Patients register upon arrival using the designated terminals Shortly before examination the patient is called to the clinic. In the case af a patient with multiple examinations, the patient has to capable of navigating to different waiting areas. Fraunhofer Seite 10

Way finding basic principles Areas Crosspoints Entrances Fraunhofer Seite 11

The intension of the way finding consept is: Patient can find the distination at the expected time Self-reliant patients remains self-reliant (80%) The patient pathways are supported in a holistic perspective Increased patient satisfaction Better use of hospital resources Fraunhofer Seite 12

A patients path... More options for help along the way Patient App Parking screen with touch display Thursday, december 18th 2017 kl Urology outpaitent clinic Entrance C, Floor 2, Crosspoint C218 Palle Juul-Jensens Boulevard 99, 8200 Aarhus Digitale information signs Way Finding screen Entrance C Waiting ares Self-registration Urology outpatient clinic Fraunhofer Seite 16

Patient logistics out patients When the patient has navigated to the correct waiting area, the patient scans his/her social sequrity card at the terminal. The patient recieves a bon with a number. On the digital call sign the patient can follow the progress Fraunhofer Seite 17

Patient logistics out patients In order to optimize the out patient flow, real time information about the current status of different appointments must be shown to the patient. This implies that the staff and/or systems must perform real time updates. Fraunhofer Seite 18

Out patient logistics As out patients with multiple appointments often spends many hours at the hospital, the clinic needs to communicate directly to the individual patient. Patients with smartphones can use an application My Appointments using the bon code recieved at registration or directly using the national identification service. Fraunhofer Seite 19

Acute patient logistics The Emergency Center at Aarhus University Hospital is a new innovation Patient can stay in EC up to 48 hrs. The different specialities are present in EC. 76 single patient rooms Patients are pre-triaged and are directed/transported to the designated room. Fraunhofer Seite 20

Acute patient logistics Pre Hospital Journal Clinical logistics Electronic patient journal Visitation module Acute module Continuous dataflow Referal Visitation Arrival Reception Examination/ Treatment Discarge/ Transfer Fraunhofer Seite 21

Conclutions Patient logistics must be an integrated part of hospital planning and engineering! Patient flow and processes must prevail over building design and IT-systems! Efficiency is important - but don t forget the human factor! Denmark hospital grants dying man final wish of cigarette and wine while watching sunset Fraunhofer Seite 22
