2 California Ambulance Strike Team Dispatch Training Course Outline Course Objectives To provide participants with the knowledge to: 1. Effectively respond to Ambulance Strike Team (AST) requests using SEMS and NIMS standards. 2. Understand the California Medical/Health Mutual-Aid System. 3. Receive and process AST requests and deployments. 4. Understand the communications equipment and protocols of the units within an AST. 5. Maintain current lists of AST resources, and 6. Apply their knowledge through demonstration. Course Content (Each unit includes a lesson plan, PowerPoint slide presentation and presentation overview for the instructor.) 3 SEMS / NIMS SEMS Standardized Emergency Management System 2 N.I.M.S. The National Incident Management System (NIMS) NIMS represents a core set of doctrine, concepts, principles, terminology, and organizational processes to enable effective, efficient, and collaborative incident management at all levels. It is not an operational incident management or resource allocation plan. 3 1

11 Summary Both SEMS and NIMS provide a basis for a standardized and coordinated response to emergency events. All information should be coordinated through key centralized points at each level of the emergency management system. 25 9

14 RDMHC Responsibilities: supporting the mutual aid requests of the MHOAC for disaster response within the region and providing mutual aid support to other areas of the state in support of the state medical response system, and serves as an information source to the state medical and health response system. 8/28/ RDMHS The Regional Disaster Medical/Health Specialist is a staff person in a LEMSA where that agency has agreed to manage the regional medical and health mutual aid and emergency response system for the OES Mutual Aid Region. 8/28/ RDMHS Responsibilities: to manage and improve the region medical and health mutual aid and mutual cooperation systems; coordinate medical and health resources; support development of the Operational Area Medical and Health Disaster Response System; and, support the State medical and health response system through the development of information and emergency management systems 8/28/ Unit 2 Intro to MutiCasulty ICS 3

15 MHOAC The Medical/Health Operational Area Coordinator is an individual designated by a County Health Officer and EMS Agency Administrator who is responsible, in the event of a disaster or major incident where mutual aid is requested, for obtaining and coordinating services and allocation of medical and health resources within the Operational Area (county). 8/28/ /28/ JEOC The Joint Emergency Operations Centers (JEOC) is a facility staffed jointly by the State Department of Health Services and State EMS Authority during a state of emergency to support and coordinate medical and health resource requests in California. 8/28/ Unit 2 Intro to MutiCasulty ICS 4

17 California Disaster Medical Network (CDMN) A Project Sponsored by the California EMS Authority August CDMN The CDMN is an internet-based application, powered by E Team, for the management of emergency medical response activities. E Team is the underlying application of the California Governor s Office of Emergency Services Response Information Management System (RIMS) 2 CDMN Applications CDMN can currently be used for: Incident/Event Notification Resource Status hospital beds medical transport units Resource Ordering Document Sharing Personnel Directory 3 1

19 CDMN Login CDMN Usernames & Passwords have been provided to staff at CDHS, EMSA, RDMHSs, and LEMSAs. Each LEMSA has been assigned 3 usernames/passwords + 3 usernames/passwords per Medical Operational Area 7 Passwords are Case Sensitive 8 CDMN When you complete the personal profile, this provides contact information to other CDMN users. You will be asked to update this information each time you login to the system. If the information is all correct, simply click the Submit button. 9 3

21 CDMN - Pre-defined Duty List Duty List can be pre-defined by position (e.g. RDMHS, LEMSA, MHOAC, EMSA, Regional Dispatch, etc) This is designed to assist personnel with standard responsibilities assigned to the position. 13 Duty Lists can be predefined for various positions 14 CDMN Quick Incidents These can be created to quickly notify the appropriate groups or individuals of an event 15 5

23 Enter Initial Situation Summary 19 Select the Notification tab in order to notify the appropriate personnel of the incident. 20 Be sure to click Yes if you want others to be notified of the incident. You can choose Individuals, Groups, or enter other addresses for notifying others of the incident. 21 7

24 Once your notification message and recipients is complete, click Submit. 22 CDMN Hospital Bed Availability shows information on inpatient and ED capacity This information is populated automatically from the local hospital interoperability systems (e.g. ReddiNet, EMSystem, DataTech 911) 23 EMS Hospital by Op Area: will display the latest information entered by the local hospital interoperability systems throughout the state. 24 8

29 Status: This is the current status of the resource. This field is now color-coded look closely. Requestors: Choose Red-Action Required (this lets the RDMHC/S & State EMSA know to go in and approve) Only the Responding RDMHC/S or EMSA will make updates to this field. As the mission progresses, the Status will be updated accordiningly. 37 CDMN Creates a Request Number automatically. Quantity: Enter number of resources needed (and Unit Measure, e.g. Each ) When Needed: Reasonable Date when the resource is needed. 32 REQUESTORS STOP HERE! You are done making entries in this form. Please click the Submit button. (Remember, make a quick heads-up phone call to the next in the chain of command; LEMSA/MHOAC call RDMHC/S, RDMHC/S call EMSA Duty Officer) RESPONDERS PLS. CONTINUE DOWN THE FORM 46 The Request can be forwarded to an Individual or a Position. This can then be forwarded again to the next SEMS level (e.g. Affected Op Area to RDMHS to EMSA to unaffected RDMHS). If more than one Vendor or Agency is necessary to fill the request, the Request must be Split

39 Receiving Resource Requests 1 Ambulance Mutual Aid Request Categories IMMEDIATE: Units receiving, and able to comply with, an Immediate AST request will depart within 60 minutes of receiving the request. Units will normally respond initially to an identified rendezvous point with other units of an AST prior to moving to the assigned reporting location. PLANNED: Units receiving, and able to comply with, a Planned AST request will depart up to 24 hours or more after receiving the request. Units will normally respond initially to a given rendezvous point with other units of an AST prior to moving to the assigned reporting location. 2 Ambulance Resource Typing* Type I, Advanced Life Support (ALS) Haz Mat, Type II, ALS Non-Haz Mat, Type III Basic Life Support (BLS) Haz Mat, Type IV BLS Non-Haz Mat, and Other strike teams consisting of five (5) units, one ambulance strike team leader, and one assistant strike team leader. *(U.S. Department of Homeland Security, FEMA 508-3, May 2005) 3 1

40 Ambulance Resource Typing 4 Ambulance Resource Typing 5 Order/Request Number Units must have an Order/Request Number in order to be considered an official mutual aid response. Order/Request Numbers should be written on the ambulances themselves and maintained by the crews. 6 2

41 Order/Request Number An Order Number consists of an agency designator preceded by an M and followed by a four-digit number, e.g. MXSJ The agency designator, in this case XSJ identifies the Operational Area making the request. Op Area identifiers can be found in the FIRESCOPE Field Operations Guide (FOG) appendix. The M indicates that the request is through the California Medical/Health Mutual Aid System. 7 Order/Request Number The four digits are a sequential set used by the requesting agency to designate a specific incident. Agencies usually begin counting their incidents on January 1st with Ambulance Strike Teams Designators The Ambulance Strike Team Designator will consist of the Operational Area Designator from which the strike team leader is drawn and a four digit number beginning with the OES Region number, e.g. XSJ4000. (The number four indicates that the ambulance strike team is from OES Mutual Aid Region IV). Each ambulance strike team organized for a specific incident will be assigned a sequential number, e.g. XSJ4000, SAC4001, etc. 9 3

52 Local, Regional, and State Notifications 1 RDMHC/S The Regional Disaster Medical Health Coordinator (RDMHC) is a volunteer nominated by the medical/health professionals within an OES Region and appointed by the Directors of the California EMS Authority (EMSA) and the California Department of Health Services (DHS). 2 RDMHS The Regional Disaster Medical Health Specialist is a staff person in a LEMSA where that agency has agreed to manage the regional medical and health mutual aid and emergency response system for the OES Mutual Aid Region. 3 1

53 RDMHC/S Responsibilities of the RDMHC/S include supporting the mutual aid requests of the MHOAC for disaster response within the region and providing mutual aid support to other areas of the state in support of the state medical response system. The RDMHC/S also serves as an information source to the state medical and health response system. 4 NOTIFY RDMHC/RDMHS Contact the RDMHC/S to notify them of an AST request. If no response within 5 minutes, recontact the RDMHC/S. If no response after 10 minutes, contact the alternates in order listed on the RDMHC/S Directory. 5 RMTC The Regional Medical Transportation Coordinator (RMTC) is a volunteer position, appointed to assist the RDMHC/S with the development and management of the AST/MTF project within the OES Mutual Aid Region. The RMTC may be activated by the RDMHC/S to assist with resource coordination, tracking, and support. 6 2

54 NOTIFY REGIONAL FIRE DISPATCH AGENCY Contact the 24-hour designated dispatch center for the OES Regional Fire Coordinator and notify them of the AST/MTF request. Confirm that the AST/MTF order request is not being duplicated. 7 Prior to Activating the AST Leader When making contact with the AST Lead agency or AST Leader, ensure you obtain the following information: Determine the rendezvous point for the AST units Assignment of a travel frequency Call back number (cell or satellite) 8 Determining the Rendezvous Point A listing of pre-determined Rendezvous Points should be maintained in the Regional Dispatch Center System Manual. The listing of Rendezvous Points should be periodically reviewed and updated by the RDMHC/S and RMTC. During activation of the AST Leader, a Rendezvous Point is selected. 9 3

55 Assigning a Travel Frequency In order for the AST units to maintain communications during travel, a radio frequency must be assigned by the Regional AST Dispatch Center. The State EMS Authority holds licenses for statewide communications on UHF Med-net Channels 9 & Assigning a Travel Frequency The Regional Ambulance Dispatch Center shall assign a Travel Frequency for each AST. The Travel Frequency will be either UHF Med-9 (narrowband) or UHF Med-10 (narrowband). If multiple ASTs are being deployed simultaneously, they shall be assigned Med-9 or Med-10 alternately. 11 Assigning a Travel Frequency Since various local EMS systems use Med-9 and Med-10, all radio communications during travel must be limited to non-repeated low-watt portable radios. The RDMHC/S (or designee) shall notify the EMS Duty Officer of the Travel Frequency Assignment. 12 4

56 Disaster Assistance Support Units DASUs 13 DASU 25 state-owned Disaster Ambulance Support Units (DASUs) have been strategically located throughout the state. These vehicles are designed to provide 3-4 days of medical supplies and rations for the AST. 14 DASU The DASU is also outfitted with the AST Communications Cache. In order to complete the Task Book, they are deployed as an Assistant AST Leader 18 6

58 ACTIVATE the AST Units 19 ACTIVATE AST The AST Leader Agency or the MHOACs will provide deployment information to the AST units, including: 1) contact information for the AST Leader 2) the travel frequency 3) AST Designator 4) Date/time to meet at Rendezvous Point 5) Order/Request Number. 20 NOTIFY EMSA DUTY OFFICER The RDMHC/S (or designee will notify the EMSA Duty Officer of AST deployment, including: 1) AST Designator, 2) AST Leader Assignment, 3) Order/Request Number, 4) Travel Frequency. 21 7

59 MHOAC The Medical Health Operational Area Coordinator is an individual appointed by a County Health Officer and EMS Agency Administrator who is responsible, in the event of a disaster or major incident where mutual aid is requested, for obtaining and coordinating services and allocation of medical and health resources within the Operational Area (county). 22 NOTIFY MHOACs In response to any AST request, the RDMHC/S (or designee) will contact the MHOACs to request AST resources. The RDMHC/S will also notify MHOACs from the counties of the units assigned to the AST to provide AST deployment information. 23 Summary Timely notification to all parties responsible for elements of the medical/health mutual aid system is essential, including: RDMHC/S MHOACs RMTC Regional Fire Dispatch AST Leaders AST Units EMSA Duty Officer 24 8

60 Maintenance of AST/MTF Resource Lists 1 Regional AST Leader List The California EMS Authority shall periodically provide the Regional Medical Dispatch Center with an updated listing of all personnel within the OES Mutual Aid Region who have successfully completed an AST Leader Course In cooperation with the RDMHC/S and RMTC, the Regional Dispatch Center shall periodically, but no less than annually, verify that each AST Leader candidate is employed and in good standing with a Medical Transportation Provider within the OES Mutual Aid Region. 2 Regional AST Leader List The RDMHC/S and RMTC shall periodically, but no less than annually, assign a status to the AST Leader candidates. Statuses shall include: Certified (completed task book on file at EMSA) Trainee (task book in progress less than 3 years) Ineligible (not currently sponsored by Medical Transport provider within the OES Mutual Aid region or failed to complete Task Book within 3 years) 3 1

61 Regional AST Leader List The RDMHC/S and RMTC shall periodically, but no less than annually, establish an AST Leader Deployment Schedule. All AST Leader deployments shall be noted on the AST Leader Deployment Schedule in order to maintain equitable rotation of personnel. 4 AST/MTF Provider Agencies The California EMS Authority shall periodically, but not less than annually, provide the RDMHC/S with an updated California Standard Ambulance and EMS Personnel Fee Schedule. In cooperation with the RDMHC/S and RMTC, the Regional Medical Dispatch Center shall periodically, but no less than annually, verify that all pre-designated ASTs within the OES Mutual Aid Region have current Agreements on file with the Local EMS Agency or state EMSA. 5 AST/MTF Provider Agencies The RDMHC/S and RMTC shall periodically, but no less than annually, evaluate and update as necessary, the listing of predesignated ASTs within the OES Mutual Aid Region. 6 2
