4 1.0 Purpose This document describes the procedure to lift the DESI spectrograph from the IHF to the truck bed that transports the Spectrograph to the Mayall, and the procedure to lift the Spectrograph off the truck once it reaches the Mayall Telescope. 2.0 Applicability and Starting Conditions Applies to DESI spectrograph forklifting for transport of the spectrograph from the IHF to the Mayall Telescope 2.1 State of the DESI Spectrograph prior to start: DESI Spectrograph shall be packaged and protected inside its inner crate and positioned inside the Outer Crate at the IHF, with front and top covers of Outer Crate removed and base modifications completed (refer to beginning of Section 8.0 for item weights) 2.2 State of the Linde E18C forklift Forklift shall be stored and fully prepped for the move at the IHF 2.3 State of the Linde H40T forklift Forklift shall be stored and fully prepped for the move at the Mayall 3.0 Scope and Analyses This document covers all the steps required to lift the DESI Spectrograph onto the flat-bed truck at the IHF and off the flat-bed truck at the Mayall Telescope 4.0 Definitions 4.1 Outer Crate: Outer shipping crate for the DESI Spectrograph, houses the Inner Crate 4.2 Inner Crate: Inner shipping crate for the DESI Spectrograph; this is re-used so please take care when handling it. 4.3 Winlight: Manufacturer of the DESI Spectrograph 5.0 Training 5.1 Forklift operator training and current certification for operator of Linde E18C and H40T forklifts 6.0 Safety Job Hazard Analysis (JHA) and remediation actions are included in the procedure in red. See Legend below for Hazards Present Acronyms. Size Weight 2053mm x 1536mm x 821mm 101mm x 1524mm N [2276 lbf] N [92.0 lbf] Task Leader to initial each step as it is completed. 8.1 Verify the DESI Outer Crate has had its front and top removed, along with any insulating foam that may be in the way. Note: Please take care with Outer Crate materials as this crate must be sent back to Winlight to be re-used for future Spectrograph shipments, see Figure AD Docx Page 6 of 18

8 8.3 Deploy personnel as follows: Forklift driver to Linde E18C One signal person, with two lookouts Safety watch to monitor space and personnel 8.4 Install forklift extensions and drive forklift up to crate and position forks under inner crate Note: The Inner Crate is slightly recessed inside the Outer Crate, which will not allow the load to be fully positioned against the backstop of the forklift. This requires an initial re-positioning using the forklift where the Inner Crate is lifted just enough to move across the base of the Outer Crate until there is enough clearance for the forks to fully engage under the load, see Figure 3 Figure 3: Inner Crate re-positioning for lift; note that this is a composite of two images, depicting the ends of the same side of the inner crate. 8.5 Insert forks under the Inner Crate as far as they can reach 8.6 Bring forks up on slowest setting until Inner Crate load is just released from the base of the Outer Crate 8.7 Backup forklift 3-4 or until the front edge of the Inner Crate first meets the foam insulation on the base JHA: CB Keep hands and fingers clear of sides while moving crate. Make sure to be wearing appropriate gloves while working with the wooden crate. 8.8 Lower Inner Crate back down until it is fully supported on the Outer Crate base, drive forklift the rest of the way in until the Inner Crate wall meets the forklift backstop. Note: the forklift extensions are 60 and the Inner Crate is 70 deep, so the forks will not contact the backside of the Outer Crate. 8.9 Gently lift the Inner Crate and pull it clear of the Outer Crate, see Figure AD Docx Page 8 of 18

Make sure area is clear of people and objects that could be in the way. Use caution while transitioning the load from the building to the flatbed Raise garage door and make single point turn to drive forklift onto loading bay, see Figure Note: Watch for transition between building and loading bay, drive slowly 2100-AD Docx Page 9 of 18

Use caution while transitioning load from the building on to the flatbed Pull forklift back and make sure forks are clear of the Inner Crate before raising forks or making turns 8.14 Strap crate down to bed 8.15 Store Linde E18C forklift for future lifts 8.16 DESI Spectrograph is driven from the IHF to the Mayall following the route outlined in red in Figure 6 JHA: CB, CW While walking alongside truck, take care to not come in contact with the truck and maintain safe distance. Be aware of your surroundings Note: DESI Spectrograph shall not be moved in inclement weather (or if ambient is below 0 C), this includes any weather conditions that would forbid dome crane use at the Mayall Note: Prior to move, recon and inspect the whole route for an obstacles that could hinder the move Note: Kitt Peak traffic controls shall be used where necessary to ensure safe load transport as well as safety to visitors 2100-AD Docx Page 10 of 18

Make sure area is clear of people and objects before moving load from flat-bed to the garage.
