15 Investigations Committee Updates Investigations Committee meeting was Monday, December 21 2:00pm To assist with determining how many investigations are overdue, a list of employees still out will be sent to Safety Initial OJI reports appear to be getting better with including additional conditions Based on the last Committee meeting, the group discussed the need for the top ten conditions to report when completing the initial OJI report. The group came up with the following six and an additional four will be identified If the OJI involves a vehicle, include the vehicle number and contact GSE Weather Lighting If bag was heavy, was a heavy tag attached If Cargo, the weight of the shipment Did the employee ask for assistance

Focus is on people and an awareness of employee safety and compliance. 18

19 Observations Committee Updates Observations Committee met on December 16 at 3:00pm Attendance significantly improved Reviewed November results and performance More even spread of entries through the month continues to be a challenge More ven spread of green positive and red corrective observations continues to be a challenge seeing a very high number of only green Reviewed current status for November observations targets On target Facilities observations attributes have been enhanced withthin eobs to reflect actual activities accomplished by the work group higher compliance rates overall for that department

26 Rules and Procedures Committee Updates Investigations Committee meeting Monday, December 21 3:30pm A safety risk assessment (SRA) underway for long tows/alternate tow plan is in progress suggested mitigations Eliminate wingwalkers if on a taxilane Enhanced communication, digital headsets Suggested review, guidance, or SRA for accessing aircraft for miscellaneous items STILL PENDING Passenger items, regulatory paperwork, etc. Touches multiple stakeholder departments Cust Care, Cust Ops, Flight, Flight Service, Tower, possibly business partners A systemwide issue Next meeting scheduled for January 18 at 3:30pm

Collaboration with the other committees is essential to continually improve safety at the DFW hub. 28

29 Activities Committee Old Business Update on scrolling sign with safety message. Request daily scroll, Julie will advise John Haywood. New Business Seatbelt compliance is now in DFW code of regulations Stay tuned for how it will be enforced. All department reps are being asked to remind their employees to report accidents or injuries to their supervisor immediately. Refer to: newjetnet.aa.com/community/our policies Julie working on local safety bulletin concerning portable space heaters. Initiatives December Safety Newsletter A/C Maintenance (Holiday fun,holiday stress, Holiday accidents). Currently meets 2 nd Thursday of each Month, next meeting, TBD 2016.

Maintains the DFW hub at a constant state of OSHA compliance and readiness 31

32 OSHA Committee Updates OSHA Committee met on December 1 at 2:30pm Discussed adjusting schedule for better participation Station oversight audit date not yet announced, but last year audit was in February Corporate Safety continues to accompany for self-audits Will continue with additional departments for upcoming months Next meeting scheduled for January 5 at 2:30pm

39 Recordable Rate System ATA Recordable By DFW Category Damage Event S80 4XG 12/11/2015 Cust Ops Customer Ops- Region Birdstrikes FOD Slides Customer Ops- US/Canada Customer Ops- Category Customer Ops- Outsourced Incident Description: During positioning at a remote pad by Customer Ops the right wing made contact with maintenance stairs Aircraft removed from service and initial inspection found damage to trailing edge of the right hand wing near the wingtip - trailing edge wedge requires replacement The maintenance stand was positioned correctly behind the equipment safety line Several attempts were made to maneuver the aircraft properly, however it was not positioned correctly during the approach to the pad or where the plane was ultimately parked/chocked on the lead-in line Root Cause: Non-compliance to policies/procedures Maintenance Contributing Factors: Inattention to task at hand/complacency Maintenance- Category ATA Recordable Cost- C4 Total Cost- C4 Risk Register
