3 INTRODUCTION Height Dynamics is an innovative height safety company that provides a diverse range of products and services. This is supported by an experienced rope access/height safety team with a background in the construction industry enabling them to perform repairs and maintenance on multi-story buildings. To ensure our work is of the highest quality, Height Dynamics enlists the services of specialist companies such as Structural Engineers and Protective Coating experts to offer technical advice and assist as required in the completion of repairs. We also have tradesmen who can work off rope including electricians, builders and painters. Working at heights is a potentially dangerous practice; Height Dynamics is here to assist you in: Inspections and Non-destructive testing on structures and spans. Provision of rescue supervision of persons whilst working at heights. Rope Access technicians for work at heights. Hire of equipment and appropriate levels of supervision to allow your personnel to carry out more specialized tasks. Nationally Accredited Training specifically suited to your organization. Development and Installation of Height safety systems on new or existing structures. Site Assessment or Suitability reports. Asset management including annual inspection and certification of height safety systems and equipment. Height safety equipment sales and maintenance. To ensure supplied equipment and height safety installations are maintained and re certified for safe use as per AS/NZS :2009 and AS/NZS :1997, an asset management tracking system unique to Height Dynamics is in place generating notification of due inspections and post inspection reports to ensure industry compliance and product serviceability. Height Dynamics competent inspectors boast specific equipment brand training to ensure the highest inspection standards. Height Dynamics adopts an innovative approach to Height Safety. We ensure that Australian Safety Standards are never compromised while fulfilling the needs and expectations of our customers. RESPONSIBILITIES It is the responsibility of the building owners or management, to ensure the safety of its workers by maintaining currency and integrity of installed height safety anchors and all associated components within any systems currently in use with the building. This responsibility includes certifying systems annually, through regular periodic inspections performed by certified, competent technicians to ensure systems and components are to a safe standard for rope access workers to perform relative tasks COMPLIANCE INFORMATION Height Dynamics use the below document for the use of reference and compliance to the required standards. 4 July 27, 2015 Queensland University of Technology Attn: Frank Milanesi 459 Tufnell Road Banyo QLD 4014 Dear Frank, RE: 2015 Anchor Installation Report Banyo Pilot Plant Precinct As part of our ongoing service to customers Height Dynamics have compiled a comprehensive overview of the work recently completed for Queensland University of Technology with regards to the Banyo Pilot Plant Precinct, Banyo. Please find attached our installation report complete with images showing the type and approximate location of the anchors. The walkway/guardrail system installed at Banyo Pilot Plant Precinct is a surface mounted system, positioned in the approximate locations as per the layout included in this report. Please note the following points: It is the recommendation of Height Dynamics that the height safety system described in this report is not used for any thing except that which it is designed for. The correct use of the safety system remains the responsibility of the owners of this building. Prior to use of the system, a risk assessment must be undertaken for the identification and control of hazards encountered when working at heights. Any persons using the installed safety system for working at height should do so only if they have received professional training to a Nationally Accredited level on the safe use of height safety equipment and systems relative to the task they are performing. Height Dynamics accepts no responsibility for the safety of inadequately trained / supervised personnel using the height safety system mentioned in this report.
