1 Revised Pre-Bid Minutes for Tender Number E-21, CT Scan Machine A Pre-Bid meeting was held in the Directorate of Health Services for tender E-21 on The following officers were present in this meeting. 1) Dr. R. Representatives from the following bidders were present in the said meeting. 1) M/s.Erbis Engineering. 2) M/s. 3) M/s.Philips India Ltd. 4) M/s. Gunjan Surgical. Expert remarks on queries raised in this meeting by the bidders is as follows Sr.N o. 1 Name of Bidder ERBIS ENGINEERING CO. LTD. Tender Specifications / Terms & Conditions The successful tenderer who is liable for award of contract should submit demand draft amounting to 1.5 % of order value to meet expenditure of tender processing i.e. publicity charges, experts honorarium, stationary charges etc.of sample testing fee and other incidental expenditure. Query Raised by Bidder Please note that 1.5% amount is very very high. Because you have asked for 5 nos CT Scan machines and accordingly the order value will also be huge. LTD. 18 : The goods should be delivered within 45 (Forty Five) days from the date of receipt of supply order to the consignee. The consignees may be Medical Superintendent/Civil Surgeon in the State as per quantity indicated in the supply order on door delivery basis as mentioned in The delivery period asked by you is 45 days, which is too short for delivery of 5 no.s CT Scan machines. As such the delivery period should be ammended to read as 120 days from the date of order. Please note that the CT scan will be imported by us from Japan by sea route and the total procedure of shipment itself takes minimum 45 days. 18 : The goods should be delivered within 90 (Ninety ) days from the date of receipt of supply order to the consignee. The consignees may be Medical Superintendent/Civil Surgeon in the State as per quantity indicated in the supply order on door delivery basis as mentioned in

2 3 ERBIS ENGINEERING CO. LTD. We understand that, the CT Scan machines are to be installed at the existing CT Scan sites of Civil Hospital Thane, Nashik, Bhandara, Wardha & Chandrapur. We do not know the exact condition of these sites because the sites are quite old. We feel that the sites should be completely ready for installation of new CT Scan machines before we deliver the machines. As such we request you to please inform us whether any work is to be carried out at these sites. If yes, please let us know the exact work to be carried out, so that, we can quote our price for the said job also along with CT Scan price in the tender. The Bidders should visit installation site (which include the Gantry, Console and UPS area) at Thane, Nashik, Chandrapur, Wardha, Bhandara & Amaravati Civil hospitals and review the civil, electrical and air conditioning pre-installation requirement in view of installing the CT scan machine in compliance with AERB norms. The bidder should quote the average cost for the pre-installation requirement in accessory section of the Price Schedule along with the CT Scan Machine cost. The Lowest Price will be calculated considering Machine cost + Average Pre-installation requirement Cost. The entire Unit along with the Civil, Electrical & Airconditioning work will carry two years warranty. Total Quantity of CT Scan Machine is 6. Therefore allow a user to compare low contrast resolution of different CT scanners. All manufacturer of CT scanners in world have the low contrast resolution on an 8 inch (20cm) CAPTHAN Phantom. This change shall allow your technical experts to compare the different CT scanners. The low contrast resolution should not be more than 5mm.Shoulder, Pelvis Streak Artifact suppression Software should be standard : Special Software ( Like MA Modulation in Routine ) to ensure Dose efficiency should be standard. make a new Dose reduction technique i-e Itterative Reconstruction technique on the RAW Data space as standard. This allows all patients to be scanned with upto 40% Lower dose. This is now available with all latest CT scanners of all manufacturers. By Including the Itterative Reconstruction technique on the RAW data space. Shall benefit all patients at your district hospital to be scanned at upto 40% lower dose. Which is help in unnecessary X ray radiation to patients. 5 12 20 : Inspections and tests Pre-dispatch inspection will be carried out in the premises of the manufactures by a team of officers nominated by the Purchaser. If goods are offered for Inspection in the factory premises all expenditure shall be borne by the tenderer. The entire store ordered shall have to be offered for inspecting team for inspection in open condition. Inspection charges, including the expenses for the experts, will be payable by the tenderer. carry out the inspection at the respective sites where the equipments are to be delivered. The Warraty will inclusive of CT X Ray Tube and all spare parts and accessories like UPS, Injector, Printer (Inkjet, Laser, Dry printer) and will exclude consumables like UPS batteries, Injector syringes, printer cartridge or ink and films : 22.5 Annual maintenance Contract & CMC confirm that post the warranty period CMC shall include CT X ray and accessories supplied with the CT scanner.the warranty shall exclude consumables like UPS Battries,Printer Ink, Injector patient tubes, films etc. This is essential to maintain uptime. The CMC will inclusive of CT X Ray Tube and all spare parts and accessories like UPS, Injector, Printer (Inkjet, Laser, Dry printer) and will exclude consumables like UPS batteries, Injector syringes, printer cartridge or ink and films.

6 15 25 : Payment. The 80% Payment shall be made upon submission of following documents: (i) 3 copies of supplier's invoice. (ii) Receipt certificates issued by the consignees. The balance 20% Payment shall be made after installation of equipment. amend this term to a 100% INR Inland Letter of Credit. Where in 80% shall be released on submission of documents with proof of shipment and 20% on Installation. As the CT scanners has to be imported from our principles we request you accept the suggested terms. No Change.
