2 SUPPLEMENTAL GENERAL CONDITIONS (SGC) Elevator Maintenance and Repair Services SGC-1 PURPOSE OF THIS BID The purpose of this bid, and the requirement contract awarded following an evaluation of the bids received, is to provide preventive maintenance and repair services as listed in the Detailed Specifications for twenty-two (22) elevators at six (6) different facilities owned and operated by the Northeast Ohio Regional Sewer District (District/NEORSD). SGC-2 DURATION For the purposes of this bid, the duration of the requirement contract is for a two (2) year period, or until the funds appropriated to this contract are exhausted, whichever should occur first from the effective date of this contract. SGC-3 CONCURRENT WORK WITHOUT SUBCONTRACTING Concurrent work may be required at the six (6) District facilities. The contractor must have a sufficient number of experienced personnel and contractor-owned equipment needed to perform the work specified. No subcontracting shall be permitted. SGC-4 EMPLOYEE QUALIFICATIONS & COMPANY REFERENCES Please complete the two (2) forms attached with the following information. Forms must be attached to the bid submitted: 1. The names, qualifications and experience of the Elevator Service Technicians to be assigned to work on the District elevators. 2. A list of a minimum of five (5) companies with whom the bidder has entered into agreements for elevator preventive maintenance and repair and the year each of the agreements was entered into. The bidder must demonstrate through such information that it has at least six (6) years of elevator preventive maintenance and repair experience. The list shall include contact names and telephone numbers. The list of companies may include the District. The information obtained from contacting the companies on the lists will be used to check customer service history and may be used as a factor in awarding the contract. SGC-5 MANDATORY ELEVATOR INSPECTIONS All contractors, who have not previously held the elevator contract in the last four (4) years, are required to acquaint themselves with the elevators and the scope of the tasks to be performed under this contract. Contractors must make an inspection of the elevators at each facility prior to March 18, 2016 and determine all the conditions related to the fulfillment of this contract. The required inspections may be made by appointment with the designated representatives listed below. Bids may not be considered responsive if the vendor fails to perform a mandatory inspection. Also listed are the addresses, telephone numbers and number of elevators at each facility. SGC-1

Likewise, no pleas of ignorance of conditions that may exist or difficulties that may be encountered after commencement of the work under this contract will be accepted if it is determined that these complications were a result of the bidder's failure to make reasonably prudent examinations and investigations. The District reserves the right to cancel the contract awarded at any time it is determined that service is inadequate. SGC-6 MANDATORY ELEVATOR CONDITION REPORTS By the end of 60 days from the start of the contract, the successful bidder shall conduct an Elevator Condition Report for each of the twenty-two (22) elevators listed in the Detailed Specifications. The condition reports shall be prepared based on the contractor s experience and knowledge of elevator systems. This condition report is an opinion from the contractor related to the maintainability of the elevator, an estimate of longevity, and shall list any observed problems. The condition reports shall be provided to each District Facility Representative. SGC-7 BID INQUIRIES All questions regarding any irregularities and request for information should be submitted via e- mail to the Northeast Ohio Regional Sewer District s Operation and Maintenance Dept., Delilah Capeles at capelesd@neorsd.org and they will be answered as soon as is practicable. Those questions regarding information that, in the opinion of the District, should be available to all persons or firms submitting a bid, will be distributed as an addendum to the bid documents. The District will not be responsible for any oral instructions and/or opinions. Questions will not be answered after 12:00 p.m. (noon), March 23, SGC-2

4 1DETAILED SPECIFICATIONS (DS) Elevator Maintenance and Repair Services DS-1 SCOPE OF WORK The contractor(s) shall furnish all labor, materials, supplies, equipment, diagnostic and other specialized tools and incidentals necessary to perform the work in accordance with these specifications on the twenty-two (22) elevators listed by facility location below. All elevators noted below with the exception of the three (3) shown in red italic text, have phase I and/or phase II fire service and therefore require a monthly inspection per ANSI A-17.1 Refer to section SGC-19 for additional details. The monthly inspection logs are to be kept in the elevator equipment room. 5 operation after the Elevator Service Technician leaves the facility, a safety lock and tag for proper lock-out/tag out procedures shall be left on the power panel(s) by the contractor. In addition a sign must be left on all elevator floor doors that legibly reads Out of Order, The contractor s Out of Order sign, lock(s) and tag(s) shall remain until the repairs are completed. 5. The Elevator Service Technician(s) upon completion of the repairs or at the end of the work shall contact the Designated Facility Representative and provide an Elevator Service Ticket and status of the elevator(s) prior to departure. 6. The Designated Facility Representative will also sign the Elevator Service Ticket at the completion of each site visit. DS-5 PREVENTIVE MAINTENANCE The contractor shall perform Bi-monthly preventive maintenance and routine repair service on three (3) elevators covered in this agreement. The remaining nineteen (19) require Monthly preventive maintenance and routine repair service inspections in compliance with ANSI A The scheduled date/time for the preventive maintenance shall be pre-arranged with the designated District representatives at each facility prior to reporting. Preventive maintenance on the three (3) elevators designated in section DS-1 shall be performed a minimum of once every two (2) months unless the contractor based on their experience determines that more frequent visits are needed for certain elevators to keep them in optimal operating condition to fulfill the requirements of the contract. All preventive maintenance inspections and routine repair service shall be paid under the price bid. No advanced billing is allowed. Payments will only be made after the following has been completed: 1. Bi-monthly preventive maintenance visits 2. Monthly preventive maintenance visits 3. Written Service Reports (refer to section DS-6) In addition, calls for minor repair service that can be performed on site, the same day, by a technician, may not count as preventive maintenance service visits. If following a preventive maintenance and routine repair services visit, additional repairs beyond the work covered under the preventive maintenance and repair sections of these specifications are identified, the contractor shall notify the District immediately. A written estimate for performing the additional work shall be prepared and provided to the District within forty-eight (48) hours of the notification of the problem The preventive maintenance shall include, but is not limited to, the lubrication, adjustments, cleaning, inspection and load testing of the elevators and associated equipment. The contractor s experience and the recommended preventive maintenance from the elevator manufacturer shall also be utilized. A. LUBRICANTS AND FLUIDS - The contractor shall furnish and apply all greases and synthetic lubricants, hydraulic, operating and lubricating oils and any and all other needed fluids. The amount of lubricants and fluids shall be applied in a manner as recommended by the manufacturer so as not to cause conditions of over or under application. DS-6
