Included in the attachment is an adendum notification which details the changes and the respective proposal pages which were added and/ or changed. Except for new bid insert pages, it is unnecessary to return any of the pages attached. Bid insert pages must be returned with the bid proposal submitted to the Department, unless your firm is submitting a bid using a computer print out. The computer print out must be changed to reflect the new bid item information. Contractors and material suppliers, etc. who have previously been furnished informational proposals are not being furnished a copy of the addendum. If you have a subcontractor on the above project, please advise them of this addendum. Acknowledgment of this addendum is not requested if your company has been issued a proposal stamped This Proposal Issued for Informational Purposes. You are required to acknowledge receipt of this addendum on the Addendum Acknowledgement form contained in your bid proposal by placing a mark in the box next to the respective addendum. 3 Sheet 8A GENERAL NOTES: Specification Data Basis of Estimate Item Description Rate Unit 314 Asph Mat'l (CSS-1P) 0.2 gal/sy* ** Gal 341 Hot Mix (All Types) 115 lb/sy/in Ton Mailbox manipulation made necessary because of construction shall be done in accordance with Item 560, except that this work will not be paid for directly but will be considered subsidiary to the various bid items. Remove any obstructions to existing drainage due to the contractor's operations, as required, at the Contractor's expense. Item 8. Prosecution and Progress Working days will be computed and charged in accordance with Article 8.3.A.4, Standard Workweek for the north and south end of SH 171 and the south end of SH 174. SH 174, north end, work hours will be restricted from 9 am to 4pm in accordance with Standard workweek. There are 48 working days to complete the work on this contract. Item 134. Backfilling Pavement Edges TXDOT will provide type A backfill material. The type A backfill material (RAP) is available from project millings or project stockpiles. Provide emulsified asphalt CSS-1P at a rate of 0.2 GAL/SY to the finished backfill material. Item 301. Asphalt Antistripping Agent Furnish a liquid antistripping agent unless directed. Item 305. Salvaging, Hauling, and Stockpiling Reclaimable Asphalt Concrete Stockpile all rap salvaged and not used for this project at the following locations: Sheet A

4 Sheet 8B Salvaged material from SH 171 north of Godley is to be stockpiled on the southeast corner of FM 917 and SH 171. Salvaged material from SH 171 south of Cleburne is to be stockpiled at Parker on SH 171. Salvaged material from SH 174 north of Cleburne is to be stockpiled on the overpass slope on the north side of US 67 across from Home Depot in Cleburne. Salvaged material from SH 174 south of Cleburne is to be stockpiled in the island where SH 171 & SH 174 converge south of Cleburne. Build stockpiles between 10 and 15 feet in height with layers approximately 2 feet in depth. Item 341. Dense-Graded Hot Mix Asphalt (QC/QA) Use of RAP is allowed only for base and underlayment courses on this project. RAP aggregate must meet the requirements of Table 1. Provide aggregate with a Surface Aggregate Classification value of A for the surface course of the travel lanes. Provide a PG70-22 asphalt for the surface course. Provide a PG64-22 for base repair. Furnish a CSS-1P for the tack coat on this project. From Table 7: The tensile strength is waived for this project. Use the boil test, test method TEX-530-C, and provide only mixes that produce zero percent (0%) stripping for design verification and during production. The requirements shown in Table 8 are required for this project. Place mixture when the roadway surface temperature is equal to or higher than the temperatures listed in Table 1 unless otherwise approved or shown on the plans. Measure the roadway surface temperature with a handheld infrared thermometer. The Engineer may allow mixture placement to begin prior to the roadway surface reaching the required temperature requirements if conditions are such that the roadway surface will reach the required temperature within 2 hrs. of beginning placement operations. Unless otherwise shown on the plans, place mixtures only when Sheet B

5 Sheet 8C weather conditions and moisture conditions of the roadway surface are suitable in the opinion of the Engineer. Minimum Pavement Surface Temperatures High Temperature Binder Grade Minimum Pavement Surface Temperatures in Degrees Fahrenheit Subsurface Layers or Night Paving Operations Surface Layers Placed in Daylight Operations PG Note 1: Contractors may pave at temperatures 10 F lower than the values shown in Table 1 when utilizing a paving process or equipment that eliminates thermal segregation. In which cases, the contractor must use either an infrared bar attached to the paver, or a hand held thermal camera, or a hand held infrared thermometer operated in accordance with Test Method 244-F to demonstrate to the satisfaction of the engineer that the uncompacted mat has no more than 10 F of thermal segregation. Item 502. Barricades, Signs, and Traffic Handling Maintenance of roadways, not paid as "constructing detours", and designated in the traffic control plan to carry traffic, will be the responsibility of the Contractor and will be paid for by "Contractor Force Account or Agreed Unit Price". Permanent signs may be installed when construction in an area is complete and they will not be in conflict with the traffic control plan for the remainder of the job. Existing signs are to remain as long as they do not interfere with construction and they do not conflict with the traffic control plan. Any sign not detailed in the plans but called for in the layout shall be as shown in the current "Standard Highway Sign Designs for Texas. When traffic is obstructed, arrange warning devices in accordance with arrangements indicated in the latest edition of the "Texas Manual on Uniform Traffic Control Devices". Cover or remove any work zone signs when work or condition referenced is not occurring. Sheet C

6 Sheet 8D Special Traffic Control Notes: 1. Reopen all lanes to traffic at the end of each daily construction period. Temporary tabs are required where pavement markings have been removed. 2. During full width milling operations, complete full width of the milling across the roadbed by the end of each daily construction period. Provide 10 ft. minimum transition to abutting pavement. 3. During overlay operations complete full width of the paving across the roadbed by the end of each daily construction period. Provide 5 ft. minimum transition to abutting pavement. 4. Place permanent thermoplastic striping at 25%, 50%, 75% and 100% of completed paving operations. 5. Pilot vehicle required for one way traffic operation when directed by the engineer. Item 504. Field Office and Laboratory Furnish the following structures for this project: Type Field Lab (Ty. A) 1 Field Lab(Ty. D) 1 No. Furnish the following for the Field Office structure. The copier and facsimile machine must meet the requirements of DMS-10101, Computer Equipment. 7 Sheet 8E Enclose the field office or laboratory and the parking area with a 6-ft. chain-link fence, a topmounted 3-strand barbed wire, and a 12-ft. gate. Provide one cell phone with service for the use of the chief inspector. This cell phone is in addition to the required telephone and telephone service for the Field Office structure. This phone is not in lieu of the Field Office telephone and facsimile service. Laptop Microcomputers shall have an Intel Pentium IV (1.5 GHz) processor or better. Integrated printer/copier/scanner/fax units are permitted. Item 506. Temporary Erosion, Sedimentation, and Environmental Controls The SW3P for this project shall consist of using the following items as directed: a. Temporary erosion control logs b. Temporary seeding for erosion control c. Earthwork for erosion control Remove accumulated sediment and/or replace SW3P controls when the capacity has been reduced by 50% or when the depth of sediment at the control structure exceeds one foot. Item 585. Ride Quality for Pavement Surfaces Use Surface Test Type B pay adjustment schedule 3 to evaluate ride quality of the travel lanes in accordance with Item 585, Ride Quality for Pavement Surfaces. Item 618. Conduit A continuous bare or green insulated copper wire No. 6 or larger shall be installed in every PVC and rigid metal conduit throughout the electrical system in accordance with the electrical detail sheets and the latest edition of the National Electrical Code except for conduit with loop detectors only. PVC conduit shall be heavy wall schedule 40 unless otherwise approved by the engineer. Sheet E

8 Sheet 8F Item 668. Prefabricated Pavement Markings Provide Type C (heat applied) prefabricated pavement markings. Item 688. Traffic Signal Detectors The contractor shall, unless otherwise directed by the engineer, arrange the installation of loop detectors or other work in the roadway so as to close only one lane at a time. Construction operations should be conducted to provide minimal interference to traffic. Unless otherwise shown in the plans, all loops in the same lane shall be the same color and have the following color code. (When facing the direction that traffic flows, the color code will read from right to left for all lanes carrying traffic in that direction.) The first loop on the right shall be white followed by black, orange, green, brown and blue. Unless otherwise shown on the plans, vehicle detector loop wire shall be No. 14 AWG XHHW and shall be black in color. Color coding shall be a half lap of colored tape, six inches in length, on each pair of wires in the ground box. Splices for loop to loop lead-in connections will be permitted only with Scotch Cast, Hysol Splice Kit or equal. Note: There is a Special vehicle detector loop for Phase 8. The location is designated on the Intersection Layout plan sheet. The Special Type III detector loop installation details are presented on sheet Details of Detectors and Ground Boxes. Measurement of this item shall be paid by the linear foot of sawcut containing loop wire, and payment shall be made under item 688, Traffic Signal Detectors. Special Specification Reflectorized Pavement Markings with Retroreflective Requirements. If retroreflectivity readings can not be performed within the 10 day period, then the performance period (F.3. Retroreflective Requirements) will extend until readings are taken at Contractor s risk. Contractor will be responsible for replacement of all markings that do not meet reflectivity requirements at the time readings are taken. Payment will not be made on pavement markings until Contractor provides State with acceptable retroreflectivity data, meeting minimum requirements of F.3. Sheet F
