Delivery Time: days. The quotation must include a delivery time estimate, expressed in number of calendar days, after receipt of order. All items are required to be collected in the Supplier s shop for inspection in single location before delivery, and all items inspected must be the items intended for the delivery. 2. Delivery location: The Delivery location is Mar Matia, Mosul. Specific details of delivery location and point of contact are to be provided as an Annex to the resulting contract. 3. Prices: Quotations for all items must be fixed price, inclusive of delivery price, expressed in USD. 4. Validity of submitted quotation: Quotations should be valid for at least thirty (30) calendar days. 5. Taxes and VAT: No such Cooperating Country taxes, VAT, charges, tariffs, duties or levies will be paid under an order resulting from this RFQ. 6. Evaluation and Selection:

4 The award will be made to a responsible supplier whose offer follows the RFQ instructions, meets the eligibility requirements, and is determined via a trade-off analysis to be the best value based on application of the following evaluation criteria. The relative importance of each individual criterion is indicated by the number of points below: Technical 20 points: Responsiveness to the technical specifications and requirements. Please note that all offerors are responsible to deliver sample units for requested items and the evaluation point will be provided based on sample unit and during the actual delivery all items will be checked if the quality is different than the sample units the contact will be canceled. Please be prepared to deliver sample unit salon with the full quote. Delivery 30 points: The speed of delivery. Please specify the delivery date in calendar days. Price 50 points: The overall cost presented in the offer. 7. Purchase Order: For the selected best quotation, contractual obligations between ICRI and vendor will be set up by means of Purchase Order using ICRI standard terms and conditions. 8. Warranty Requirements: Warranty service and repair within the cooperating country is required for all commodities under this RFQ. The warranty coverage must be valid on all commodities for a minimum of twelve (2) months after delivery and acceptance of the commodities. 9. Eligibility: By submitting an offer in response to this RFQ, the supplier certifies that it and its principal officers are not debarred, suspended, or otherwise considered ineligible for an award by the U.S. Government. Chemonics will not award a contract to any firm that is debarred, suspended, or considered to be ineligible by the U.S. Government. 0.
