TOLL RESTRICTION Access lines equipped with Toll Restriction will prohibit the completion of the following types of calls: Message Telecommunication Service (a.k.a Local Plus), Interexchange Toll Service including PIC, 10XXX and 950 access, 900 /1/ and 700 calls, 0-, 0+ and 00+ originating calls, collect calls and third number billed calls. /2,4/ Access to the toll network is not allowed and the customer agrees not to access the toll network by other available means. Toll Restriction will only be provided to the following types of service: Residence Flat Rate Service /3/, Residence Measured Rate Service /3/, Universal Lifeline Telephone Service /3/ and single line business service. Customers will continue to have access to the following types of calls: Local calls, Zone 1 and 2 (1+ local calls), 800 calls, 411 (directory assistance), 611 (repair), and 911 (emergency service). Toll Restriction will be provided where facilities and operating conditions permit. The customer with Toll Restriction will be responsible for the payment of all completed calls as set forth in Schedule Cal.P.U.C. No. A Rendering and Payment of Bills. It is the responsibility of the customer to inform the Company of any failure of the service to perform as specified herein. K. PRODUCT GROUPING The Company may refer to groups of products and/or services by distinctive, collective phrase(s). These phrases will be used when discussing the Company's product line with customers and in advertisements. (Group names may not be included in Guidebook descriptions.) /1/ California 900 and California 976 services withdrawn effective November 1, /2/ Some calls originating from locations that do not have screening capabilities may not be intercepted and denied. Access Line Signaling Arrangement Access lines services are furnished with rotary dial service and Touch Tone Calling Service as the standard signaling arrangements. 6. Reserved 7. Toll Terminal Lines Measured rate toll terminal lines are furnished at charges and rates equivalent to that of business measured rate individual access lines. USOCs LD2 and ZZYT2 are furnished for terminations where primary station sets are not required. 8. End User Common Line Access (EUCL) Offset The End User Common Line Access charge (EUCL) stated in Tariff F.C.C. No. 128 will be offset by a change in equal amount in the monthly measured service rates, as shown in B.4.c. following. The offset rates shall become effective upon approval by the California Public Utilities Commission. 9. It is the customer s responsibility to provide, and update if necessary, accurate Automatic Number Identification (ANI) and Automatic Location Identification (ALI) sub-address information to the 911 database administrator. Once the customer provides ANI and ALI sub-address information to the 911 database administrator, it is the responsibility of the Company to provide the location of the pilot number to the PSAP for 911 calls, and where technically and operationally feasible the Company will deliver ANI to the PSAP at a station level behind a Multiline Telephone System.

Conference calls and any other calls requiring operator handling except as specified in C.2.b., following, are not included and will be billed at the regular toll or ZUM rate schedule applicable to the type of call. b. Metro Plan SM may not be furnished to any residence exchange service (except Remote Call Forwarding service as specified in Guidebook, Part 7, Section 4) that is interconnected, physically, acoustically or by any other means to any other line, facilities or service at the customer's premises to extend a two-point connection beyond the originating station and called station location. c. The premises of a customer is that shown in Schedule Cal.P.U.C. No. A2.1, Premises. In the event the premises is located in more than one exchange, the premises will be considered as in the exchange in which the primary service is located. d. Metro Plan SM will not be furnished in connection with multiparty (operator identified) or any business service (except Centrex Dormitory service with billing arrangement Option "C" as specified in Guidebook, Part 5, Section 1 e. Metro Plan SM is limited to one plan per one-party residence service. The Service Area subscribed to by the customer must be the Service Area in which their residence service is served from. f. The Metro Plan SM cannot be added to foreign exchange services. g. Toll restricted access lines will not be eligible for Metro Plan SM service.

Most Economical Routing (MER) - #2ESS Type Centrex (1) Charges applicable to changes and rearrangements. Where a change in the quantity of patterns is required, the charges and rates for the increased patterns shown in D.9.e.(2) following, apply. (2) Call Detail is available for calls placed via MER service. Centrex customers using MER service may obtain this call detail by subscribing to Supplemental Billing Service. The Call Detail available and the applicable rates and charges are shown in Guidebook, Part 8, Section 8 USOC items Y18, BDTXW, CMM, CMMAA and BDTXF. k. Where tie line or interexchange channel - interwire center private line telephone line service is furnished with AIS, only the AIS stations of the same customer are provided access to these lines. These lines may access stations of the same or other AIS customers. b. Arrangements using central office equipment for access codes are offered subject to the operating limitations of the central office equipment and the associated terminating equipment. c. Equipment arrangements, rearrangements and services available for use with Dial-Series 100 and 300 PBX service and Centrex service will be furnished where suitable, with AIS unless otherwise specified in the Guidebook. Where furnishing such arrangements and rearrangements involves costs in excess of those contemplated by the Guidebook, additional charges may be applicable, subject to prior authorization of the Public Utilities Commission of the State of California. d. Joint user service is applicable and will be furnished upon application by the customer subject to the rates set forth in D.10. following, and regulations of Guidebook, Part 4,Section Sectional Billing Service a. The Company will render to the customer, in accordance with D.3.f. following, a statement that will consist of two or more sectional bills as determined by the customer. b. A sectional bill may include all exchange message, zone calling (ZUM) units and message toll charges, credits, charges and rates for exchange service, supplemental equipment, directory listings and classified directory advertising charges associated with AIS Centrex station service line numbers included in each sectional bill. c. Other rates and charges considered to be common to the AIS Centrex station service not normally associated or identified with AIS Centrex service lines will be included on one of the bills designated by the customer. d. Each sectional bill will be rendered in the customer's name to the AIS Centrex station service who is responsible for the payment of all sectional bills applicable to the service.

Except for the optional services specified in this Guidebook, (C. 1.,2., 3.), Directory Number Call Forwarding is not available with all of the services available to Network and Exchange grades and classes of service listed in the Guidebook. j. A local interconnection service must be established to provide the exchange of voice telephone traffic, that includes calls forwarded by DNCF from the Company s switch to the CLC s switch. The CLC must provide to the Company, information to permit the Company to request the required incremental trunk capacity to meet the expected DNCF demand. k. DNCF may be subject to transmission limitations, so that the quality of the transmission of calls which are forwarded may vary depending on the network routing necessary to complete each call to the CLC switch. The Company does not guarantee the transmission of data information over DNCF Service. l. The installation charge and monthly rate are applicable to each DNCF service. m. DNCF only applies when the CLC s customer remains in the same physical location or a location within the same area served by the Company serving central office. If the CLC s customer changes their physical location to a location associated with a different serving central office, the DNCF must be disconnected. n. Reserved o. A CLC may order up to a maximum of 99 call paths associated with the same CLC customer for a single CLC number. p. In exception to regulation i. in this Guidebook, DNCF is available to numbers that were in service as Remote Call Forwarding service as described in Guidebook, Part 7, Section 4 and Foreign Exchange Service as described in Guidebook, Part 4, Section 3, preceding. q. DNCF is available to facilities based and non-facilities based CLCs.

Directory Listings or Directory Assistance Listings A Directory Listing for DNCF numbers will be furnished as a primary listing in the alphabetical (white) section of the telephone directory that serves the exchange in which the DNCF number is located, and/or a Directory Assistance Listing (only) will be furnished (DNCF numbers included in the directory assistance data base) as set forth in Schedule Cal.P.U.C. No. 175-T,Section Alternate Billing Services /1/ a. Collect calls to the DNCF number will be allowed upon request of the CLC, otherwise collect calls will be blocked. The CLC is responsible for billing it s customer. b. Bill to Third Party calls to the DNCF number will be allowed upon request of the CLC, otherwise Bill to Third Party calls will be blocked. The Remote Call Forwarding service does not include a local message unit allowance or a zone calling unit allowance. c. The charges for forwarded calls may be comprised of two separate message charges: (1) A charge for that portion of the call from the originating station to the call forwarding location, if applicable; (2) The Dial-Station-to-Station charge or FGB usage charge for that portion of the call from the call forwarding location to the terminating location designated by the customer. d. The respective charge for each such portion c.(1) and c.(2) preceding shall be the rate specified in Guidebook, Part 4, Section 2; Part 9, Section 1;and Part 10, Section 1. e. On an Operator Station-to-Station call, the originating customer is charged the respective Operator Station-to-Station rate for the portion covered in c.(1) preceding. The charge for the portion of the call described in c.(2), preceding is the Dial Station-to-Station rate and charged to the call forwarding customer or FGB usage charge and charged to the FGB customer.

Description Toll Blocking will prohibit the completion of billable toll calls. Customers will continue to be able to complete the following types of calls: 411 (directory assistance), 611 (repair), 911 (emergency service) and 800 calls. Attempts to complete billable calls will be blocked. Customers subscribing to Toll Blocking will have the option of placing toll calls using the IEC s calling card, if calls are placed from lines that are not equipped with Toll Blocking. Customers subscribing to Toll Blocking will also have the option of accepting or rejecting collect and third number billed calls. /1,3/ 2. Regulations a. Toll Blocking is only available to customers with the following types of service: Residence Flat Rate Service, Residence Measured Rate Service, California LifeLine and single line business service. b. Toll Blocking will prohibit the completion of the following types of calls: Message Telecommunications Service (a.k.a. Local Plus), Interexchange Toll Service (including PIC), 10XXX, 950 access, 900, 700 service, 0-, 0+, 00+ originating calls, collect calls, and third number billed calls /1, 3/. c. 17 PART 9 - Message Toll Services 2nd Revised Sheet 1 SECTION 1 - Message Telecommunications Services and Rate Schedules 1. TWO-POINT MESSAGE TELECOMMUNICATION SERVICE /1/ A. GENERAL 1. Description A toll telephone message is a completed call or telephonic communication between two exchange stations located in different local service areas, between toll stations or between a toll station and an exchange station to which rates are applicable in accordance with the provisions of this Guidebook. 2. Regulations a. The toll service charges specified in this Guidebook are in payment for all service furnished between the calling and the called stations. b. Toll service is classified and rated as either: /2/ - Dial station - Operator station c. Regulations applicable to Sections A. thru D. following. Use of Service for Unlawful Purposes Legal requirements for refusal or discontinuance of service are set forth in Guidebook, Part 2, Section 2. d. The designated commercial credit cards, authorized by contract for use, are as follows: American Express (AE), Master Card, VISA, Carte Blanche and Diner's Club, JCB (Japanese Credit Bureau). /1/ Also known as Local Toll. /2/ Also see Guidebook, Part 11, Section 1.

GENERAL (cont'd) 2. Regulations (cont'd) f. Express Call Completion (cont'd) This service is furnished solely for the telephone calling purposes of the caller. Provisions concerning limitations of liability and allowance for interruptions in service are set forth in Guidebook Part 2, Section 2. The usage associated with a call will be subject to any applicable discount. Centrex, PBX and Commstar II /2/ customers with toll restriction will be offered ECC on an alternate billing basis only. /3/ g. The MTS usage charges associated with up to five (5) ISDN lines, used for data and video applications, for Public Schools, Libraries and Community Colleges and Private Schools as defined in Guidebook Part 2, Section 2, participating in the Education First Program shall be waived for a period of one (1) year. The one (1) year period shall begin on the date of installation of the ISDN lines and continue for twelve (12) months after which time the customer shall begin paying the usage charges associated with these lines. h. When calls are made to or from prefixes used by Emergency Preparedness Trailers within the lata of each prefix, no charge will be made to the calling party. For calls made outside of the LATA, toll charges will apply. The Emergency Preparedness prefixes are: LATA AREA CODE PREFIX /1/ Also known as Local Toll. /2/ Commstar II is a Grandfathered service. See Part 20, Section 7, for details. /3/ Effective March 19, 2016, Collect, Person-to-Person, Bill to Third Number calls and Busy Line Verification/Interruption services are discontinued.

GENERAL (cont'd) 3. Territory Between two points within a LATA where the respective rate centers of such points also are located in said LATA. 4. Rates and Charges a. Method of Applying Rates /1/ (1) Toll rates between points (cities, towns or localities) are based on the airline distance between rate centers. In general, each point is designated as a rate center; certain small towns or localities are assigned adjacent rate centers with which they are closely associated for communication purposes. (2) Determine the rate airline distance between the rate centers involved. Where a point is divided into district areas specified in Guidebook, Part 9, Section 2, the rate airline distance is the mileage between the rate center of the district area and the rate center of the other point, except that when such mileage exceeds 40 miles the rate airline distance is the mileage between the exchange rate center and the rate center of the other point with a minimum of 41 miles. If both points are divided into district areas the rate airline distance is the mileage between the rate centers of the district areas, except that when such mileage exceeds 40 miles the rate airline distance is the mileage between the exchange rate centers with a minimum of 41 miles. (3) Refer to (11) following. Opposite the mileage step which includes the mileage determined as in (2) preceding, will be found the rates for sent-paid dial initial periods and rates for additional minutes for all classes of service between the rate centers involved. Below the table are the service charges for operator station service. /1/ In addition to the rates shown in this schedule, one or more of the operator assistance service charges in Guidebook Part 11, Section 2, may apply per message.

Three months prior to the end of the term, a letter will automatically be generated to alert the customer they are nearing the end of their term. At that time, the customer must select an option of either rolling over to another term of the same length or choose the alternate term. If the customer takes no action the current term will be automatically renewed for the same length. Customers will have a 60-day grace period after the new term begins to cancel the new term without incurring any penalties. 10. If the customer elects to renew their term or is automatically renewed, the customer will receive a Loyalty Bonus credit after 60 days after the end of the grace period as set forth in B.9. preceding. 11. At the expiration of the current term if the customer elects to renew their 24 month term or migrate from a 36 month term to a 24 month term they will receive a Loyalty Bonus credit of $500. Customers who renew their 36 month term or migrate from a 24 month term to a 36 month term will receive a loyalty bonus credit of $1,000. Renewal will be at the current Guidebook rate. 12. If the Guidebook rates for Access Advantage Plus Packages or the Access Advantage Plus Packages options and connecting services decrease during the term the customer will automatically receive the lower Guidebook rate. If the rates increase, the current rates will be grandfathered for the duration of the term. 13. Installation charges may be waived if a Access Advantage Plus Packages customer moves from one location to another location. In order to receive the waiver of installation charges, the customer must commit to a new 36 month term at the current rate. The new 36 month term starts when the service order for the change of address is completed. If the customer does not commit to a new 36 month term, they may elect to pay installation charges for their move and maintain the Access Advantage Plus Packages service for the remainder of the initial term. The customer must remain a retail AT&T California customer and the new location must be within AT&T California s serving area where Access Advantage Plus Packages is technologically available.

One-party residence exchange service customers will make local toll calls which includes 1000 minutes of calling and a postalized rate for local toll calls over 1000 minutes. The local toll calls over 1000 minutes are rated at a flat per minute rate regardless of mileage band or time of day. f. Each individual call is computed on the basis of whole minutes, a fraction of a minute being considered a whole minute. For all messages, a fraction of a minute is charged as a full minute. For Saver 60, Saver Plus and OnePrice Saver, the minutes of calling included in the monthly rate are computed by first adding the customer s peak minutes calls followed by off-peak minutes calls, if needed, to total 60, 80 or 1000 minutes of calling identified with each of the respective plans. The calls must be service area calls within California and for the service area in which the customer resides. g. Minimum billing as set forth in Guidebook, Part 2, Section 2 is applicable to the monthly rate for Saver 60, Saver Plus and OnePrice Saver. C. REGULATIONS 1. Limitations of Service a. DESCRIPTION Customer-owned pay telephone (COPT) service provides an access line for use with a pay telephone set (including pay telephone sets owned by the Utility) which is equipped with Touch- Tone dialing capability. Access lines can be ordered for use with an instrument-implemented pay telephone set ( smart set ) or a central-office-implemented pay telephone set ( dumb set ). Most services can be ordered as either outward only or bothway service. This service allows the customer, within certain limitations, to establish the call rate for sent paid local and long distance calls placed from the COPT. The customer, for purposes of this service, is the person or entity, including the Company, who subscribes to the access line. B. TERRITORY Within the exchange areas of all exchanges as said area is defined on maps filed as part of the tariff schedules.
