Tender documents can be obtained from the office of Materials department, Scooters India Limited (A Govt. of India Enterprise) Head Office & Registered Office, Post Bag No. 23, Lucknow , on all working days on payment of Rs. 500/- (Rupees Five Hundred Only) by demand draft in favor of Scooters India Limited payable at Lucknow towards cost of tender documents. Tender documents can also be down loaded from our website www. scootersindia.com. Bidders quoting documents can also be downloaded from website should submit a DD of Rs. 500/- alongwith Techno- Commercial Bid towards cost of Tender documents, failing which their offer will not be considered in any case. The sealed envelope should be super scribed with tender enquiry no. and due date. The quotation submitted shall be subject to our terms and conditions enclosed for the above services as per scope of work (Annexure-B). The tender shall reach us on or before the due date by hrs. The tender shall be opened at hrs. on the same day. A. Please submit your offer in two parts in separate sealed envelope as detailed below: 1. First Envelope must be superscripted as Techno Commercial Bid consisting of following: a. Instruction for Bidders (Annexure-A) b. Scope of work (Annexure-B) c. Unpriced bid (Annexure-C/1) d. Special Terms & Conditions (Annexure-D)

3 Instruction for Bidders Annexure A Your offer must be submitted in two bid system in sealed envelope super scripting each envelope with tender no., due date and contents. 1. The Bidder must have valid authorization from UP Pollution Control Board for collection, transportation, treatment, storage and disposal facilities. A photocopy of certificate of Authorization/ certificates should be attached with technical bid. 2. Unconditional acceptance of scope of work must be duly signed and stamped. (Annexure-B) 3. Duly filled checklist must be duly signed and stamped. (Annexure-G) 4. Your unpriced bid should be attached with unpriced techno- commercial bid duly signed as per Annexure C/1. No Price should be indicated in the techno commercial bid. In case it is found so, then the offer shall be summarily rejected. 5. List of 05 minimum clients where hazardous waste disposal have been under taken alongwith purchase order/ Job order/work order of last five years. 6. Health, Safety and Environment a. Do not carry out any work without clearance from concerned officer. b. The bidder shall necessary make gate entry pass from security for the manpower for job in advance duly completing all security formalities and be guided by the rules and regulations prevailing at SIL. c. Any vehicle/ equipments/ materials (both consumables and non consumables) shall be brought inside SIL premises with proper gate entry only and proper record shall be maintained. These gate entry records will be referred to while taking out after completion of jobs, failing which such material will not be permitted to be taken out. d. Safety of the persons if any deployed will be the responsibility of the contracting agency. Bidder will provide necessary safety guidance and PPE s to his person if any an also to SIL person during currency of contract. e. The Bidder should follow all guidelines issued by Central Government, State Government, Central/ State Pollution Control Boards/ MoEF from time to time for standards of performance for disposal process and are complied with. f. All provisions mentioned in the Hazardous wastes (Management, Handling and transboundry) Rules and any other applicable rules shall be strictly followed. g. The Bidder shall ensure that all due care is taken not to spill the wastes during transportation from SIL site and that the vehicle carrying the waste is fully covered. h. The Vehicle driver carrying the waste should have valid driving license and duly authorised by the party alongwith proper documents to enter at SIL premises. i. In case of any accident during transportation the party shall inform SIL immediately. And shall inform SPCB/CPCB where the accident has occurred apart from taking necessary immediate remedial action.

Manifest System (Movement Document to be used within country only) (i) The required copies of the manifest in Form 10 comprising of color code indicated below shall be arranged and signed by the transporter/ Authorised representative of the Bidder. Copy Number with Color code Purpose Copy 1 (White) To be forwarded by the occupier to the State Pollution Control Board Copy 2 (Yellow) To be carried by the occupier after taking signature on it from the transporter and the rest of the four copies to be carried by the transporter. Copy 3 (Pink) To be retained by the operator of the facility after signature Copy 4 (Orange) To be returned to the transporter by the operator of facility/recycler after accepting waste. Copy 5 (Green) To be returned by the operator of the facility to SPCB / Committee after treatment and disposal of wastes Copy 6 (Blue) To be returned by the operator of facility to the occupier after treatment and disposal of hazardous materials / wastes. (ii) (iii) (iv) (v) (vi) No transporter shall accept hazardous wastes from an occupier for transport unless it is accompanied by copies 3 to 6 of the manifest. The transporter shall submit copies 3 to 6 of the manifest duly signed with date to the operator of the facility alongwith with the waste consignment. Operator of the facility upon completion of treatment and disposal operations of the hazardous wastes shall forward copy 5 (Green) to the SPCB and copy 6 (Blue) to the occupier and the copy 3 (pink) shall be retained by the operator of the facility. All necessary forms mentioned above alongwith other forms as required time to time should be arranged by the successful Bidder. The final disposal certificate should be submitted to SIL immediately after disposal in the prescribed format in all respects for complying with the rules in force. 8. Liability of Tender (i) (ii) The Bidder shall be liable for all damages caused to the environment or third party due to improper handling of the hazardous wastes. The Bidder shall be liable to re-imbues any financial penalties as levied for any violates of the provisions under these rules by the SPC/CPCB.

5 Scope of Work Annexure B 1. Disposal of Wastes (i) Landfillable Waste (i.e. ETP/Phosphating / Grinding Sludge) (ii) Incineration Waste (i.e. Paint sludge, Cotton/Leather/Asbestos- gloves, leg guards, aprons, Hand sleeves etc.) 2. The Bidder should submit the detailed literature explaining the methodology of ultimate disposal (As per guideline of SPCB/CPCB) alongwith Techno-commercial Bid. 3. The Bidder should have valid authorization form UPPCB for collection, transportation, treatment, storage and disposal facilities. The Bidder must submit authorization certificates copy alongwith techno-commercial Bid. 4. The Bidder will provide vehicle at his own cost for lifting waste alongwith necessary documents. However labour and forklift shall be provided by SIL for loading hazardous waste. The responsibility of supervision will wholly rest with Bidder. 5. The party will dispose the wastes as per UPPCB/ CPCB guidelines. 6. The required copies of the manifest in Form-10 comprising of color code shall be arranged and signed by the Transporter/ authorised representatives of the Bidder. 7. Party will provide necessary safety guidance to the person during currency of contract. 8. Land fillable waste and incineration waste should be weighed separately. 9. Weighment of the waste will be done as per weighbridge reading at SIL vehicle dispatch gate. 10.The waste will be stored in plastic containers/ HDPE bags and shall be lifted in as is where is condition in the plastic container /HDPE bags. 11. Payment shall be released as per actual weighment and satisfactory completion of work alongwith submission of bills in duplicate alongwith receipt of certificates regarding disposal of Hazardous wastes material. 12. The working hours of SIL, Lucknow is from 8.00 hrs to hrs. 6 days a week except Sunday and Holiday. However the party shall inform well in advance of their arrival for lifting the waste for security formalities and should lift after concurrence from the concerned SIL official. 13. The vehicle driver should have valid driving license and vehicle documents to entre in SIL premises.

8 Special Terms and Conditions Annexure D 1. The Bidder should possess the valid authorization from UPPCB for collection, transportation, treatment, storage and disposal facilities. A Photocopy of certificate of authorization/certificates should be attached with techno-commercial bid, failing which their bid will automatically be treated as canceled. 2. Duration of the contract: SIL intend to award the contract to the successful bidder for a period of 2 years and there after this contract can be extended for another one year on mutual agreement. SIL reserves the right to foreclose the contract at any point of time by serving one month notice during currency of contract on the basis of unsatisfactory or poor performance. 3. The Quoted rate should be inclusive of collection, transportation, storage, treatment and disposal and all statutory levies, taxes (as applicable, please specify). The price variation is certain cases can be considered only on change of statutory levies if any subject to the submission of breakup of prices at the time of quotation provided that such adjustments are part of the bid accepted. 4. Payment shall be made as per actual weightment and satisfactory completion of work alongwith submission of bills in duplicate and also alongwith receipt of certificates of the disposal of hazardous waste as per UPPCB norms. 5. Bidder should have valid Income Tax Pan no. and Service registration no. (Photocopy to be enclosed). 6. The successful bidder shall collect the offered quarterly/ lot of hazardous waste from SIL or mutually decided subject to the availability of hazardous waste. 7. The bidder shall ensure that all due care is taken not to spill the waste during transportation from SIL site to disposal site and the vehicle carrying the waste is fully covered. 8. The Vehicle driver carrying the waste should have valid driving license with vehicle documents and duly authorised by the party alongwith proper documents to enter at SIL premises. 9. Liquidated Damages and penalty: If the successful bidder fails to lift and dispose the item within the agreed/ prescribed period, SIL may impose liquidated 0.5% for every week or part thereof for the portion of work executed beyond the agreed completion date subject to a maximum of 5.00% P.O. value.

14 Minimum Qualifying Criteria Annexure F 1. Bidder should agree to all the special terms and conditions (Annexure-D) and General Terms and Conditions (Annexure-E) singed and stamped on each page to be enclosed. 2. Bidder should sign integrity pact (Annexure-I) signed and stamped copy to be enclosed. 3. The Bidder should possess the valid authorization form UPPCB for collection, transportation, treatment, storage and disposal facilities. A photocopy of certificate of authorization/ certificates should be attached with techno- commercial bid. 4. Bidder should have valid Income Tax Pan No. and service registration. (Photo copy to be enclosed). 5. Bidder should have attached tender cost as per tender conditions specified. 6. List of 5 minimum clients where hazardous waste has been under taken alongwith copy of last five years P.O. / Job order/ work order. 7. The Bidder has to confirm the acceptance of Annexure- A and Annexure B.

15 Bid Evaluation Criteria Annexure G 1. Price Bid to only those successful bidders will be opened who fulfills minimum qualifying criteria as per Annexure -F. 2. Bid shall be evaluated on the basis of overall lowest (L-1) cost to SIL of qualified bidders.
