2 As a prospective OMSA member, here are some things you can do while working with ODOT for your authority. FORMS - Power of Attorney and Code of Ethics Complete & sign the Power of Attorney and Code of Ethics forms and mail or drop by our office along with a $75.00 check (non-refundable Power of Attorney filing fee) made out to OMSA. New Member OMSA200 Orientation & Membership Dues Once OMSA is advised that ODOT is prepared to issue you authority, you'll need to schedule an orientation meeting. This time also allows you time to ask questions about our tariff & moving issues. At that time, we'll collect your 1st Quarterly membership dues, which will activate your OMSA membership. We will then file a ODOT petition to add your company to the OMSA 200 tariff. Once ODOT approves the petition, you are authorized to legally operate as a Household Goods Carrier in the State of Oregon.

3 You will need the following two forms to conduct business in Oregon as a household mover: 1. Oregon HHG Bill of Lading 2. General Information Bulletin for Moving Household Goods in Oregon You should contact Willamette Traffic Bureau to purchase both the forms (Form 50 and 37H) or call for a custom Bill of Lading quote. 4 Our current tariff is available for download on our website front page and is split into five sections depicted below:: Section A Section P Section 1 Section 2 Section 3 Front & back end of the tariff Participation Section General Rules & accessorial charges Mileage Rates & provisions (including costs of packing materials and labor costs for packing/unpacking) Hourly Rates & provisions (including packing materials costs) for such areas as Portland, Eugene, Salem, Medford, Grants Pass, Klamath Falls, Astoria, Coos Bay, Bend, and Pendleton.

5 AUTHORTY TYPES 1A Intrastate property 1C Local Cartage 1G Other than Local Cartage 1B For HHG within exempt cities (incorporated and under 10,000 population). ODOT maintains the list of exempt cities. These movements are exempt from tariff rates and carriers are free to establish any rate they deem appropriate.

6 AUTHORITY: Local Cartage Service Local Cartage means the transportation of any class of property by motor vehicle for compensation when the transportation is performed wholly within an incorporated city or a commercial zone adjacent to an incorporated city. In general, only hourly rates in Section 3 are authorized using Local Cartage authority. Other than Local Cartage Service All other rates, except for hourly, are authorized under Other than Local Cartage authority, including mileage rates in Section 2. Tariff 200 Item 10 Tariff 200 Item 20 Tariff 200 Item 30 ORS Incorporated cities Local Cartage Service (authority listed by carrier) Other than Local Cartage Service (authority listed by carrier) Exempts moves within certain local cartage areas from economic regulation. These are cities under 10,000 in population. A carrier needs a 1B permit for such service; rates for such service is not regulated. Contact ODOT to confirm whether a city is exempt.

7 MILEAGE RATES (Item 2100)u apply only when Section 3 hourly rates or Expedited Rates (Item 2530) do not apply EXPEDITED MILEAGE RATES (Item 2530) apply when customer requires specific pickup & delivery dates on a mileage move. HOURLY RATES (Items ) Except for exempt cities, movements within incorporated cities or Commercial Zones (listed on Page P-1) are based on hourly rates. Each Section 3 item allows for rates to be used within a certain air-mile radius of a zone's limits or city's limits. For example, in item 3200, the Portland Commercial Zone allows for a 10 airmile radius beyond its zone limits. So a shipment from Portland to a point in another city whose city limits are within 10 miles of Portland city limits uses rates found in Items 3200, 3250; and Each Item lists the cities to/from which the item rates apply, with its own air-mile radius provisions. Commercial Zones also have their own specific air-mile radius provision (see Page P-1).

8 NOTE: For the Portland area, three items, 3200, 3250 and 3275 provide different rate applications Origin to Destination - Time starts upon arrival at Origin and ends at departure from Destination) 3250 Terminal to Terminal - Time starts when carrier's vehicle leaves its equipment point and ends when the equipment returns to its equipment point Terminal to Terminal - Application of rates is based on Origin & Destination zip code combinations. General information regarding Section 3 Items: Rates in Section 3 Items based on Origin to Destination: 3150 and Rates in all other Section 3 Items are based on Terminal to Terminal.

Operating outside carrier's authorized scope of certificated authority No cargo insurance at time of move, reference (2) and [$1000 penalty for each occurrence, under ORS (l){b), and not subject to level 1 penalty limitations If evidence that a vehicle was on the road] ORS Any charges to the shipper (customer) that are not as specified in the carrier's tariff Over charges Undercharges or rebates/discounts Discriminatory, preferential, prejudicial rates, rules or practices Failing to charge for replacement cost protection if shipper did not select a valuation option

Subject to suspension under ORS / OAR Failure to file Annual Report with the Department on or before April 1 of each year. ORS / OAR Failure to obtain and maintain criminal background checks on all subject individuals in the manner and time required. Allowing a subject individual with a criminal background described in this rule to perform duties restricted under this statute and rule.

13 Household Goods Transportation Rate Audits Complaint of Violations (continued) ORS Failing to include carrier's certificate number in print or internet advertising OAR Failure to keep a copy of the carrier's tariff readily available for inspection by the public at all reasonable times OAR Not keeping and maintaining complete and comprehensive records of all business transactions OAR Failing to prepare bill of lading with all necessary documentation OAR Carrier not maintaining records of receipts and disbursements as required.
