1 BOND TO BE EXECUTED FOR IN SERVICE POST GRADUATE STUDENTS AT THE TIME OF POST GRADUATE ADMISSION STAMP PAPER VALUE NOT LESS THAN Rs.200/- KNOW ALL MEN BY THESE PRESENTS THAT WE Dr S/o,D/o,W/o Aged about and resident of Door No Cross Main Post Taluk District of at present employed as in the department / office of the (herein after called the OBLIGOR) and (1) Dr [S/o,D/o,W/o at present employed as in the department / office of (herein after called Surety) do hereby jointly and severally bind ourselves and our respective heirs, executors and administrators to pay to Government of Karnataka (herein after called the Government ) on demand of sum of Rs (Rupees Only) together with interest there on from the date of demand of Government rates for the time being inforce on Government loans, AND TO GETHER with all costs between attorney and client and all charges and expenses that shall or may have been incurred by the Government. WHEREAS the obliger is granted deputation to higher studies/specialised training by Government and has been granted a seat for post graduate studies in AND WHEREAS for the better protection of the Government the obliger has agreed to execute this bond with such condition as hereunder written.

2 AND WHEREAS the aforesaid surety has agreed to execute this bond as surety on behalf of the above bounded Dr NOW THE CONDITION OF THE ABOVE WRITTEN OBLIGATIONS IS THAT The obligor Dr leaving the course by discontinuance or otherwise and thus failing to complete the course, the obligor and surety shall forthwith pay to the Government on demand the sum of Rs.5,00,000/-(Rupees Five Lakh FiftyThousand Only) in case of Post graduate degree course as a penalty and Rs.2,50,000/-(Rupees Two Lakh Fifty Thousand Only) in case of post graduate, diploma course as a penalty and also the obligor has liable to be debarred from applying to post graduate course for next three years in the event of leaving the course before its completion, the oblige and the surety shall forthwith pay to the Government or as may be directed by the Government on demand the said sum of Rs (Rupees Only) together with interest there on from the date of demand of Government rates for the time being in force on Government loans. It shall be lawful to the Government to make recovery of the amount from the obligor in case it is not possible to recover from the obligor, the obligor and the surety shall be jointly and severally liable to pay a sum of Rs as a penalty and the obligor to be debarred from applying to post graduate course for next three years.

3 And upon the obligor and or the surety afore said making such payment the above written obligation shall be void and of no effect, otherwise it shall be and remain in full force and virtue. PROVIDED ALWAYS that the liability of the surety here under shall not be impaired or discharged by reason of time being granted or by any forbearance act or omission of the government or any person, authorized by then (whether with or without the consent or knowledge of the surety) nor shall it be necessary for the Government to sue the obligor before suing the surety or any of them for amount due hereunder. THE BOND SHALL in all respects be governed by the laws of India for the time being in force and the rights and liabilities here under shall where necessary be accordingly determined by the appropriate courts in India. The Government of Karnataka have agreed to bear the stamp duty payable on this bond. 5 SERVICE BOND FOR ALL INDIA QUOTA & STATE QUOTA POST GRADUATE STUDENTS STAMP PAPER VALUE NOT LESS THAN Rs.100/- KNOW ALL MEN BY THESE PRESENTS THAT WE, Dr. D/o,s/o resident at at present PG student in (herein after called the obligor ) and (1) Shri. Designation Bangalore (herein after called surety) do hereby jointly and severally bind ourselves and our respective heirs execution and administrations to pay the Governor of Karnataka (herein after called the Government )on demand the sum of Rs (Rupees Only) for degree and Rs. (Rupees Only) together with interest thereon from the date of demand at Govt. rates for the time being in force of Government loans AND TOGETHER with all costs between attorney and client and all charges and expenses that shall or may have been incurred by the Government. That in consideration of the Government of Karnataka selecting vide order No. the post graduate course in during the academic year the post graduate student and his surety convenent with the Government as follows : 1.Candidates who are selecting Government Colleges and Government seats in Private Medical College (under concession fee) for the post graduate courses should furnish an undertaking that he/she should serve the Government for a minimum period of three years after completion of the course, if Government desires.

Candidates who are availed 100% tuition fee reimbursement from the Government should furnish an undertaking that he/she should serve the Government for a period of five years, if Government desires. 3. That in case the post graduate students, who fails to fulfil the above conditions, the surety of the post graduate student shall be jointly and separately liable for penalty of Rs. 5,00,000/- for degree and Rs. 3,00,000/- for diploma course after completion of the course as per Undertaking. 4. The post graduate student shall be bound by the rules governing the selection as per the Gazette Notification No. Bangalore dt:. 5. The post graduate student shall prosecute his/her studies deligently. 8 his/her post graduate study in together with interest thereon from the date of demand at Govt. rates for the time being in force of Government loans. In addition to the prescribed fine, every candidate shall pay the remaining period course fee on his own to the Government/private Colleges in the event of he/she leaving the course before its completion. PROVIDED always that the liability of the surety hereunder shall not be impaired or discharged by reason of time being granted or by any forbearance, act or omission of the Government of any person authorized by them (whether with or without the consent or knowledge of the surety) nor shall it be necessary for the Government to sue the obligor before suing the surety amounts due hereunder. 9 BOND TO BE EXECUTED FOR IN SERVICE POST GRADUATE STUDENTS AT THE TIME OF POST GRADUATE ADMISSION AFFIDAVIT STAMP PAPER VALUE NOT LESS THAN Rs.50/- I, S/o,D/o,W/o aged about ( ) years, residing at do hereby solemnly affirm on oath and states as under. I am one of the successful candidates for admission of the post graduate student in, in service quota for Government college on payment of Tuition fee Degree/Diploma a sum of Rs. (Rupees Only) as notified by Government of Karnataka in its letter G.O.No.AKUKA Dt:. I hereby undertake that, I shall pay a sum of Rs. (Rupees only) to the Government in case I take allotment order during counseling and fail to join the course. In the event of leaving the course before its completion, I shall pay to the Government the said sum of Rs. (Rupees only) as penalty. The amount equal to the fee for the remaining period of the course may be recovered from my salary. I may also be debarred for the next three from appearing for any entrance examinations.

10 In addition to the prescribed fine, every candidate shall pay the remaining period course fee on his own to the Government/Private Colleges in the event of he/she leaving the course before its completion. I shall serve the Government for a minimum period of ten(10) years, or till the attainment of superannuation, whichever is earlier I shall pay a penalty of Rs. (Rupees Only) if I fail to serve the Government after completion to the course. I understand that, such an undertaking is an absolute necessity while getting admission to Post Graduate Degree /Diploma Course. I hereby declare that the content of the affidavit and the undertaking therein is true and correct having been given out of my free will and no external pressure from any source including the authority of this college.
